Ende 2016 wurde hier eine Metaanalyse zum Intubationerfolg der <strong>Videolaryngoskopie vs. direkte Laryngoksopie<\/strong> publiziert:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Lewis S et al. Videolarymgoscopy versus direct laryngoscopy for adult patients requiering tracheal intubation.\u00a0Cochrane Database of Systematic Reviews 2016, Issue 11. Art. No.: CD011136.\u00a0DOI: 10.1002\/14651858.CD011136.pub2.<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">Das prim\u00e4re Ziel der vorliegenden Cochrane Analyse war es zu eruieren, ob die Videolaryngoskopie im Rahmen einer Allgemeinan\u00e4sthesie das Risiko f\u00fcr Komplikationen und Intubationsversagen im Vergleich zur\u00a0direkten Laryngoskopie reduziert. Dar\u00fcber hinaus sollte evaluiert werden, ob Vorteile bez\u00fcglich bestimmter Patientengruppen (z.B. \u00a0adip\u00f6se Patienten, Patienten mit erwarteten oder bekannten schwierigen Atemweg) bestehen.<\/p>\n<p style=\"text-align: justify;\">Die Autoren f\u00fchrten eine Literatursuche innerhalb des\u00a0Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE und Embase bis zum 10.02.2015 durch. Alle RCT (randomisierte kontrollierte Studien) und quasi-randomisierte Studien erwachsener Patienten, die mittels Videolaryngoskop vs. Macintosh Laryngoskop intubiert wurden (inkl. im innenklinischen, notfallmedizinischen und pr\u00e4hospitalen Setting) ber\u00fccksichtigt.<\/p>\n<p style=\"text-align: justify;\">Insgesamt wurden 64 Studien bis 2015 mit insgesamt 7.044 erwachsenen Patienten erfasst. Hier die Hauptergebnisse zugunsten der Videolaryngoskopie im Vergleich zur direkten Laryngoskopie:<\/p>\n<ul>\n<li><strong>signifikant weniger Intubationsversagen<\/strong> (OR: 0,35, 95% CI: 0,19 -0,65; n=4.127 )<\/li>\n<li><strong>signifikant weniger Intubationsversagen bei Patienten mit erwarteten schwierigen Atemweg<\/strong>\u00a0(OR: 0,28, 95% CI: 0,15-0,55, n=830).<\/li>\n<li><strong>signifikant weniger Intubationsversagen bei Patienten mit simulierten schwierigen Atemweg<\/strong> (OR: \u00a00,18, 95% CI: 0,04-0,77; n=810)<\/li>\n<li><strong>signifikant weniger Laryngeal- und Airwaytraumata<\/strong> (OR: 0,68, 95% CI: 0,48-0,96, n=3.110)<\/li>\n<li><strong>signifikant bessere Einsteilbarkeit der Stimmbandebene<\/strong> (OR: 6,77, 95% CI: 4,17-10,98, n=2240)<\/li>\n<li><strong>signifikant weniger Intubationsversuche ohne\u00a0Einsteilbarkeit der Glottis<\/strong> (OR: 0,18, 95% CI: 0,13-0,27, n=2.240)<\/li>\n<li><strong>signifikant einfachere Nutzbarkeit<\/strong> (OR:\u00a07,13, 95% CI: 3,12-16,31, n=568)<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Kein Unterschied zwischen Videolaryngoskopie und\u00a0direkter Laryngoskopie fand sich hinsichtlich:<\/p>\n<ul>\n<li>der <strong>Hypoxierate<\/strong> (OR: 0,39, 95% CI: 0,10-1,44, n=1.319)<\/li>\n<li>der <strong>Mortalit\u00e4rsrate<\/strong> (OR: 1,09, 95% CI: 0,65-1,82, n=663)<\/li>\n<li>des <strong>First-Pass intubation Success<\/strong> (OR: 1,27, 95% CI: 0,77-2,09, n=4,731)<\/li>\n<li>der <strong>Rate an mehr als einem Intubationsversuch<\/strong><\/li>\n<\/ul>\n<hr \/>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Die Autoren schlussfolgerten aus ihrer Analyse, dass die\u00a0Videolaryngoskopie die Anzahl der misslungenen Intubationen reduzieren vermag und dies insbesondere bei Patienten mit schwierigen Atemweg. Die\u00a0Videolaryngoskopie verbessert die Sicht auf die Stimmbandebene und reduziert Laryngeal- und Atemwegtraumen.
