Nun wurde ganz aktuell eine gro\u00dfe randomisierte Studie im JAMA ver\u00f6ffentlicht.<\/p>\n<p style=\"text-align: justify\">Diese kann \u201eopen access\u201c unter folgender DOI aufgerufen werden: 10.1001\/jama.2021.22002<\/p>\n<blockquote><p><strong>Driver BE et al. <\/strong><strong>Effect of Use of a Bougie vs Endotracheal Tube With Stylet on Successful Intubation on the First Attempt Among Critically Ill Patients Undergoing Tracheal Intubation. <\/strong><strong>A Randomized Clinical Trial. JAMA 2021<\/strong><\/p><\/blockquote>\n<p style=\"text-align: justify\">Die Studie kurz zusammengefasst:<\/p>\n<p style=\"text-align: justify\"><strong>Methodik:<\/strong><\/p>\n<ul style=\"text-align: justify\">\n<li>randomisiert kontrollierte Studie<\/li>\n<li>7 Notaufnahmen und 8 Intensivstationen in den USA<\/li>\n<li>556 Patienten wurden mit Hilfe eines Bougie intubiert, 546 unter Nutzung eines F\u00fchrungsstabes (\u201eStylet\u201c)<\/li>\n<li>Untersuchungszeitraum: April 2019 bis Februar 2021 (pausiert w\u00e4hrend der Corona-Pandemie in den USA von Februar 2020 bis August 2020)<\/li>\n<li>Einschlusskriterien: alle notfallm\u00e4\u00dfigen Intubationen mit Sedierung unter Einsatz eines nicht-hyperangulierten Spatels (Macintosh oder Miller)<\/li>\n<li>Ausschlusskriterien: Schwangerschaft, Einsatz eines hyperangulierten Spatels etc.<\/li>\n<li>prim\u00e4rer Outcomeparameter: first pass success-Rate (FPS)<\/li>\n<li>sekund\u00e4rer Outcomeparamter: schwere Hypox\u00e4mie (Abfall der Sauerstoffs\u00e4ttigung &lt;80% bis 2 min nach der Intubation)<\/li>\n<\/ul>\n<p style=\"text-align: justify\"><strong>Ergebnisse:<\/strong><\/p>\n<ul style=\"text-align: justify\">\n<li>99,6% der eingeschlossenen Patienten konnten evaluiert werden<\/li>\n<li>FPS mit Bougie: 80,4%, mit Stylet 83,0% (keine Signifikanz)<\/li>\n<li>schwere Hypox\u00e4mie: 11% in der Bougie-Gruppe, 8,8% in der Stylet-Gruppe (keine Signifikanz)<\/li>\n<li>\u00f6sophageale Fehlintubation: 0,7% mit Bougie, 0,9% mit Stylet (keine Signifikanz)<\/li>\n<li>Verletzungen im Mund, Rachen, Kehlkopf oder Thorax: 0 mit Bougie, 0,5% mit Stylet (keine Signifikanz)<\/li>\n<li>Zeit von Narkose-Einleitung bis Intubation im Median: 124 s (Interquartilenabstand: 97-180 s) mit Bougie, 112 s (85-157) mit Stylet (statistisch signifikant!)<\/li>\n<\/ul>\n<p style=\"text-align: justify\"><strong>Schlussfolgerung:<\/strong><\/p>\n<ul style=\"text-align: justify\">\n<li>die Autoren schlussfolgern aus ihren Ergebnissen, dass der Einsatz eines Bougie zur notfallm\u00e4\u00dfigen trachealen Intubation im Vergleich zum F\u00fchrungsstab keinen Vorteil bringt<\/li>\n<\/ul>\n<p style=\"text-align: justify\"><strong>Fazit f\u00fcr die Praxis:<\/strong><\/p>\n<p style=\"text-align: justify\">Bringt der Bougie also wirklich nichts? Leider beantwortet auch diese Studie die Frage nicht wirklich. Auch wenn es mit Sicherheit die beste Studie ist, die bisher zu diesem Thema existiert, kann man die Ergebnisse nicht verallgemeinern. Die Intubierenden (zu 62% Assistenz\u00e4rzte, zu 33% Fach\u00e4rzte) hatten eine sehr geringe Erfahrung in der trachealen Intubation. Leider findet man diese Information nur im Supplement. Im Median hatten sie nur 60 mal zuvor einen Patienten tracheal intubiert (Interquartilenabstand 35-100), mit Bougie hatten sie zuvor sogar nur im Median 10 mal intubiert (IQR 4-20). Dass mit dieser geringen Erfahrung in der Bougie-assistierten Intubation, das Device einen m\u00f6glichen Vorteil nicht \u201eausspielen\u201c kann, ist eigentlich zu erwarten. Auch war in der Studie der Bougie nicht mit dem Tubus \u201evorgeladen\u201c, sondern wurde \u201esolo\u201c eingef\u00fchrt und dann der Tubus durch eine Assistenzperson eingef\u00e4delt. Dies erkl\u00e4rt m\u00f6glicherweise die 12 Sekunden l\u00e4ngere Intubationsdauer mit Bougie.<\/p>\n<p style=\"text-align: justify\">Wenn das gesamte Team ausreichend Erfahrung in der Bougie-assistierten Intubation hat und beispielsweise der Bougie im Standard vorgesehen wird, kann der Bougie eventuell doch Vorteile bieten. Dies legt zumindest eine weitere aktuelle Studie zum Thema Bougie aus den USA nahe:<\/p>\n<blockquote>\n<p style=\"text-align: justify\"><strong>Latimer AJ et al.
