Und wie bekommt man als Kliniker diese dann vom Tisch, wenn schon der Begriff &#8222;Aortendissektion&#8220; gefallen ist? Neue Erkenntnisse zum Vorgehen liefert eine aktuelle Untersuchung aus Circulation:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Nazerian P et al.\u00a0Diagnostic Accuracy of the Aortic Dissection Detection Risk Score Plus D-Dimer\u00a0for Acute Aortic Syndromes:\u00a0The ADvISED Prospective Multicenter Study. Circulation 2017;\u00a010.1161\/CIRCULATIONAHA.117.029457\u00a0<a href=\"https:\/\/www.clinicaltrials.gov\/ct2\/show\/NCT02086136?term=NCT02086136&amp;rank=1\" target=\"_blank\" rel=\"noopener\">Clinical Trial Registration<\/a><\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">Die <strong>akute Aortendissektion<\/strong> ist eine sehr seltene aber lebensbedrohliche kardiovaskul\u00e4re Erkrankung mit h\u00e4ufig unspezifischen Symptomen. In diesem Kontext wird daher die Probleme <strong>Fehldiagnose<\/strong> und <strong>\u00dcberdiagnostik<\/strong> immer wieder kritisch diskutiert. D-Dimere werden in strukturierten Diagnosestrategien und Algorithmen zum Ausschluss einer Aortendissektion immer wieder eingesetzt, haben aber f\u00fcr sich alleine keinen abschliessenden Stellenwert. Die finale Fallzuordnung wurde nach umfassender Bildgebung, Autopsie, chirurgischer Versorgung oder einem 14-t\u00e4gigen Follow-up-Intervall vorgenommen.<\/p>\n<p style=\"text-align: justify;\">Insgesamt wurde <strong>1.850 Notaufnahme-Patienten<\/strong> analysiert. Insgesamt wiesen 438 (24%) Patienten 0 Punkte im Aortendissektions-Risikoscore auf, 1.071 (58%) Patienten wiesen 1 Punkt, und 341 (18%) Patienten mehr als 1 Punkt auf.<\/p>\n<p style=\"text-align: justify;\">Von allen untersuchten Patienten hatten letztendlich\u00a0<strong>241 (13%) eine Aortendissektion<\/strong>: 125 hatten eine Typ A-Dissektion, 53 eine Typ B-Dissektion und 35 wiesen ein intramurales H\u00e4matoma auf. 18 Patienten wiese eine Aortenruptur auf und 10 eine penetrierendes Aortenulkus.<\/p>\n<ul>\n<li>Ein <strong>positiver D-Dimer-Test<\/strong> hatte eine Sensitivit\u00e4t von 96,7% (95% CI: 93,6-98,6%) und eine Spezifit\u00e4t von 64% (95% CI 61,6-66,4%) f\u00fcr das Vorliegen einer Aortendissektion. Letztendlich hatten <strong>8 von 241 Patienten mit einer Aortendissektion eines negativen D-Diemer-Test (3,3%)<\/strong>.<\/li>\n<li>Bei 294 Patienten mit der Konstellation<br \/>\n<strong>&#8222;Aortendissektions-Risikoscore: 0 Punkte + negativen D-Dimer-Test&#8220;<\/strong><br \/>\nwurde eine Aortendissektion beobachtet (Versagensrate: 0,3%; 95% CI: 0,1-1,9%).<\/li>\n<li>Bei 924 Patienten mit der Konstellation<br \/>\n<strong>&#8222;Aortendissektions-Risikoscore \u00a0\u22641 Punkte + negativer D-Dimer-Test&#8220;\u00a0<\/strong><br \/>\nwurden 3 F\u00e4lle mit Aortendissektion beobachtet (Versagensrate: 0,3%;\u00a095% CI 0.1-1%).<\/li>\n<\/ul>\n<blockquote><p><strong>Letztendlich kann man zusammenfassen, dass <\/strong><\/p>\n<ul>\n<li><strong>mit der Rule-Out-Strategie &#8222;Aortendissektions-Risikoscore + D-Dimer-Test&#8220; 1 aus 300 Patienten \u00fcbersehen wird.\u00a0<\/strong><\/li>\n<li><strong>bei einem\b Aortendissektions-Risikoscore &gt;1 Punkt unabh\u00e4ngig vom D-Dimerewert eine weitere Diagnostik angestrengt werden muss.\u00a0<\/strong><\/li>\n<\/ul>\n<\/blockquote>\n<hr \/>\n<p><strong><a href=\"https:\/\/twitter.com\/news_papers_eu?refsrc=email&amp;s=11\" target=\"_blank\" rel=\"noopener\">News-Papers.eu auf TWITTER<\/a>\u00a0 \u00a0 \u00a0 \u00a0\u00a0<a href=\"http:\/\/news-papers.eu\/follow\" target=\"_blank\" rel=\"noopener\">Follow news-papers.eu<\/a><\/strong><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Immer wieder kommen wir in Notaufnahmen in die Verlegenheit eine Aortendissektion in die differentialdiagnostischen \u00dcberlegungen mit aufzunehmen.
