Gerade an Feiertagen, Freitags und am Wochenende und vor allem Nachts kommt dieses Patientenkollektiv immer wieder in der Notaufnahme zur Aufnahme.<\/p>\n<p style=\"text-align: justify;\">Die <strong>PURE-(Prospective Urban Rural Epidemiological)-Studie<\/strong> aus <strong>Lancet\u00a0<\/strong>(<em>Smyth A, et al. Alcohol consumption and cardiovascular disease, cancer, injury, admission to hospital, and mortality: a prospective cohort study.\u00a0<strong>Lancet<\/strong>\u00a02015;\u00a0386: 1945-1954<\/em>) mit 114.970 erwachsenen Patienten zeigte, dass ein hoher Alkoholkonsum mit einem deutlich erh\u00f6hten Letalit\u00e4tsrisiko und dem Auftreten von Tumorerkrankungen bzw. Traumata assoziiert ist.<\/p>\n<p style=\"text-align: justify;\">Nun ist gerade in der Fachzeitschrift <strong>Notfall &amp; Rettungsmedizin<\/strong> ein aktuelles Themenheft zum <strong>Alkoholpatienten<\/strong> erschienen. Die publizierten Beitr\u00e4ge sind absolut lesenswert und ungemein tiefgr\u00fcndig:<\/p>\n<blockquote><p><strong>Hans FP, et al. Akute Alkoholintoxikation. Vorgehen bei Erwachsenen und Jugendlichen. Notfall Rettungsmed 2016; 19: 12-21<\/strong><\/p><\/blockquote>\n<p>Kurz ein paar Zahlen:<\/p>\n<ul>\n<li><strong>1,9 Mio.<\/strong> Menschen sind aktuell in Deutschland alkoholabh\u00e4ngig<\/li>\n<li><strong>1,6 Mio.<\/strong> leben zus\u00e4tzlich einen sch\u00e4dlichen Gebrauch von Alkohol aus<\/li>\n<li><strong>74.000<\/strong> Menschen kommen j\u00e4hrlich in Deutschland durch Alkoholkonsum zu Tode<\/li>\n<li><strong>mind.\u00a030 Mrd. Euro <\/strong>sind die j\u00e4hrlich assoziierten Kosten f\u00fcr das Gesundheitssystem<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Mit Alkoholintoxikation ist hier der Konsum von Ethylalkohol gemeint, zu differenzieren sind die viel selteneren Vergiftungen mit Methanol und Ethylenglycol. Ethylalkohol (C2H5OH) ist hydrophil und hepato-\/neuro- und embryotoxisch. Das Maximum des Blutalkoholwertes wird auf n\u00fcchternen Magen nach 30-90 min erreicht. Resorption erfolgt vorwiegend im Magen (70%), weniger im Duodenum (25%). Am Ethanolalkoholabbau ist haupts\u00e4chlich das Enzym Alkoholdehydrogenase (ADH) beteiligt. Die Eliminiationsrate (cave: weisse Mitteleurop\u00e4er) betr\u00e4gt 0,1 g\/kgKG\/h. Nebenwirkungen: Hypoglyk\u00e4mie (selten), (Pseudo)-Hyponatri\u00e4mien durch verminderte ADH-Aussch\u00fcttung bedingte Natriurese, Hypomagnes\u00e4mie. Stadieneinteilung der Alkoholvergiftung (cave: unterschiedliche Auspr\u00e4gung je nach Gew\u00f6hnungsgrad). <strong><a href=\"http:\/\/psychiatrietogo.de\/2015\/03\/20\/neulich-im-gericht-promille-mgl-gl-gkg-oder-wie-betrunken-war-der-angeklagte-denn-nun\/\" target=\"_blank\">Umrechnung<\/a>\u00a0<\/strong>(Blutalkoholkonzentration = BAK)<strong>: [BAK Ethanol] in\u00a0g\/l = [BAK Ethanol] in Promille x 1,23 oder [BAK Ethanol] in Promille = [BAK Ethanol] in g\/dl:1,23 <\/strong>(Annahme: BAK im Serum bestimmt). Beim Sturztrinken (&#8222;binge drinking&#8220;\/Rauschtrinken: Aufnahme gro\u00dfer Ethylakoholmengen in kurzer Zeit) tritt das \u00fcbliche Unwohlsein, das physiologisch zum Sistieren der Alkoholaufnahme f\u00fchrt, nicht mehr auf und so folgt der Ingestion ein komat\u00f6ser Zustand.<\/p>\n<p><strong>Notfallmedizinische Aspekte:<\/strong><\/p>\n<ul>\n<li>WWW-Fragen (Was, Wieviel, Wann) und\u00a0SAMPLE- Schema<\/li>\n<li>\u00dcberwachung (RR, Hfr, SpO2, Temperatur), inkl. Kontrolle nach Standardschema\/Zeitintervall<\/li>\n<li>Diagnostik (k\u00f6rperlicher Untersuchung inkl. Entkleidung, cave: Fahndung nach Begleitverletzungen)<\/li>\n<li>Laborabnahme [BAK, Blutgasanalyse (Elektrolyte, Glucose, Laktat und Gase),\u00a0Toxscreening (zB Benzodiazepine, Barbiturate, Trizyklika), kleines Blutbild, Gerinnung-, Leberwerte, Creatinkinase, CRP]<\/li>\n<li>bei Hinweise auf Sturzfolge und Vigilanzst\u00f6rung: CCT bzw.<br \/>\nbei Prellmarken an Extremit\u00e4ten etc.: entsprechende konventionelle radiologische Diagnostik<\/li>\n<li>ggf. Tetanusauffrischung<\/li>\n<li>bodennahe Lagerung<\/li>\n<li>ggf. Vitamin B1\/Thiamin-Substitution<\/li>\n<li>Entlassmanagement<\/li>\n<\/ul>\n<p><strong>Cave:<\/strong> Aktivkohle hat bei reiner Ethylalkoholingestion keine Wirkung!<\/p>\n<p>Das genaue Gegenteil der Alkoholintoxikation ist das <strong>Alkoholentzugssyndrom<\/strong>, gekennzeichnet durch <strong>Delir, Krampfanfall und weitere kardiopulmonale Komplikationen<\/strong>. Delirformen sind in 29% hypoaktiv, in 21% hyperaktiv und in 43% ein Mischbild, in 7% zeigen sich keine psychomotorische Ver\u00e4nderungen.<\/p>\n<blockquote><p><strong>Merke: Differentialdiagnose des Delirs: \u00a0I \u00a0W A T C H \u00a0 D E A T H<\/strong><\/p><\/blockquote>\n<p>Mit der Thematik <strong>Alkoholentzugssyndrom<\/strong>\u00a0besch\u00e4ftigt sich der nachfolgende Artikel der Serie eingehend:<\/p>\n<blockquote><p><strong>Haas G.
