Gerade an der Nahtstelle zwischen pr\u00e4hospitaler und innerklinischer Versorgung besteht eine gro\u00dfe Herausforderung darin, die Patientenbehandlung ohne Zeitverlust und auf hohem Niveau fortzuf\u00fchren. In den letzten Jahren hat sich daher ein Interessenschwerpunkt zum Thema (konservatives) \u201enichttraumatologische Schockraumversorgung\u201c in Zentralen Notaufnahmen entwickelt. Mittlerweile liegen erste monozentrische Daten vor (OBSERvE-Studie) und es wird kontinuierlich an der Entwicklung eines nichttraumatologischen Schockraummanagements zur optimalen Versorgung von kritisch kranken Patienten in der Zentralen Notaufnahme gearbeitet.<\/p>\n<p style=\"text-align: justify;\">Der vorliegende Beitrag beschreibt erstmalig auf der Basis von Erfahrungen und Expertenmeinungen eine strukturierte Vorgehensweise im nichttraumatologischen Schockraummanagement.<\/p>\n<div class=\"page\" title=\"Page 3\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<div class=\"page\" title=\"Page 12\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li>Die Stabilisierung und Aufrechterhaltung der Vitalfunktionen eines kritisch kranken Patienten unterscheidet sich nicht von der eines traumatologischen Patienten.<\/li>\n<li>Da die Ursachen der lebensbedrohlichen Situation h\u00e4ufig unbekannt sind, sind klare diagnostische Pfade notwendig, um m\u00f6glichst schnell eine Diagnose sichern und die not- wendige Initialtherapie einleiten zu k\u00f6nnen.<\/li>\n<li>Aufgrund der Komplexit\u00e4t der h\u00e4ufig multipel erkrankten Patienten bedarf es eines notfall- und intensivmedizinisch geschulten Notaufnahmeteams mit klaren Festlegungen im Diagnostik- und Behandlungsablauf.<\/li>\n<li>Um Kontinuit\u00e4t und Routine in der Versorgung kritisch Kranker zur gew\u00e4hrleisten und weil die Intensivressourcen h\u00e4ufig knapp sind, sollte das Notaufnahmeteam in der Lage sein, eine erweiterte Intensivtherapie durchzuf\u00fchren.<\/li>\n<li>Umfangreiche Kenntnisse in der Notfallsonographie erleichtern die schnelle Diagnose von Erkrankungen, deshalb muss die Ausbildung in der Notfallsonographie weiter ausgebaut werden.<\/li>\n<li>Um weitere R\u00fcckschl\u00fcsse auf die Versorgung kritisch Kranker zu erlangen, ist die Etablierung eines Registers analog zum TraumaRegister DGU\u00ae notwendig.<\/li>\n<li>Ein standardisiertes Training bzw. ein Kurs zum nichttraumatologischen Schockraummanagement, der in den Anforderungen \u00fcber die bisher etablierten Kurssysteme hinausgeht,
