Intensive Care Med 2017;\u00a0DOI 10.1007\/s00134-017-4739-7<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p><strong>Hier ein paar der angef\u00fchrten Punkte:<\/strong><\/p>\n<ul>\n<li>Forschungsinhalte der letzten 15 Jahre waren:\n<ul>\n<li>Fokussierung auf die Optimierung des neurologischen Behandlungsergebnis<\/li>\n<li>Fokussierung auf Notfallkoronarangiographie mit Intervention &#8211; laufende Studien:\n<ul>\n<li><a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02309151?term=NCT02309151&amp;rank=1\" target=\"_blank\" rel=\"noopener noreferrer\">DISCO study (NCT02309151<\/a>)<\/li>\n<\/ul>\n<ul>\n<li><a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02387398?term=NCT02387398&amp;rank=1\" target=\"_blank\" rel=\"noopener noreferrer\">PEARL study (NCT02387398<\/a>)<\/li>\n<li><a href=\"http:\/\/www.isrctn.com\/ISRCTN96585404\" target=\"_blank\" rel=\"noopener noreferrer\">ARREST trial (ISRCTN9658540<\/a>4)<\/li>\n<li><a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02876458\" target=\"_blank\" rel=\"noopener noreferrer\">EMERGE trial (NCT02876458)<\/a><\/li>\n<\/ul>\n<\/li>\n<li>Fokussierung auf das Temperaturmanagement (Target Temperature Management, TTM) -aktuell laufende Studien:\n<ul>\n<li><a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02908308\" target=\"_blank\" rel=\"noopener noreferrer\">NCT02908308<\/a>,<\/li>\n<li><a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT01689077\" target=\"_blank\" rel=\"noopener noreferrer\">TTH48,\u00a0study (NCT01689077<\/a>)<\/li>\n<\/ul>\n<\/li>\n<li>Fokussierung auf eine vorsichtige Prognose beim Patienten mit \u00fcberlebten Herzkreislaufstillstand<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<p style=\"text-align: justify;\"><strong>Wichtige Punkte in der Forschung zum Herzkreisstillstand und Umsetzung im pr\u00e4hospitalen Umfeld waren:<\/strong><\/p>\n<ul>\n<li>Fr\u00fcherkennung des pr\u00e4hospitalen Herzkreisluafstillstandes<\/li>\n<li>Unterweisung von Laien und von Leitstellenmitarbeitern (Telefonreanimation)<\/li>\n<li>Fokussierung auf die zwei wesentlichen Fragen bei Annahme des Notrufs in der Leitstelle:\n<ul>\n<li>&#8222;Ist der Patient bei Bewusstsein?&#8220;<\/li>\n<li>&#8222;Atmet der Patient normal ?&#8220;<\/li>\n<li>Der Beginn einer Laienreanimation und assoziierte Verletzung werden als wenig relevant angesehen (bei einem Patienten mit Kreislauf) im Vergleich zur Bedeutung der Laienreanimation bei einem Herzkreislaufstillstand<\/li>\n<\/ul>\n<\/li>\n<li>Kurze Aktiverungsphase des Rettungsdienstes<\/li>\n<li>Kurzes Anfahrtsintervall des Rettungsdienstes<\/li>\n<li>Fokussierung auf kurze Unterbrechungen vor Defirbillationeen (pre-shock pauses) und tiefe Thoraxkompressionen<\/li>\n<li>Fokussierung auf AED-Programme f\u00fcr die \u00d6fftlichkeit und &#8222;Mobile Retter&#8220;-Systeme (z.B. SMS an registrierte Ersthelfer)<\/li>\n<\/ul>\n<hr \/>\n<p style=\"text-align: justify;\"><strong>Wichtige Punkte in der Forschung zum innenklinischen Herzkreisstillstand\u00a0und Umsetzung im innerklinischen\u00a0Umfeld waren:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify;\">Rapid Response Teams \/ Medical Emergency Teams zur Verhinderung von\n<ul>\n<li style=\"text-align: justify;\">ungeplanten ICU-Aufnahmen<\/li>\n<li style=\"text-align: justify;\">innerklinischen Herzkreislaufstillst\u00e4nden<\/li>\n<li style=\"text-align: justify;\">innerklinischen Todesf\u00e4llen<\/li>\n<\/ul>\n<\/li>\n<li>Fokussierung auf Ausbildung, Training und konzeptionelle Strukturierung<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Wichtige Punkte in der Forschung zum Herzkreisstillstand aus technischer Sicht:<\/strong><\/p>\n<ul>\n<li>Messung der Qualit\u00e4t der Reanimationsbem\u00fchungen\n<ul>\n<li>minimale pre-shock Pausen<\/li>\n<li>maximale Thoraxkompressionen-Fraktion (CCF) und Thoraxkompressionstiefe<\/li>\n<li>Vorteile durch invasive Messungen (arterielle Druckkurve), jedoch vorwiegend nur \u00a0innerklinisch verf\u00fcgbar, Ziel ist ein diastolischer Blutdruck &gt; 25 mmHg (bei Erwachsenen)<\/li>\n<li>Vorteil durch etCO2 (Kapnographie) mit dem Ziel eines etCO2 &gt;20 mmHg\n<ul>\n<li>aktuell laufende Studie: TAME study: RCT mit vergleich einer milden Hyperkapnie (PaCO2 50\u201355 mmHg) vs. Normokapnie (PaCO2 35\u201345 mmHg)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Wichtige Punkte in der Forschung zur Postresuscitation Care:<\/strong><\/p>\n<ul>\n<li>klar strukturiertes Vorgehen mit Koronarangiographie\/-intervention, TTM und Prognostizierung<\/li>\n<li>zur TTM fand sich zuletzt Unterschiede in der Zieltemperatur von 33\u00b0 und 36\u00b0C<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Unklare Forschungsergebnisse:\u00a0<\/strong><\/p>\n<ul>\n<li><strong>Automatische externe Reanimationshilfen zur Thoraxkompression (ACCD)<\/strong>\n<ul>\n<li>kein Vorteil von ACCD im Vergleich zur manuellen Thoraxkompression in RCT mit 12.000 Patienten f\u00fcr den pr\u00e4hospitalen Herzkreislaufstillstand (LINC, PARAMEDIC, CIRC)<\/li>\n<li>Vorteil von ACCD im Vergleich zu manuellen Thoraxkompression in 3 RTC und 6 Observationsstudien bei 689 Patienten f\u00fcr Kurzzeit- und Langzeit\u00fcberleben<\/li>\n<\/ul>\n<\/li>\n<li><strong>Adrenalin bei der kardiopulmonalen Reanimation<\/strong>\n<ul>\n<li>Vieldiskutierter Therapieansatz, aktuell kein sicherer Nachweis eines besseren Langzeitoutcomes<\/li>\n<li>aktuelle laufende Studie: <a href=\"http:\/\/www.isrctn.com\/ISRCTN73485024\" target=\"_blank\" rel=\"noopener noreferrer\">PARAMEDIC-2 study nicht-beobachteten Herzkreislaufstillstand, Ursache unklar<\/li>\n<\/ul>\n<\/li>\n<li><strong>E-CPR (ECMO zur Reanimation)<\/strong>\n<ul>\n<li>positive Ergebnisse in Subgruppen an Patienten<\/li>\n<li>aktuell noch widerspr\u00fcchliche Ergebnisse in den verf\u00fcgbaren Studien<\/li>\n<li>es laufen noch aktuelle\u00a0<a
