Negovsky in der ersten Ausgabe von RESUSCITATION 1972 formulierte: <strong>&#8222;The second step in resuscitation &#8211; the treatment of the \u00b4post-resuscitation disease\u00b4&#8220;<\/strong><\/em> (<a href=\"http:\/\/www.resuscitationjournal.com\/article\/0300-9572(72)90058-5\/abstract\" target=\"_blank\">Resuscitation 1972; 1: 1-7<\/a> und <a href=\"http:\/\/www.resuscitationjournal.com\/article\/S0300-9572(12)00380-2\/abstract\" target=\"_blank\">Resuscitation 2012; 83: 1187-1190<\/a>).<\/p>\n<p style=\"text-align: justify;\">Die Etablierung eines Spontankreislauf bei einer kardiopulmonalen Reanimation ist also nur die halbe Miete. Wichtig ist daher die strukturierte weitere Versorgung des erfolgreich reanimierten Patienten sowohl in der Pr\u00e4klinik, der Notaufnahme und der Intensivstation. Hierzu hat das German Resuscitation Council nun strukturierte Empfehlungen zur Vorgaben f\u00fcr Cardiac Arrest Center (CAC) erarbeitet:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Scholz, KH et al. Qualit\u00e4tsindikatoren und strukturelle Voraussetzungen f\u00fcr\u00a0Cardiac-Arrest-Zentren \u2013Deutscher Rat f\u00fcr Wiederbelebung \/\u00a0German Resuscitation Council (GRC).\u00a0Notfall Rettungsmedizin 2017 (<a href=\"http:\/\/download.springer.com\/static\/pdf\/662\/art%253A10.1007%252Fs10049-017-0288-4.pdf?originUrl=http%3A%2F%2Flink.springer.com%2Farticle%2F10.1007%2Fs10049-017-0288-4&amp;token2=exp=1491040099~acl=%2Fstatic%2Fpdf%2F662%2Fart%25253A10.1007%25252Fs10049-017-0288-4.pdf%3ForiginUrl%3Dhttp%253A%252F%252Flink.springer.com%252Farticle%252F10.1007%252Fs10049-017-0288-4*~hmac=bc3ba792ba9690c6bb8648b28f3bb8f8c00ef16852c959a162ac15602bb34acd\" target=\"_blank\">Open Access, PDF<\/a>)<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p><strong>Hier ein paar der Inhalte der GRC-Empfehlungen:<\/strong><\/p>\n<ul>\n<li>pr\u00e4hospital werden j\u00e4hrlich rund 75.000 Patienten reanimiert<\/li>\n<li>von diesen erreichen 40% einen Spontankreislauf (ROSC)<\/li>\n<li>60% der Ursachen eines Herzkreislaufstillstandes sind Folge eines akuten Koronarproblems<\/li>\n<li>bei STEMI AKutversorgung der Koronarien binnen 90-120 min<\/li>\n<li>Cardiac Arrest Center (CAC) m\u00fcssen eine standardisierte Postreanimationsbehandlung garantieren<\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #800000;\"><strong>Allgemeine Voraussetzungen eines CAC:<\/strong><\/span><\/p>\n<ul>\n<li>Vorhandensein folgender Fachdisziplinen: interventionelle Kardiologie,\u00a0Anaesthesiologie, Neurologie, Unfallchirurgie (nur in den Zentren, in denen\u00a0auch Traumapatienten aufgenommen werden)<\/li>\n<li>Umfangreiche technische Ausr\u00fcstung (Herzkatheter-Labor, CT-R\u00f6ntgen,\u00a0Echokardiografie einschlie\u00dflich TEE, hochwertige Beatmungsger\u00e4te,\u00a0Dialyseger\u00e4t)<\/li>\n<li>Standardisierte Behandlungspfade, wie sie z.B. f\u00fcr Patienten mit ST-Hebungsinfarkt\u00a0und\/oder Traumapatienten existieren<\/li>\n<li>Standardisierte Protokollierung der zeitlichen Abl\u00e4ufe<\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #800000;\"><strong>Strukturqualit\u00e4t:<\/strong><\/span><\/p>\n<ul>\n<li>24\/7 Verf\u00fcgbarkeit einer geeigneten Notaufnahmeinrichtung\n<ul>\n<li>Schockraum, Reanimationsraum, Notaufnahmeraum einer ICU, Herzkatheterlabor<\/li>\n<\/ul>\n<\/li>\n<li>24\/7 Verf\u00fcgbarkeit eines Herzkatheterlabors\n<ul>\n<li>24-h Rufbereitschaft (&#8230; unmittelbare Durchf\u00fchrung)<\/li>\n<li>20 min nach Alarmierung soll das Personal im Herzkatheterlabor verf\u00fcgbar sein<\/li>\n<li>Ablaufprotokolle f\u00fcr STEMI\/NSTEMI, inkl. Dokumentation der zeitlichen Abl\u00e4ufe<\/li>\n<li>M\u00f6glichkeit der Direkt\u00fcbernahme im Herzkatheterlabor<\/li>\n<li>mind. vier erfahrene Interventionalisten und eines erfahren Assistenzteams mit Nachweis intensivmedizinischer Ausbildung und Nachweis eines ALS-Kurses (Arzt) und ILS (Pflegepersonal)<\/li>\n<\/ul>\n<\/li>\n<li>24\/7 Verf\u00fcgbarkeit der Notfallsonographie<\/li>\n<li>24\/7 Notfallr\u00f6ntgen und Computertomographie<\/li>\n<li>24\/7 ICU-Platz mit Temperaturmanagement\n<ul>\n<li>pro Schicht soll ein ICU-Arzt ALS zertifiziert sein und eine Person aus dem Pflegepersonal ILS zertifiziert<\/li>\n<li>Personal soll bez\u00fcglich des Temperaturmanagement geschult sein<\/li>\n<\/ul>\n<\/li>\n<li>24\/7 Verf\u00fcgbarkeit eines fach-neurologische Dienstes<\/li>\n<li>regelm\u00e4ssige Qualit\u00e4tszirkel zur Reanimationsversorgung\n<ul>\n<li>2-4 mal\/Jahr<\/li>\n<li>Analyse der Prozess- und Ergebnisqualit\u00e4t und Einzelfallanalyse<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<p><strong><span style=\"color: #800000;\">Prozessqualit\u00e4t<\/span><\/strong><\/p>\n<ul>\n<li>Fokus auf interdisziplin\u00e4re und unterprofessionelle Standard Operating Procedures<\/li>\n<li>SOP &#8222;\u00dcbernahme nach pr\u00e4hospitaler Reanimation durch definiertes Cardiac-Arrest-Team&#8220;<\/li>\n<li>SOP &#8222;Schnittstellenkommunikation mit dem Rettungsdienst&#8220;\n<ul>\n<li>inkl. Die Pr\u00e4sidien der Deutschen Gesellschaft f\u00fcr An- \u00e4sthesiologie und Intensivmedizin (DGAI), der Deutschen Gesellschaft f\u00fcr Kardiologie, Herz- und Kreislaufforschung (DGK), der Deutschen Gesellschaft f\u00fcr Internistische Intensiv- und Notfallmedizin (DGIIN) und des GRC haben die- sem Konsensuspapier in der vorliegenden Form zugestimmt.
