Ob diese ACCD aber wirklich das \u00dcberleben von Patienten mit Herzkreislaufstillstand verbessern ist bisher Gegenstand laufender Untersuchungen. Auch aus der Tschechischen Republik erreicht uns nun eine entsprechende Arbeit:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Karasek J et al. LUCAS II DEVICE FOR CARDIOPULMONARY RESUSCITATION IN A NONSELECTIVE OUT-OF-HOSPITAL CARDIAC ARREST POPULATION LEADS TO WORSE 30-DAY SURVIVAL RATE THAN MANUAL CHEST COMPRESSIONS. Journal of Emergency Medicine 2020; Vol. 59, No. 5, pp. 673\u2013679<\/strong><\/p>\n<\/blockquote>\n<\/div>\n<p style=\"text-align: justify;\">Das LUCAS (Lund University Cardiopulmonary Assist System; Physio-Control Inc.\/Jolife AB, Lund, Schweden)-Device wurde f\u00fcr automatische Thoraxkompressionen w\u00e4hrend der kardiopulmonalen Reanimation entwickelt. Die bekannte Evidenz \u00fcber den Einsatz dieses Ger\u00e4tes bei pr\u00e4hospitalem Herzstillstand \u00a0legt nahe, dass es nicht routinem\u00e4\u00dfig eingesetzt werden sollte, da (bisher) keine \u00fcberlegenen Wirkungen nachgewiesen wurde. Der Einsatz ist damit auf spezifische Fallkonstellationen beschr\u00e4nkt.<\/p>\n<p><strong>Methodik: <\/strong><\/p>\n<ul>\n<li>\u00a0prospektives Register aller konsekutiven pr\u00e4hospital kardiopulmonal reanimierter Patienten an vier Notarztstandorten<\/li>\n<li>zwei Standorte nutzen das LUCAS-Ger\u00e4t und zwei Standorten die manuelle Thoraxkompressionen<\/li>\n<\/ul>\n<p><strong>Ergebnisse:<\/strong><\/p>\n<ul>\n<li>n=278 Patienten wurden in die Analyse einbezogen\n<ul>\n<li>n=144 mit LUCAS<\/li>\n<li>n=134 mit manuellen Thoraxkompressionen<\/li>\n<\/ul>\n<\/li>\n<li>In der LUCAS-Gruppe\n<ul>\n<li>gab es mehr bezeugte Herzkreislaufstillst\u00e4nde (79% vs. 64%; p=0,0074)<\/li>\n<li>waren die Patienten \u00e4lter (p=0,03)<\/li>\n<\/ul>\n<\/li>\n<li>\u00a0es fand sich keine signifikanter Unterschied in der ROSC-Rate\n<ul>\n<li>31% in der Gruppe mit alleinigen manuellen Thoraxkompressionen vs. 25% in der LUCAS-Gruppe (p=0,35)<\/li>\n<li>aber es fanden sich mehr Konversionen vom nicht schockierbaren zum schockierbaren Rhythmus in der LUCAS-Gruppe (21% vs. 10%; p=0,04)<\/li>\n<\/ul>\n<\/li>\n<li>30-Tage-\u00dcberlebensrate war in der LUCAS-Gruppe signifikant niedriger (5% vs. 16%) im Vergleich zur Gruppe mit alleinigen manuellen Thoraxkompressionen (p=0,044)<\/li>\n<li>f\u00fcr das 180-Tage-\u00dcberleben fand sich kein signifikanter Unterschied (5,45% in der LUCAS-Gruppe vs. 9,42% in der Nicht-LUCAS-Gruppe; p=0,25) (Cave: Die Angaben im Abstract sind inkorrekt im Vergleich zu den Angaben in der Publikation, es lohnt sich immer die ganze Publikation zu lesen!)<\/li>\n<\/ul>\n<p><strong>Schlussfolgerungen: <\/strong><\/p>\n<p style=\"text-align: justify;\">Der Einsatz des LUCAS-DEVICE zeigte eine niedrigere \u00dcberlebensrate an Tag 30 und Tag 180 bei Patienten mit pr\u00e4hospitalem Herzkreislaufstillstand im Vergleich zur Gruppe mit alleinigen manuellen Thoraxkompressionen.
