Die Erstellung der Leitlinie erfolgte mit Unterst\u00fctzung der Deutschen Interdisziplin\u00e4ren Vereinigung f\u00fcr Intensiv- und Notfallmedizin (DIVI).<\/p>\n<p style=\"text-align: justify;\">Der Entwicklung neuerer Punktionssysteme musste hierbei ebenso Rechnung getragen werden, wie der Bewertung der optimalen Punktionsstellen und deren Alternativen.<\/p>\n<p style=\"text-align: justify;\">In der S1 Leitlinie enth\u00e4lt zahlreiche Empfehlungen, die einen ad\u00e4quaten Umgang in der Praxis unter h\u00f6chstm\u00f6glichen Sicherheitsaspekten erm\u00f6glichen sollen (<a href=\"http:\/\/www.awmf.org\/leitlinien\/detail\/ll\/001-042.html\" target=\"_blank\" rel=\"noopener\"><strong>Auszug<\/strong><\/a>):<\/p>\n<ul>\n<li style=\"text-align: justify;\"><strong>Nach aktuellem Stand k\u00f6nnen nahezu alle bei der Behandlung von\u00a0p\u00e4diatrischen und erwachsenen Notfallpatienten eingesetzten\u00a0intraven\u00f6sen Medikamente auch intraoss\u00e4r appliziert werden.\u00a0<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Die intraven\u00f6sen Medikamentendosierungen gelten damit auch f\u00fcr die\u00a0intraoss\u00e4re Applikation.<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Die korrekte klinische Indikation und die medizinische Notwendigkeit\u00a0einer intraoss\u00e4ren Infusion sollen sowohl im Kindes- als auch im\u00a0Erwachsenenalter in jedem Einzelfall gegeben sein. Die rein\u00a0\u201eprophylaktische\u201c Anlage einer intraoss\u00e4ren Infusion ohne unmittelbar\u00a0notwendige Medikamentenapplikation und \/ oder Infusionstherapie ist\u00a0nicht indiziert und deshalb zu unterlassen.<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Aufgrund der ab dem 6. Lebensjahr zunehmend dicker werdenden\u00a0Kortikalis der Tibia sollten bei \u00e4lteren Kindern entweder entsprechende\u00a0halbautomatische bzw. automatische Punktionssysteme verwendet\u00a0werden oder auf die genannten alternativen Punktionsstellen\u00a0ausgewichen werden (alle Systeme).<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Aus anatomischen Gr\u00fcnden (insbesondere Gefahr einer mediastinalen\u00a0Perforation) ist bei Kindern die sternale intraoss\u00e4re Punktion obsolet.\u00a0<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Bei erwachsenen Notfallpatienten wird die Punktion der proximalen\u00a0medialen Tibia auf H\u00f6he der Tuberositas tibiae als Insertionsort der\u00a0ersten Wahl empfohlen (halbautomatische und automatische Systeme).\u00a0<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Alternativ wird die Punktion an der distalen Tibia an der medialen Fl\u00e4che\u00a01-2 cm proximal des Malleolus medialis empfohlen (alle Systeme).<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Die Autoren empfehlen bei Verwendung von halbautomatischen\u00a0Punktionssystemen (z.B. EZ-IO\u00ae) und in allen Altersklassen als\u00a0Punktionsstelle der 1. Wahl grunds\u00e4tzlich die proximale mediale Tibia zu\u00a0w\u00e4hlen.<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Das Infusionssystem soll nicht direkt an die intraoss\u00e4re Kan\u00fcle\u00a0konnektiert, sondern eine kurze, auf der Haut pflasterfixierte\u00a0Schlauchleitung mit Dreiwegehahn dazwischengeschaltet werden.
