Um Patienten mit erh\u00f6htem Risiko zu identifizieren, wurde in der folgenden Untersuchung die Beziehung zwischen der Patientensterblichkeit und den Aufnahme-Laktatwerten bei der Behandlung von nicht-traumatisierten kritisch kranken Patienten in der Notaufnahme (ED) untersucht.<\/p>\n<blockquote><p><strong>Bernhard M et al. Scand Journal Trauma, Resusc, Emerg Med (2020) 28:82\u00a0https:\/\/doi.org\/10.1186\/s13049-020-00777-y (<a href=\"https:\/\/sjtrem.biomedcentral.com\/track\/pdf\/10.1186\/s13049-020-00777-y\" target=\"_blank\" rel=\"noopener noreferrer\">PDF<\/a>)<\/strong><\/p><\/blockquote>\n<ul>\n<li>In dieser prospektiven, monozentrischen Beobachtungsstudie in einer deutschen universit\u00e4ren Notaufnahme wurden alle erwachsenen Patienten ausgewertet, die zwischen dem 1. September 2014 und dem 31. August 2015 in den Schockraum der Notaufnahme aufgenommen wurden.<\/li>\n<li>Blutproben f\u00fcr die Blutgasanalyse, einschlie\u00dflich Laktatspiegel, wurden unmittelbar bei der Aufnahme entnommen.<\/li>\n<li>Studienendpunkt war die 30-Tage-Mortalit\u00e4t.<\/li>\n<li>W\u00e4hrend des Studienzeitraums wurden 532 Patienten in den Schockraumraum der Notaufnahme aufgenommen.<\/li>\n<li>Die Daten von 523 Patienten (98,3%) waren verf\u00fcgbar.<\/li>\n<li>Die Gesamtmortalit\u00e4t nach 30 Tagen betrug 34,2%.<\/li>\n<li>Patienten, die sich im Schockraum mit Laktatwerten &lt;2,0 mmol\/l einlieferten, hatten eine 30-Tage-Mortalit\u00e4t von 22,7%, w\u00e4hrend Laktatwerte \u00fcber 8,0 mmol\/l mit einer h\u00f6heren Mortalit\u00e4t assoziiert waren (8,0-9,9 mmol\/l: OR: 2,83, 95%CI: 1,13-7,11, p=0,03, und \u226510 mmol\/l: OR: 7,56, 95% CI: 4,18-13,77, p&lt;0,001).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Schlussfolgerung:<\/strong> Hohe Laktatspiegel bei der Aufnahme sind mit einer erh\u00f6hten 24-Stunden- und 30-Tage-Mortalit\u00e4t assoziiert. Diese Messungen k\u00f6nnen nicht nur zur Vorhersage der Mortalit\u00e4t verwendet werden, sondern auch zur Identifizierung von Patienten mit einem Risiko, kritisch zu erkranken.
