Die Kollegen untersuchten den Einfluss einer postintubationsbedingten Hyperoxie in einem kritische kranken Patientenkollektiv in der Notaufnahme:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Page N et al.,\u00a0Emergency department hyperoxia is associated with increased mortality in mechanically ventilated patients: a cohort study.\u00a0<span class=\"JournalTitle\">Critical Care<\/span><span class=\"ArticleCitation_Year\">2018<\/span><span class=\"ArticleCitation_Volume\">22:9 (<a href=\"https:\/\/ccforum.biomedcentral.com\/articles\/10.1186\/s13054-017-1926-4\" target=\"_blank\" rel=\"noopener\">PDF<\/a>)<\/span><\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">Die Kollegen untersuchten Patienten, die in der Notaufnahme endotracheal intubiert wurde und nachfolgend f\u00fcr mindestens 24 h auf einer Intensivstation beatmet wurden. Patienten, die bereits postintubationem in der Notaufnahme oder binnen 24 h auf der Intensivstation verstarben bzw. vor Ablauf von 24 h auf der Intensivstation extubiert wurden, wurden ausgeschlossen. Auf der Intensivstation mussten die Patienten konsequent normox\u00e4misch sein.<\/p>\n<p style=\"text-align: justify;\">Technisch gesehen war diese Beobachtungsstudie eine Datenbankanalyse eines gut vernetzten IT-Systems aus der Notaufnahme und der Intensivstation. Eine Normox\u00e4mie wurde ein paO2-Wert zwischen 60\u2013120 mmHg gewertet. Eine Hypox\u00e4mie lag bei Werten paO2 &lt;60 mmHg, und eine Hyperoxie bei paO2-Werten &gt;120 mmHg vor.<\/p>\n<p style=\"text-align: justify;\">Insgesamt wurden 688 Patienten in die Studie eingeschlossen. Eine Normox\u00e4mie in der Notaufnahme lag nach der Intubation bei 350 (51%) Patienten vor [Median (IQR) Notaufnahme paO2 88 mmHg (76\u2013101)]. 300 (43,6%) Patienten waren hyperox\u00e4misch [Median (IQR) Notaufnahme paO2 189 mmHg (146\u2013249)].<\/p>\n<p>Die <strong>28-Tage-Mortalit\u00e4tsraten<\/strong> waren wie folgt:<\/p>\n<ul>\n<li><strong>Hypox\u00e4mie: 5\/38, 13,2%<\/strong><\/li>\n<li><strong>Normox\u00e4mie: 68\/350, 19,4%<\/strong><\/li>\n<li><strong>Hyperox\u00e4mie: 89\/300, 29,7%<\/strong><\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Im Rahmen einer multivariablen logistischen Regression ware eine Hyperox\u00e4mie in der Notaufnahme ein unabh\u00e4ngiger Prediktor for die Krankenhausmortalit\u00e4t [adjusted OR 1,95 (1,34\u20132,85)].\u00a0<span style=\"text-align: justify;\">Aus Sicht der Autoren um Page k\u00f6nnte eine Hyperox\u00e4mie in der Notaufnahme nach Intubation in der Notaufnahme mit einer erh\u00f6hten Letalit\u00e4t f\u00fcr mechanisch beatmete Patienten die normox\u00e4misch auf der Intensivstation weiterbehandelt werden sein. Warum hypox\u00e4mische Patienten die niedrigste Letalit\u00e4tsrate in dieser Untersuchung aufwiesen wird von den Autoren in der Diskussion nicht diskutiert. Jedoch lassen die Ausschlussfaktoren darauf schliessen, dass eine sehr starke Selektion betrieben wurde.
