LUCAS-Device) zur Unterst\u00fctzung im notfallmedizinischen Setting konnte bisher bei randomisierten Studien kein verbessertes neurologisches Behandlungsergebnis im Vergleich zur traditionellen manuellen Brustkorbkompression gezeigt werden. Damit r\u00fcckten Studien in den Vordergrund, die die Verletzungen nach beiden Reanimationsformen verglichen und hinsichtlich ihrer Intensit\u00e4t bewerteten.<\/p>\n<p style=\"text-align: justify;\">Aktuell haben Ondruschka et al. im forensischen Journal \u201e<strong>Forensic Science, Medicine and Pathology<\/strong>\u201c (<a href=\"https:\/\/link.springer.com\/article\/10.1007\/s12024-018-0024-5\">https:\/\/link.springer.com\/article\/10.1007\/s12024-018-0024-5<\/a>) die bisher gr\u00f6\u00dfte diesbez\u00fcgliche Obduktionskohorte analysieren und vergleichen k\u00f6nnen.<\/p>\n<blockquote><p><strong>Ondruschka B et al.\u00a0Chest compression-associated injuries in cardiac arrest patients treated with manual chest compressions versus automated chest compression devices (LUCAS II) \u2013 a forensic autopsy-based comparison.\u00a0<span class=\"jrnl\" title=\"Forensic science, medicine, and pathology\">Forensic Sci Med Pathol<\/span>. 2018 Sep 10. doi: 10.1007\/s12024-018-0024-5.<\/strong><\/p><\/blockquote>\n<p style=\"text-align: justify;\">Durch Auswertung aller rechtsmedizinischen Obduktionsprotokolle der Institute f\u00fcr Rechtsmedizin in Leipzig und Chemnitz zwischen 2011-2017 konnten 614 Obduktionsf\u00e4lle \u00a0analysiert werden (manuell vs. LUCAS: n=591 vs. n=113). Die Wahrscheinlichkeit von reanimationsbedingten Verletzungen steigt mit l\u00e4ngerer Reanimationsdauer. Im Vergleich zur manuellen Reanimation hatten Personen nach LUCAS-Reanimation jeweils unabh\u00e4ngig von anderen Faktoren signifikant h\u00e4ufiger<\/p>\n<ul>\n<li style=\"text-align: justify;\"><strong>Hautemphyseme (5 vs. 0%; p=0,012), <\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Pneumothoraces (6 vs. 1%, p=0,008), <\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Lungen- (19 vs. 4%, p=0,008), <\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Leberl\u00e4sionen (10 vs. 1%, p=0,001), und <\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Herzbeuteltamponaden (3 vs. 1%, p=0,025). <\/strong><\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Im Mittel ist hingegen die Anzahl an Rippenfrakturen nicht different zwischen beiden Reanimationsmethoden (6\u00b13 vs. 7\u00b12, p=0,09).Ein h\u00f6heres Lebensalter und l\u00e4ngere Reanimationsintervalle beeinflussen die H\u00e4ufung von kn\u00f6chernen Frakturen (p&lt;0,001).<\/p>\n<p><strong>Fazit f\u00fcr die Praxis:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify;\">In der untersuchten Kohorte mit frustran verlaufenen Reanimationsversuchen waren damit nach LUCAS-Reanimation signifikant mehr Verletzungen als nach manueller Reanimation festzustellen, aber die Intensit\u00e4t der Verletzungen war zwischen beiden Gruppen vergleichbar.<\/li>\n<li style=\"text-align: justify;\">Relevant erscheint, dass auch in dieser Studie <em>keine\u00a0<\/em>Reanimationsverletzung als unmittelbar todesurs\u00e4chlich eingestuft worden war, obgleich mehrere der festgestellten Verletzungen bei zun\u00e4chst erfolgreichem ROSC sicher als potentiell letal anzusehen sind.<\/li>\n<li style=\"text-align: justify;\">Im klinischen Alltag soll nach erfolgreichem ROSC so schnell wie klinisch m\u00f6glich ein CT-Scan zum Ausschluss bzw. Nachweis von CPR-assoziierten Verletzungen erfolgen, auch um diesbez\u00fcglich eine suffiziente Therapie initiieren zu k\u00f6nnen.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Nach der Einf\u00fchrung von automatischen Reanimationshilfen (z.B.
