Frau Kranz et al. hat nun eine sch\u00f6ne Originalarbeit im Deutschen \u00c4rzteblatt hierzu publiziert:<\/p>\n<blockquote>\n<p class=\"unterzeile\" style=\"text-align: justify;\"><strong>Kranz J et al. Katheter-assoziierte Harnwegsinfektionen bei erwachsenen Patienten. Pr\u00e4ventionsstrategien und Therapieoptionen. Dtsch Arztebl Int 2020; 117: <span class=\"nobr\">83-8<\/span>; DOI: 10.3238\/arztebl.2020.0083 (<a href=\"https:\/\/www.aerzteblatt.de\/archiv\/212331\/Katheter-assoziierte-Harnwegsinfektionen-bei-erwachsenen-Patienten\" target=\"_blank\" rel=\"noopener noreferrer\">PDF<\/a>)<\/strong><\/p>\n<\/blockquote>\n<p>Eine kurze Zusammenfassung wesentlicher Punkte:<\/p>\n<ul>\n<li style=\"text-align: justify;\">Die routinem\u00e4\u00dfige Katheterisierung ist h\u00e4ufig eher mit Risiken als mit einem Nutzen verbunden. Daher muss die Notwendigkeit zur Katheterisierung immer kritisch gepr\u00fcft werden.<\/li>\n<li style=\"text-align: justify;\">Es muss unterschieden werden zwischen einer nicht behandlungsbed\u00fcrftigen Katheter-assoziierten asymptomatischen Bakteriurie und antimikrobiell zu therapierenden Katheter-assoziierten Harnwegsinfektionen.<\/li>\n<li style=\"text-align: justify;\">Bei der antibiotischen Therapie ist die lokale Resistenzlage zu beachten, um die Effektivit\u00e4t der Therapie zu steigern und eine weitere Resistenzselektion zu vermeiden.<\/li>\n<li style=\"text-align: justify;\">Bei der antimikrobiellen Therapie sind die Aspekte des \u201eantibiotic stewardship\u201c (ABS) zu ber\u00fccksichtigen. Hierunter versteht man den rationalen und verantwortungsvollen Einsatz von antimikrobiellen Substanzen:\n<ul>\n<li style=\"text-align: justify;\">die Auswahl der Substanz,<\/li>\n<li style=\"text-align: justify;\">ihre Dosierung und Form<\/li>\n<li style=\"text-align: justify;\">sowie das Anpassen der Therapiedauer.<\/li>\n<li style=\"text-align: justify;\">Ziel ist es, den Patienten bestm\u00f6glich zu behandeln und eine Resistenzentwicklung zu verhindern.<\/li>\n<\/ul>\n<\/li>\n<li style=\"text-align: justify;\">Die Katheter-Verweildauer ist ein entscheidender Risikofaktor f\u00fcr die Entstehung von CA-UTIs. Daher ist die Indikation zur Katheterisierung zu pr\u00fcfen und auf eine m\u00f6glichst kurze Verweildauer zu achten.<\/li>\n<li style=\"text-align: justify;\">Verschiedene Ma\u00dfnahmen (Pr\u00fcfen der Katheter-Indikation und Liegedauer, Schulen des Pflegepersonals) sind zu b\u00fcndeln, um das Risiko von CA-UTIs f\u00fcr den einzelnen Patienten so gering wie m\u00f6glich zu halten.<\/li>\n<li style=\"text-align: justify;\">Im Hinblick auf die Entstehung von Katheter-assoziierten Harnwegsinfektionen besteht derzeit keine Evidenz f\u00fcr einen Unterschied zwischen den beiden Katheterisierungsrouten transurethral versus suprapubisch.<\/li>\n<li style=\"text-align: justify;\">Es zeigen sich keine relevanten Unterschiede in Abh\u00e4ngigkeit vom verwendeten Kathetermaterial (Latex versus Silikon versus Polyvinylchlorid versus Polyurethan) auf den Endpunkt Katheter-assoziierte Harnwegsinfektionen.<\/li>\n<li style=\"text-align: justify;\">Der Nutzen antiseptisch beschichteter Katheter im Vergleich zu nichtbeschichteten Standardkathetern ist derzeit unklar.
