Computertomografie\/Kernspintomografie bei unzureichendem sonografischen Befund)<\/li>\n<li style=\"text-align: justify;\">Einteilung in eine <strong>unkomplizierte oder komplizierte Appendizitis<\/strong><\/li>\n<li style=\"text-align: justify;\"><strong>Therapieentscheidung<\/strong> (operativ\/konservativ) entsprechenden klinisches Gesamtbild und Risikofaktoren<\/li>\n<li style=\"text-align: justify;\"><strong>Appendektomie ist die Therapie der Wahl<\/strong> in allen Altersgruppen.<\/li>\n<li style=\"text-align: justify;\">Die komplizierte <strong>Appendizitis<\/strong> ist ein schwerwiegendes Krankheitsbild und kann potenziell konservativ (antibiotisch +\/- Drainagenanlage) als Alternative zur operativen Therapie behandelt werden.<\/li>\n<\/ul>\n<\/div>\n<div class=\"page\" title=\"Page 1\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\" style=\"text-align: justify;\">\n<p><strong>Inzidenz:\u00a0<\/strong>100 Neuerkrankungen pro 100 000 Einwohner\/Jahr, und damit die h\u00e4ufigste Ursache des akuten Abdomens<\/p>\n<\/div>\n<div class=\"page\" title=\"Page 1\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Risiko:\u00a0<\/strong>Appendektomie ist ein Eingriff mit sehr geringem Operationsrisiko und die Morbidit\u00e4t und Mortalit\u00e4t werden haupts\u00e4chlich durch den Schweregrad der Appendizitis selbst und Komorbidit\u00e4ten bestimmt.<\/p>\n<div class=\"page\" title=\"Page 2\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\" style=\"text-align: justify;\">\n<p><strong>Datenlage: <\/strong>keine Leitlinie in Deutschland, international wurden bis heute drei Leitlinien zur\u00a0akuten Appendizitis publiziert.<\/p>\n<\/div>\n<div class=\"page\" title=\"Page 2\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Klinische Pr\u00e4sentation:\u00a0<\/strong>enorm im\u00a0Ausma\u00df der klinischen Pr\u00e4sentation, daher als Cham\u00e4leon der Chirurgie bekannt.<\/p>\n<div class=\"page\" style=\"text-align: justify;\" title=\"Page 3\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li><strong>McBurney-Punkt:<\/strong> Druckpunkt auf der imagin\u00e4ren Linie zwischen rechter Spina iliaca anterior superior und Bauchnabel zwischen dem lateralen und mittleren Drittel*<\/li>\n<li><strong>Lanz-Punkt:<\/strong> Druckpunkt auf der imagin\u00e4ren Linie zwischen beiden Spinae iliacae anteriores superiores im lateralen Drittel<\/li>\n<li><strong>Blumberg-Zeichen:<\/strong> Ipsi- oder kontralateraler Loslassschmerz<\/li>\n<li><strong>Rovsing-Zeichen:<\/strong> Schmerz im rechten Unterbauch durch retrogrades Ausstreichen des Kolonrahmens<\/li>\n<li><strong>Psoas-Zeichen:<\/strong> Schmerz im rechten Unterbauch durch Flexion in der rechten H\u00fcfte gegen Widerstand (v.a. bei retroz\u00f6kal gelegener Appendix)<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"page\" title=\"Page 3\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Tastbefund:\u00a0<\/strong>\u00a0lokale Abwehrspannung im rechten Unterbauch als Reizung des parietalen Peritoneums, diffuse Abwehrspannung als Hinweis auf eine schwerwiegende komplizierte Form der Appendizitis.<\/p>\n<div class=\"page\" title=\"Page 3\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\" style=\"text-align: justify;\">\n<p><strong>Bildgebende Verfahren:\u00a0<\/strong>Abdomensonographie ist Mittel der ersten Wahl<\/p>\n<\/div>\n<div class=\"page\" title=\"Page 4\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\" style=\"text-align: justify;\">\n<p><strong>Adipositas:\u00a0<\/strong>\u00a0meist eingeschr\u00e4nkten sonografischen Beurteilbarkeit der Zeichen bei BMI &gt; 30 kg\/m2, daher die Rate an Computertomographien hier erh\u00f6ht.<\/p>\n<\/div>\n<div class=\"page\" title=\"Page 4\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Kinder und Schwangere:\u00a0<\/strong>Bei Kindern\/Schwangeren ist die Magnetresonanztomografie bei fehlender Strahlenbelastung eine sichere Alternative zur Computertomografie,. indiziert bei nicht eindeutigem sonografischen Befund.<\/p>\n<div class=\"page\" title=\"Page 5\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Therapie der Wahl:\u00a0<\/strong>Die Appendektomie ist in allen Altersgruppen die Therapie der Wahl in der Behandlung der akuten unkomplizierten Appendizitis.<\/p>\n<div class=\"page\" title=\"Page 5\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\" style=\"text-align: justify;\">\n<p><strong>Konservative Therapie:<\/strong>\u00a0Die momentane Datenlage reicht nicht aus, um einen\u00a0Vorteil der konservativen Therapie der akuten unkomplizierten Appendizitis erkennen zu lassen.<\/p>\n<\/div>\n<div class=\"page\" title=\"Page 6\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Perityphlitischer Abszess: <\/strong>Hier sollte gr\u00f6\u00dfenabh\u00e4ngig, risikostratifiziert und interdisziplin\u00e4r vorgegangen werden, das hei\u00dft bei Makroabszess sollte eine interventionelle Drainagenanlage in Kombination mit einer antibiotischen Therapie und je nach Verlauf eine Intervallappendektomie erfolgen.<\/p>\n<div class=\"page\" title=\"Page 6\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Mikroabzess:\u00a0<\/strong>sofortige Operation , da eine Punktion in der Regel technisch nicht machbar<\/p>\n<div class=\"page\" title=\"Page 7\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\"><strong>Therapieentscheidung der akuten Appendizitis bei besonderen Risikogruppen:\u00a0<\/strong>Insbesondere Patienten mit Nachweis eines Appendikolithen, Adipositas (BMI &gt;
