Eine Metaanalyse vergleicht nun die direkte Laryngoskopie mit der Videolaryngoskopie im Hinblick auf die Erfolgsrate (im Sinne eines First Pass Success &#8211; als Intubationserfolg bei ersten Versuch) bei der pr\u00e4hospitalen orotrachealen Intubation.<\/p>\n<div class=\"csl-bib-body\" style=\"text-align: justify;\">\n<blockquote>\n<div class=\"csl-entry\">Kent ME, Sciavolino BM, Blickley ZJ, Pasichow SH<\/div>\n<div class=\"csl-entry\">Video Laryngoscopy versus Direct Laryngoscopy for Orotracheal Intubation in the Out-of-Hospital Environment: A Systematic Review and Meta-Analysis<\/div>\n<div class=\"csl-entry\"><a href=\"https:\/\/doi.org\/10.1080\/10903127.2023.2219727\" target=\"_blank\" rel=\"noopener\"><i>Prehospital Emergency Care<\/i> <i>28:<\/i> 221\u2013230 (2024)\u00a0<\/a><\/div>\n<\/blockquote>\n<\/div>\n<p style=\"text-align: justify;\"><strong>Methodik<\/strong><\/p>\n<p style=\"text-align: justify;\">Die Datenbanken MEDLINE, Embase und Cochrane wurden bis Januar 2023 durchsucht. Eingeschlossen wurden pr\u00e4hospitale Studien, die Videolaryngoskopie und direkte Laryngoskopie hinsichtlich des Erfolgs beim ersten Versuch oder des Gesamterfolgs der Intubation verglichen.<\/p>\n<p style=\"text-align: justify;\">Prim\u00e4rer Endpunkt dieser Metaanalyse war der Erfolg beim ersten Versuch, stratifiziert nach Anwendertyp und Geometrie des Laryngoskopspatels.<\/p>\n<p style=\"text-align: justify;\">Sekund\u00e4re Endpunkte waren der Gesamterfolg der Intubation sowie die Intubationsdauer.<\/p>\n<p style=\"text-align: justify;\"><strong>Ergebnisse<\/strong><\/p>\n<p style=\"text-align: justify;\">25 Studien mit insgesamt 35.489 Intubationen erf\u00fcllten die Einschlusskriterien; 5 Studien mit \u00c4rzten, 6 Studien mit spezialisiertem Rettungsfachpersonal oder Pflegekr\u00e4ften (Critical Care Paramedics\/Registered Nurses) und 10 Studien mit Rettungsfachpersonal (Paramedics). Ein Problem bestand durch die erhebliche Heterogenit\u00e4t der eingeschlossenen Studien (&gt;75 %).<\/p>\n<p style=\"text-align: justify;\">Hinsichtlich des prim\u00e4ren Endpunkts war die Videolaryngoskopie mit einer h\u00f6heren Erfolgsrate beim ersten Versuch assoziiert in: 3 von 5 Studien mit \u00c4rzten, 4 von 6 Studien mit spezialisiertem Rettungsfachpersonal oder Pflegekr\u00e4ften und 7 von 10 Studien mit Rettungsfachpersonal.<\/p>\n<p style=\"text-align: justify;\">Videolaryngoskope mit Macintosh-Spateln waren in 7 von 10 Studien mit einem besseren Erfolg beim ersten Versuch verbunden, w\u00e4hrend Ger\u00e4te mit hyperangulierter Geometrie in 3 von 7 Studien einen besseren Erfolg beim ersten Versuch zeigten. Der Gesamterfolg der Intubation war bei Anwendung der Videolaryngoskopie durch \u00c4rzte h\u00f6her in 2 von 6 Studien und in der Anwendung durch Rettungsfachpersonal in 9 von 10 Studien. Bei Rettungsfachpersonal\/Pflegekr\u00e4ften zeigte sich kein Zusammenhang zwischen Videolaryngoskopie und Gesamterfolg der Intubation (OR = 1,89; 0,96\u20133,72; I\u00b2 = 34 %).<\/p>\n<p style=\"text-align: justify;\">Schlie\u00dflich ergaben 4 von 5 Studien, dass die Videolaryngoskopie mit l\u00e4ngeren Intubationszeiten einherging.<\/p>\n<p style=\"text-align: justify;\"><strong>Schlussfolgerungen<\/strong><\/p>\n<p style=\"text-align: justify;\">Die Autoren beschreiben eine erhebliche Heterogenit\u00e4t in den pr\u00e4hospitalen Studien, welche die Videolaryngoskopie und die direkte Laryngoskopie vergleichen, hinsichtlich des Erfolgs beim ersten Versuch, des Gesamterfolgs oder der Intubationsdauer. Diese Heterogenit\u00e4t lie\u00df sich weder durch Stratifizierung nach Studiendesign, Anwendertyp oder Spatelgeometrie erkl\u00e4ren. Die Mehrheit der Studien zeigte, dass die Videolaryngoskopie in allen Untergruppen mit einer h\u00f6heren Erfolgsrate beim ersten Versuch einherging; hinsichtlich des Gesamterfolgs galt dies jedoch nur f\u00fcr Rettungsfachpersonal, nicht f\u00fcr \u00c4rzte.<\/p>\n<p style=\"text-align: justify;\">Diese Verbesserung trat h\u00e4ufiger in Studien auf, in denen Macintosh-Spatel verwendet wurden, als in solchen, bei denen hyperangulierte Spatel zum Einsatz kamen.<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Bewertung<\/strong><\/p>\n<p>Die Videolaryngoskopie zeigt in vielen Untersuchungen eine Verbesserung des First-Pass-Success, wenn dieser vorher mit der direkten Laryngoskopie gering war &#8211; also bei weniger routinierten Anwendern.
