(TEAM APPROACH)<\/li>\n<li>geschultes Personal zur Notfallkoniotomie sollte bei erwarteten schwierigen Atemweg oder Halsverletzungen vor Ort sein<\/li>\n<li>Ziel ist ein m\u00f6glichst hoher First-pass-success<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li><strong>Risiko einer Aspiration<\/strong>\n<ul>\n<li>deutlich erh\u00f6ht!<\/li>\n<li>alle Patienten sind aspirationsgef\u00e4hrdet und ggf. unkooperativ<\/li>\n<li>insbesondere bei begleitender Intoxikation, Sch\u00e4del-Hirntrauma, trauma-induzierter reduzierter Magenmotilit\u00e4t, Unkenntnis der letzten Nahrungsaufnahme<\/li>\n<li>pharnygeale Blutungen, Blutungen aus dem maxillofacialen Bereich, Fremdk\u00f6rper<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<ul>\n<li><strong>Indikationen f\u00fcr eine Sicht auf die Stimmbandebene und Maskenbeatmung ggf verschlechtert<\/li>\n<li>Medikamentenauswahl:\n<ul>\n<li>cave: viele Traumapatienten sind hypovol\u00e4m<\/li>\n<li>Auswahl sollte sich richten nach der Schaffung optimaler Intubationbedingungen unter m\u00f6glichst geringem Einfluss auf den h\u00e4modynamischen Zustand<\/li>\n<li>Gabe parallel zur Volumentherapie (wenn n\u00f6tig)<\/li>\n<li>Etomidate (Diskussion des Einflusses auf die Nebennierenrinde)<\/li>\n<li>Ketamin (\u00e4ltere Diskussion um die Eignung von Ketamin beim Sch\u00e4del-Hirntrauma ist nicht mehr aktuell)<\/li>\n<li>Propofol, Thiopental und Benzodiazepine sollen nur mit Vorsicht appliziert werden (s. hierzu auch die Sicht der <a href=\"http:\/\/news-papers.eu\/notfallnarkose-damit-die-narkose-nicht-zum-notfall-wird\" target=\"_blank\" rel=\"noopener\"><strong>S1 Leitlinie Notfallnarkose<\/strong><\/a>, die von dieser Stellungnahme deutlich abweicht)<\/li>\n<li>Succinylcholin und Rocuronium je nach klinischer Situation (unter Beachtung der Nebenwirkungen)<\/li>\n<\/ul>\n<\/li>\n<li>Vor Narkoseeinleitung:\n<ul>\n<li>Pr\u00e4oxygenierung<\/li>\n<li>Antizipiere die vorliegenden Verletzungen<\/li>\n<li>Beachte die physiologische Situation (H\u00e4modynamik, Oxygenierung)<\/li>\n<li>Vorbereitung eines Plan B mit anderen Intubationsinstrumenten oder Techniken (ggf. fiberoptische Wachintubation)<\/li>\n<li>Evaluiere den schwierigen
