Association of Parenteral Anticoagulation Therapy With Outcomes in Chinese Patients Undergoing Percutaneous Coronary Intervention for Non\u2013ST-Segment Elevation Acute Coronary Syndrome. JAMA Internal Med 2019; 179:186-94; doi:10.1001\/jamainternmed.2018.5953<\/strong><\/p>\n<ul>\n<li>retrospektive Kohortenstudie an 5 Kliniken in China<\/li>\n<li>Studienzeitraum: 2010 bis 2014<\/li>\n<li>n=8.197 konsekutive NSTEMI-Patienten, die einer Herzkatheter-Intervention unterzogen wurden<\/li>\n<li>804 Patienten konnten letztlich ausgewertet werden, 1.393 Patienten mussten ausgeschlossen werden, \u00fcberwiegend weil sie erst <u>nach<\/u> der Katheter-Intervention eine parenterale Antikoagulationstherapie (PACT) bekamen.<\/li>\n<li>verglichen wurden also letztlich 2.115 Patienten, die in der Zeit zwischen Diagnosestellung und der Herzkatheter-Intervention neben der dualen Pl\u00e4ttchenhemmung eine parenterale Antikoagulationstherapie mit niedermolekularem Heparin oder Fondaparinux erhalten haben (Gruppe PACT), mit 4.689 Patienten, die erst w\u00e4hrend der Herzkatheter-Untersuchung leitliniengem\u00e4\u00df mit 70-100 IU Heparin\/kg antikoaguliert wurden (Gruppe non-PACT).<\/li>\n<li>hinsichtlich der Krankenhaussterblichkeit (0,3% in der PACT-Gruppe und 0,1% in der non-PACT-Gruppe und der Rate an Myokardinfarkten (0,3% in beiden Gruppen) gab es keine Unterschiede zwischen den beiden Gruppen<\/li>\n<li>aber es waren signifikant mehr schwere Blutungsereignisse in der PACT-Gruppe: 2,5% im Vergleich zu 1,0% in der non-PACT-Gruppe<\/li>\n<li>auch im Langzeit-Outcome nach ein und drei Jahren gab es keinen Unterschied in der Sterblichkeit<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Soll man also auf eine parenterale Antikoagulationstherapie mit Heparin oder Fondaparinux bis zur Herzkatheter-Untersuchung zuwarten? Das kann leider anhand dieser Daten nicht beantwortet werden. Daf\u00fcr hat die Studie zu viele Schw\u00e4chen:<\/p>\n<ul>\n<li>es handelt sich nur um eine retrospektive Beobachtungsstudie, so dass zahlreiche Faktoren die Entscheidung zur parenteralen Antikoagulationstherapie und das Blutungsrisiko beeinflusst haben k\u00f6nnen.<\/li>\n<li>bei den PACT-Patienten fand die Intervention, obwohl sie einen signifikant h\u00f6heren GRACE-Score hatten, bei 45% erst nach &gt;72 Stunden statt, d.h. sie lagen auch mehr als 3 Tage parenteral antikoaguliert und mit doppelter Pl\u00e4ttchenhemmung auf der Station;
