Internist 2017, online.<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p><strong>Myx\u00f6demkoma:<\/strong><\/p>\n<ul>\n<li>seltene Komplikation einer schweren Hypothyreose<\/li>\n<li>hohe Letalit\u00e4t von 20-25%<\/li>\n<li>&#8222;Koma&#8220; liegt dabei selten vor (misleading wording)<\/li>\n<li>Diagnose geh\u00e4uft in Wintermonaten gestellt (s. K\u00e4lteexposition als Ausl\u00f6ser)<\/li>\n<li>h\u00e4ufiger \u00e4ltere Frauen betroffen<\/li>\n<li><strong>Ursache:<\/strong>\n<ul>\n<li>zus\u00e4tzliche (dann ausl\u00f6sende parallel vorliegende) Erkrankung (z.B.\u00a0Infektionen, Herzversagen, zerebrovaskul\u00e4re\u00a0Ereignisse, Trauma, Operation) bei eine\u00a0meist schon l\u00e4nger\u00a0bestehende, unerkannte Hypothyreose)<\/li>\n<li>zus\u00e4tzliche Ausl\u00f6ser: K\u00e4lte, Narkotika (Opioide), Sedativa<\/li>\n<li>Ursache der zurgrundeliegenden Hypothyreose: Hashimoto-Thyreoditis, Strahlenthyreoiditis, 5% sekund\u00e4re Hypothyreose bei Hypophyseninsuffizienz, u.a.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Symptomatik:<\/strong>\n<ul>\n<li>Hypothermie<\/li>\n<li>neuropsychiatrische Symptome: Desorientiertheit, Halluzinationen, Depression, Vergesslichkeit bis Amnesie, Lethargie bis hin zum Koma sowie auch Kleinhirnsymptome (z.B. Ataxie, Adiadochokinese)<\/li>\n<li>Epilepsie<\/li>\n<li>Hyponatri\u00e4mie<\/li>\n<li><strong>klassisches Myx\u00f6dem<\/strong> (aufgedunsenes Gesicht, geschwollene H\u00e4nde, gro\u00dfe Zunge, trockene, raue, k\u00fchle Haut)<\/li>\n<li>kardiovaskul\u00e4re Symptome: verl\u00e4ngerte QT-Zeit, Sinusbradykardie, AV-Block, diastolische Hypertonie, ggf. Hypotonie bei verminderter myokardialer Kontraktilit\u00e4t, vermindertem Schlagvolumen und Herzzeitvolumen, Perikard- und Pleuraerg\u00fcss<\/li>\n<li>pulmonale Symptome: alveol\u00e4re Hypoventilation mit Hyperkapnie, Gefahr der Bronchopneumonie<\/li>\n<li>gastrointestinale Symptome: Obstipation bis hin zu paralytischem Ileus und Magenatonie<\/li>\n<li>cave: gleichzeitig auf ausl\u00f6sende Erkrankung achten (s.o.)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Diagnose:<\/strong><\/p>\n<ul>\n<li>V.a. Myx\u00f6demkoma bei Patienten, die sich mit klassischen neuropsychiatrischen Symptomen mit gleichzeitiger Hypothermie, Hyponatri\u00e4mie und\/oder Hyperkapnie pr\u00e4sentieren<\/li>\n<li>klinischer Hinweis auf ein Myx\u00f6demkoma bei eingeschr\u00e4nkter Ansprechbarkeit:\n<ul>\n<li>Thyreoidektomienarben<\/li>\n<li>Fremdanamnese\/Arztbriefe mit Radiojodtherapie oder Hypothyreose, meist im Rahmen einer chronischen lymphozyt\u00e4ren\u00a0Thyreoiditis<\/li>\n<\/ul>\n<\/li>\n<li>TSH erh\u00f6ht und fT4 erniedrigt, Cave: schilddr\u00fcsenspezifische Antik\u00f6rper sind in der Akutdiagnostik bedeutungslos<\/li>\n<li>weitere Laborwerte: Hyponatri\u00e4mie (bei 50% d. Patienten vorliegend &#8211;&gt; neuropsychiatrische Symptome), Hypoglyk\u00e4mie, LDH- und CK-Erh\u00f6hung<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Therapie:<\/strong><\/p>\n<ul>\n<li><strong>Schilddr\u00fcsenhormonsubstitution:<\/strong>\n<ul>\n<li>L-Thyroxin Startdosis 200\u2013400 \u03bcg i.v. am 1. Tag, Erhaltungsdosis 1,6 \u03bcg\/kg KG pro Tag entweder oral oder i.v. (Expertenmeinung und in amerikanischen Leitlinien beinhaltet)<\/li>\n<li>Ggf. zus\u00e4tzlich L-T3 Startdosis 5\u201320 \u03bcg i.v., Erhaltungstherapie 2,5\u201310 \u03bcg alle 8 h<\/li>\n<\/ul>\n<\/li>\n<li><strong>Glukokortikoidsubstitution:<\/strong>\n<ul>\n<li>Hydrocortison 200 mg\/Tag (Stressdosis)<\/li>\n<\/ul>\n<\/li>\n<li><strong>Supportive intensivmedizinischeMa\u00dfnahmen:<\/strong>\n<ul>\n<li>Intubation und Beatmung: m\u00f6glichst fr\u00fchzeitige Beatmung, unter Ber\u00fccksichtigung eines nicht zu schnellen Ausgleichs der Hyperkapnie (Ausgleich Hyperkapnie und Hypoxie, Therapie einer Bronchopneumonie &#8211;&gt; wenn vorliegt: Beginn einer Antibiose)<\/li>\n<li>Hypoglyk\u00e4mie: initial 50 ml einer 50%-igen Glukosel\u00f6sung i.v., dann weiter nach Blutglukosewerten<\/li>\n<li>Hyponatri\u00e4mie (Folge einer Wasserretention): alleinige Wasserrestriktion, kein schneller Ausgleich (Gefahr der pontinen Myelinolyse), also keine hochprozentige Natriuminfusionsl\u00f6sung notwendig (denn bei wiederhergestellter Schilddr\u00fcsenhormonwirkung an der\u00a0Niere wird Natrium wieder r\u00fcckresorbiert)<\/li>\n<li>Hypothermie:\n<ul>\n<li>bei &lt;31\u00b0C: langsame aktive Erw\u00e4rmung durch Dialyse, angew\u00e4rmte Infusionen etc., aber nicht mehr als 0,5 \u00b0C\/h<\/li>\n<li>bei &gt; 31 \u00b0C: nur passive Erw\u00e4rmung durch warme Decken<\/li>\n<\/ul>\n<\/li>\n<li>Hypotonie:\n<ul>\n<li>Hydrocortison 200 mg\/Tag \u00fcber die ersten Tage, danach langsam reduzieren<\/li>\n<li>Katecholamine und Digoxin sind weniger wirksam; es besteht eine erh\u00f6hte Gefahr von Herzrhythmusst\u00f6rungen<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li><strong>Behandlung der zugrunde liegenden Erkrankung\/ausl\u00f6senden Ursache<\/strong><\/li>\n<\/ul>\n<hr \/>\n<p>Zur <strong>Hypothyreose<\/strong> siehe auch diese Post:\u00a0<a href=\"http:\/\/news-papers.eu\/neues-zur-hypothyreose-schlimm-oder-nicht-schlimm\" target=\"_blank\" rel=\"noopener\"><strong>Schlimm oder nicht schlimm ?<\/strong><\/a><\/p>\n<hr \/>\n<h6 style=\"text-align: justify;\">F\u00fcr Angaben \u00fcber Dosierungsanweisungen und\u00a0Applikationsformen kann keine\u00a0Gew\u00e4hr \u00fcbernommen werden.
