Dtsch Med Wochenschr 2019; 144: 867\u2013875<\/strong><\/p><\/blockquote>\n<p><strong>Kurze Zusammenfassung:\u00a0<\/strong><\/p>\n<div class=\"page\" title=\"Page 1\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li>Rund 10 % der Notaufnahmepatienten besteht das Leitsymptom \u201eunklare Bewusst- seinsst\u00f6rung\u201c.<\/li>\n<li>Prognoseentscheidend ist ein z\u00fcgiges Management.<\/li>\n<li>Ziel ist eine mehrdimensionale strukturierte Vorgehensweise (Anamnese, klinischer Befund und Ergebnissen von Zusatzuntersuchungen).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Besonders bemerkenswert ist, dass Erbguth anf\u00fchrt, dass sich &#8222;Als ideale Logistik ein interdisziplin\u00e4r strukturiertes Vorgehen &#8230; im Rahmen eines konservativen \u201eSchockraum- Konzepts\u201c erweist. s. hierzu auch Post bei<strong><i>\u00a0<a href=\"http:\/\/www.news-papers.eu\" target=\"_blank\" rel=\"noopener\">news-papers.eu <\/a><\/i><\/strong>zum\u00a0<i><\/i><strong><a style=\"font-style: italic;\" href=\"http:\/\/news-papers.eu\/konservatives-nichttraumatologisches-schockraummanagement-2-0\" target=\"_blank\" rel=\"noopener\">konservativen Schockraummanagement<\/a><i>.\u00a0<\/i><\/strong>Dies auch unter Ber\u00fccksichtigung\u00a0dessen, dass &#8222;nur knapp \u00fcber die H\u00e4lfte der bewusstseinsgest\u00f6rten Patienten werden mit einer konkreten \u00e4tiologischen Einweisungsdiagnose eingeliefert werden, und hiervon wiederum nur knapp \u00fcber die H\u00e4lfte mit der letztendlich gestellten Diagnose \u00fcbereinstimmten&#8220;.<\/p>\n<\/div>\n<p><strong>Epidemiologie:<\/strong><\/p>\n<div class=\"page\" title=\"Page 1\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li>ca. 50 % der F\u00e4lle eine prim\u00e4r neurologische Ursache (meist zerebrovaskul\u00e4r),<\/li>\n<li>ca. 20 % metabolische St\u00f6rungen,<\/li>\n<li>ca. 15 % eine Intoxikation,<\/li>\n<li>ca. 10 % ein posthypoxisches Koma und<\/li>\n<li>5\u201310% psychiatrische Ursache<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Nicht-traumatische Bewusstseinsst\u00f6rung:<\/strong><\/p>\n<ul>\n<li><em>prim\u00e4r intrakranielle Erkran- kung (strukturell, funktionell)<\/em>\n<ul>\n<li>isch\u00e4misch Hirnstamm, kortikal nur bei gro\u00dfen oder bilateralen Isch\u00e4mien)<\/li>\n<li>intrakranielle Blutungen (parenchymat\u00f6s, subdural)<\/li>\n<li>entz\u00fcndlich (Erreger- oder autoimmunbedingt)<\/li>\n<li>epileptisch (konvulsiv, non-konvulsiv)<\/li>\n<li>liquorzirkulatorisch (Aufstau)<\/li>\n<li>degenerativ (selten akut\u00a0oder alleinige Ursache)<\/li>\n<\/ul>\n<\/li>\n<li><em>Grunderkrankung mit sekund\u00e4rer St\u00f6rung der Hirnfunktion<\/em>\n<ul>\n<li>metabolisch-endokrin inkl. St\u00f6rungen des Wasser- und Elektrolythaushaltes<\/li>\n<li>toxisch<\/li>\n<li>kardiozirkulatorisch<\/li>\n<li>hypoxisch<\/li>\n<li>septisch<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Das muss sich man sich wirklich fragen&#8230; :\u00a0<\/strong><\/p>\n<div class=\"page\" title=\"Page 5\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li>War tats\u00e4chlich kein Trauma im Spiel? Gibt es\u00a0Hinweise auf Fremd-\/oder Selbstverschulden?<\/li>\n<li>Bestehen erkl\u00e4rende andere\u00a0Vorerkrankungen (z. B. Herpes simplex), Hirnabszess<\/li>\n<li>postiktal (\u201eTodd\u2019sche Symptomatik\u201c)<\/li>\n<li>Hypoglyk\u00e4mie, hyperosmolares Coma diabeticum, ur\u00e4mische und hepatische Enzephalopathie<\/li>\n<li>vorbestehende Hirnsch\u00e4digung\/L\u00e4sion und\u00a0unabh\u00e4ngige Komaursache<\/li>\n<\/ul>\n<p><strong>Labor und Liqour:<\/strong><\/p>\n<div class=\"page\" title=\"Page 5\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li>Routine-Laboranalytik<\/li>\n<li>Erweiterungen um\n<ul>\n<li>metabolisch-endokrinen Parameter bzw.<\/li>\n<li>Parameter f\u00fcr elektrolytbedingten St\u00f6rungen#<\/li>\n<li>Intoxikationen<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Cave 1:<\/strong> Wernicke-Enzephalopathie bei Thiaminmangel kann nicht einfach &#8222;rasch&#8220; delektiert werden.
