J Cardiothorac Vasc Anesth 2016. (<strong>J Cardiothorac Vasc Anesth 2016)<\/strong><\/h6>\n<p><strong>Versorgung im Zentrum:<\/strong><\/p>\n<ul>\n<li>Bei schwerer h\u00e4modynamischer Instabilit\u00e4t sofort in den OP<\/li>\n<li>Meist noch ausreichend Zeit f\u00fcr CT-Diagnostik<\/li>\n<li>Weiter permissive Hypotension<\/li>\n<li>Fl\u00fcssigkeit in 250 ml-Bolus, um einen systolischen Blutdruck von 70 mmHg zu halten<\/li>\n<\/ul>\n<p><strong>Notfallnarkose:<\/strong><\/p>\n<ul>\n<li>Narkoseeinleitung erst nach Abwaschen, sterilem Abdecken und mit \u201eeingewaschenem\u201c Operateur am Tisch<\/li>\n<li>Invasive Blutdruckmessung falls zeitlich m\u00f6glich vor Narkoseeinleitung, sollte aber den OP-Beginn nicht verz\u00f6gern<\/li>\n<li>Gro\u00dflumiger zentralven\u00f6ser Zugang nach Narkoseeinleitung<\/li>\n<li>Wenn operativ eine endovaskul\u00e4re Versorgung (EVAR) geplant ist (nur in 65% der F\u00e4lle anatomisch \u00fcberhaupt m\u00f6glich), profitieren Patienten m\u00f6glicherweise von einer Lokal- (oder Regional-)An\u00e4sthesie im Vergleich zur Vollnarkose (randomisiert kontrollierte Studien zum Vergleich der Narkoseverfahren bei EVAR fehlen noch)<\/li>\n<li>Hohes Risiko an perioperativen kardialen Komplikationen<\/li>\n<li>Nach Klemmen: Normalisierung der H\u00e4modynamik anstreben (\u201eAuff\u00fcllen\u201c)<\/li>\n<li>H\u00e4modynamik evaluieren via MAD, ZVD, pulse pressure variation oder andere nicht-invasive Verfahren (Anm.:\u00a0trans\u00f6sophageale Echokardiografie)<\/li>\n<li>An die Komplikation einer Aortendissektion denken: Bedeutung der trans\u00f6sophagealen Echokardiografie<\/li>\n<\/ul>\n<p><strong>Weiteres Management, intraoperativ:<\/strong><\/p>\n<ul>\n<li><strong>Thrombelastografie<\/strong> wertvoll: Hyerfibrinolyse? Hyperkoagulopathie? Hypofibrinogen\u00e4mie?<\/li>\n<li>In England: <strong>Transfusionstrigger<\/strong> bei H\u00e4morrhagie 80 g\/l (=8,0g\/dl), Ziel dann 80-100 g\/l (8-10 g\/dl)<\/li>\n<li><strong>Normothermie<\/strong>, aber: nur durch W\u00e4rmen der Infusions-\/Transfusionsfl\u00fcssigkeit und W\u00e4rmen der oberen K\u00f6rperh\u00e4lfte! Keine W\u00e4rmematten oder W\u00e4rmen der Beine (Isch\u00e4mieschaden!)<\/li>\n<li><strong>Nephroprotektion:<\/strong> keine medikament\u00f6se Option bisher.
