H\u00e4ufig wird hier ein Stufenkonzept gefordert: Erst Sonographie, dann ggf. konventionelles R\u00f6ntgen und letztendlich vielleicht ein CT. <strong>Naciye S. Gezer und Kollegen<\/strong> aus der T\u00fcrkei untersuchten nun den Mehrwert eines Thorax-CT im Vergleich zu einer konventionellen R\u00f6ntgenaufnahme des Brustkorbs bei nicht-traumatologischen Patienten:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Gezer NS et al. \u00a0Utility of chest CT after a chest X-ray in patients presenting to the emergency\u00a0department with non-traumatic thoracic emergencies. Am J Emerg Med 2017;\u00a0http:\/\/dx.doi.org\/10.1016\/j.ajem.2016.12.058<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">Gezer et al. untersuchten 500 konsekutive Patienten mit einer nicht-traumatologischen Erkrankungsursache, die sowohl einen R\u00f6ntgenthorax als auch ein Thorax-CT binnen 12 h in der Notaufnahme erhielten. Um einen Konsens in der Auswertung des Bildmaterials zu erhalten, wurde beide Bildhebungen (R\u00f6ntgenthorax und Thorax-CT) von zwei Radiologen unabh\u00e4ngig voneinander \u00a0untersucht.<\/p>\n<p style=\"text-align: justify;\">Vergleichbare Befunde zwischen beiden Bildgebungen ergaben sich in 49,2%. In 35,4% ergab sich eine \u00c4nderung der Hauptdiagnose und des weiteren Managements nach Durchf\u00fchrung des Thorax-CTs.<\/p>\n<p style=\"text-align: justify;\">Befunde im Vergleich\u00a0R\u00f6ntgenthorax vs. CT-Thorax:<\/p>\n<ul>\n<li>Pleuraerguss:\u00a0n=133 vs. n=168<\/li>\n<li>Lungen\u00f6dem: n=120 vs. n=\u00a0110<\/li>\n<li>pneumonische Infiltrate: n=\u00a099\u00a0vs.\u00a0205<\/li>\n<li>Pneumothorax: n=5 vs. n=6<\/li>\n<li>Multiple Lymphknoten: n=4 vs.\u00a0n=10<\/li>\n<li>Rippenfrakturen: n=2 vs.\u00a0n=6<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Die Autoren schlussfolgerten, dass ein Thorax-CT eine ad\u00e4quate Untersuchungstechnik f\u00fcr Patienten in der Notaufnahme sein w\u00fcrde. Vergessen werden darf an dieser Stelle aber auch nicht die Strahlenbelastung. Somit steht der Notfallmediziner in dem Dilemma zwischen besserer Diskriminierung einer Erkrankung und dem m\u00f6glichen produzieren Schaden (im Langzeitverlauf) durch die h\u00f6here CT-assoziierte Strahlenbelastung. An dieser Stelle sollte die Wertigkeit und die rasche Verf\u00fcgbarkeit der thorakalen Sonographie auch nicht ausser Acht gelassen werden.
