Bei proximalen Gef\u00e4\u00dfverschl\u00fcssen konnte die \u00dcberlegenheit der Thrombektomie gegen\u00fcber der konventionellen Lyse-Therapie mit einer \u201enumber needed to treat\u201c (NNT) = 8 f\u00fcr ein gutes neurologisches Ergebnis gezeigt werden:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Church EW at al. Number needed to treat for stroke thrombectomy based on a systematic review and metananlysis. Clin Neurology Neurosurg 2017; 156:83-8<\/strong><\/p>\n<\/blockquote>\n<p style=\"text-align: justify;\">Damit ist die mechanische Thrombektomie \u00e4hnlich effektiv in der Vermeidung neurologischen Schadens wie die orale Antikoagulation von Patienten mit Vorhofflimmern.<\/p>\n<hr \/>\n<p style=\"text-align: justify;\">Zwei gro\u00dfe Studien der vergangenen Monate haben das Zeitfenster zur Thrombektomie auf bis zu 24 Stunden nach Symptombeginn ge\u00f6ffnet:<\/p>\n<blockquote><p><strong>Nogueira RG et al. Thrombectomy 6 to 24 hours after stroke with a mismatch between deficit and infarct. N Engl J Med 2018; 378:11-21<\/strong><\/p><\/blockquote>\n<ul style=\"text-align: justify;\">\n<li><em>\u201e\u2026 the rate of functional independence at 90 days was 49% in the thrombectomy group as compared with 13% in the control group (adjusted difference, 33 percentage points; 95% credible interval, 24 to 44; posterior probability of superiority, &gt;0.999).\u201c<\/em><\/li>\n<\/ul>\n<blockquote><p><strong>Albers GW et al. Thrombectomy for stroke at 6 to 16 hours with selection by perfusion imaging. N Engl J Med 2018; 378:1-11<\/strong><\/p><\/blockquote>\n<ul style=\"text-align: justify;\">\n<li><em>\u201eEndovascular therapy plus medical therapy, as compared with medical therapy alone, was associated with a favorable shift in the distribution of functional outcomes on the modified Rankin scale at 90 days odds ratio, 2.77; P&lt;0.001) and a higher percentage of patients who were functionally independent, defined as a score on the modified Rankin scale of 0 to 2 (45% vs. 17%, P&lt;0.001). The 90-day mortality rate was 14% in the endovascular-therapy group and 26% in the medical-therapy group (P = 0.05),\u2026\u201c<\/em><\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Dieses Verfahren wird aktuell nur in hochspezialisierten Zentren angeboten. Daher ist es von gro\u00dfer Bedeutung, bereits im pr\u00e4hospitalen Bereich, m\u00f6glichst viele Schlaganfall-Patienten zu erkennen, die f\u00fcr eine mechanische Thrombektomie qualifizieren, um sie direkt in ein entsprechendes Zentrum zu transportieren.<\/p>\n<hr \/>\n<p style=\"text-align: justify;\">In einer aktuell ver\u00f6ffentlichten Studie wurde nun die <strong>Los Angeles Motor Scale (LAMS)<\/strong> zum Erkennen von proximalen Gef\u00e4\u00dfverschl\u00fcssen (large vessel occlusion, LVO) erstmalig f\u00fcr die Anwendung im pr\u00e4hospitalen Bereich validiert und hinsichtlich der G\u00fctekriterien f\u00fcr die Vorhersage mit zahlreichen anderen Scores verglichen.<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Noorian AR et al. <\/strong><strong>Los Angeles Motor Scale to Identify Large Vessel Occlusion. Prehospital Validation and Comparison With Other Screens. Stroke 2018; 49:565-572<\/strong><\/p>\n<\/blockquote>\n<p style=\"text-align: justify;\">Der LAMS ist auch pr\u00e4hospital leicht zu bestimmen, da nur 3 motorische Qualit\u00e4ten evaluiert werden m\u00fcssen. Im innerklinischen Bereich kann ein LAMS \u22654 eine LVO mit einer Sensitivit\u00e4t von 81% und einer Spezifit\u00e4t von 89% voraussagen.<\/p>\n<table style=\"height: 559px;\" Blutung, 5% \u201eneurovascular mimic\u201c<\/li>\n<li style=\"text-align: justify;\">der LAMS wurde pr\u00e4hospital durch Paramedics erhoben<\/li>\n<li style=\"text-align: justify;\">f\u00fcr LAMS \u22654 gilt: Sensitivit\u00e4t 76%, Spezifit\u00e4t 65%, positiv pr\u00e4diktiver Wert 79%, negativ pr\u00e4diktiver Wert 61%<\/li>\n<li style=\"text-align: justify;\">damit hat dieser Score eine bessere oder zumindest vergleichbare G\u00fcte im Vergleich zu vielen andere, deutlich komplizierter zu erhebende Scores<\/li>\n<li style=\"text-align: justify;\">selbst der weit etablierte und f\u00fcr die Stroke-Versorgung zentrale NIHSS scheint in der Vorhersagequalit\u00e4t hinsichtlich einer LVO nicht \u00fcberlegen<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Fazit f\u00fcr die Praxis:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify;\">bei einem LAMS \u22654 kann nach den Ergebnissen dieser Untersuchung mit einer 79%igen Wahrscheinlichkeit von einer LVO ausgegangen werden.<\/li>\n<li style=\"text-align: justify;\">ein direkter Transport in ein Stroke-Zentrum mit endovaskul\u00e4rer Interventionsm\u00f6glichkeit w\u00e4re in diesem Fall
