Unfallchirurg 2017;\u00a0DOI 10.1007\/s00113-016-0287-9\u00a0<\/strong><\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">Die kardiopulmonale Reanimation beim traumatisch\u00a0bedingten Herz-Kreislauf-Stillstand (tCPR) wurde\u00a0lange Zeit als nicht erfolgversprechend\u00a0angesehen. Aktuelle Daten zeigen, dass\u00a0die Erfolgsaussicht einer tCPR jedoch nicht schlechter\u00a0als bei Reanimation aufgrund kardialer\u00a0Ursache sein muss. Durch zielgerichtete und\u00a0rasche Behebung reversibler Ursachen kann\u00a0die Kreislauffunktion wieder hergestellt\u00a0werden. Pre-hospital- Emergency-Resuscitative-Thoracotomy(PERT)-Kurs)<\/li>\n<li>Vorhaltung des Equipments<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li><strong>Clamshell-Thorakotomie:<\/strong>\n<ul>\n<li>ben\u00f6tigtes Equipment: Skalpell, chirurgische Schere, Kleiderschere<\/li>\n<li>Start mit Minithorakotomie 5. Intercostalraum in der mittleren Axilarlinie (B\u00fclau-Position)<\/li>\n<li>Schnitt mit dem Skalpell durch Haut und Subkutangewebe \u00a0bis auf Interkostalmuskulatur mit Verbindung \u00fcber das Sternum beider Thoraxseiten<\/li>\n<li>Durchtrennen mit der Kleiderschere der Intercostalmuskulatur und des Sternmus<\/li>\n<li>dann \u00f6ffnen des Thoraxraumes muschelf\u00f6rmig (&#8222;Aufklappen der Motorhaube&#8220;)<\/li>\n<li>durch diese Massnahme ist\/kann folgendes erfolgen\n<ul>\n<li>Entlastung eines Spannungspneumothorax<\/li>\n<li>Perikardeinschnitt und Entlastung einer Perikardtamponade und manuelle interne Herzmassage<\/li>\n<li>ggf.
