British Journal of Anaesthesia, 2017 (x): 1\u201315 (<a href=\"https:\/\/academic.oup.com\/bja\/article-abstract\/doi\/10.1093\/bja\/aex228\/4098748\/Videolaryngoscopy-versus-direct-laryngoscopy-for?redirectedFrom=fulltext\" target=\"_blank\" rel=\"noopener\">Link<\/a>)<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">In dieser systematischen \u00dcbersichtsarbeit und Metaanalyse wurde die Versagensrate und die Komplikationen bei erwachsenen Patienten verglichen. Dazu wurden Studien auf\u00a0CENTRAL, MEDLINE, Embase und clinicaltrials.gov gesucht und ausgewertet. Randomisierte kontrollierte Untersuchungen (RCT), die direkte und indirekte Videolaryngoksopie miteinander verglichen, wurden ausgewertet. Aus 64 Studien aus der Allgemeinan\u00e4sthesie wurden\u00a07.044 Patienten inkludiert.<\/p>\n<p><strong>Die Videolaryngoskopie:<\/strong><\/p>\n<ul>\n<li>reduziert ein Intubationsversagen (Odds Ratio (OR): 0,35, 95% Confidence Interval (CI): 0,19-0,65)<\/li>\n<li>auch bei Patienten mit antizipiertem schwierigen Atemweg (OR: 0,28, 95% CI: 0,15-0,55)<\/li>\n<li>insbesondere bei erfahrenen Anwendern (OR: 0,32, 95% CI: 0,13-0,75)<\/li>\n<li>reduziert Laryngeal-\/Atemwegstrauma (OR: 0,68, 95% CI: 0,48-0,96)<\/li>\n<li>und Heiserkeit (OR: 0,57, 95% CI: 0,36-0,88).<\/li>\n<li>verbessert die Einstellbarkeit der Stimmbandebene (OR: 6,77, 95% CI: 4,17-10,98)<\/li>\n<li>und reduzierte erschwerte Einstellbarkeiten der Stimmbandebene (OR: 0,18, 95% CI: 0,13-0,27)<\/li>\n<li>und schwierige Intubation (OR: 7,13, 95% CI: 3,12-16,31)<\/li>\n<\/ul>\n<p>Es fand sich <strong>keine Vorteile der Videolaryngoskopie<\/strong> f\u00fcr:<\/p>\n<ul>\n<li>die Anzahl der erfolgreichen ersten Intubationsversuche<\/li>\n<li>Rate an Hypoxie oder Mortalit\u00e4t (aber nur wenige Studien beinhalteten das Gesamtoutcome)<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Limitationen weist die Studie nat\u00fcrlich f\u00fcr die Notfallmedizin diesbez\u00fcglich auf, dass keine Studien an Notfallpatienten eingeschlossen wurden. Vorteile weist die Studie jedoch vor dem Hintergrund auf, dass einzelne Videolaryngoskope untersucht wurden, und so auch der CMAC Macintosh-like Spaten (der sowohl eine direkte als auch indirekte Videolaryngoskopie erm\u00f6glicht) und dieser gegen\u00fcber\u00a0GlideScope, Pentax or McGrath Series 5 das einzige Videolaryngoskopiesystem war, dass mit einer Reduktion an Intubationsversagen assoziiert war. Dar\u00fcber hinaus zeigt die Studie sehr gut, dass bei eine gleichwertigen Erfahrung zwischen direkten und indirekten Videolaryngoskopie, die Videolyrngoksopie Vorteile zeigte. Bestanden keine gleichwertigen Erfahrungen zwischen direkter und Videolaryngoskopie so ergab sich keine Vorteil f\u00fcr den Anwender.
