Not\u00e4rztliche Durchf\u00fchrung von Narkosen.\u00a0Eine Istanalyse der Jahre 2015\u20132017.\u00a0Anaesthesist\u00a0https:\/\/doi.org\/10.1007\/s00101-019-0562-6<\/strong><\/p>\n<\/blockquote>\n<p style=\"text-align: justify;\">Notfallnarkose und Atemwegssicherung sind eine nicht zu untersch\u00e4tzende Herausforderung in der Notfallmedizin. Luckscheiter und Kollegen untersuchten daher an einem klar definierten SQR-BW Datensatz verschiedene Parameter der Notfallnarkose und des Atemwegsmanagements. Hierbei sollte der Umsetzungsgrad der S1 Leitlinie Notfallnarkose untersucht werden, um die aktuelle Realit\u00e4t im Sinne einer Istanalyse zu erfassen.<\/p>\n<p><strong>Methodik:<\/strong><\/p>\n<ul>\n<li>Erfassung von 12.605 Datens\u00e4tzen mit Narkose (1,7%) aus initial 730.000 Datens\u00e4tzen unter der Massgabe:\n<ul>\n<li>Erwachsene Patienten, keine Analgosedierung, keine NIV,<br \/>\ndavon<\/p>\n<ul>\n<li>invasive Atemwegssicherung n=9.875<\/li>\n<li>kardiale Patienten n=1.828<\/li>\n<li>respiratorische Insuffizienz n= 1.207<\/li>\n<li>neurologisches Defizit n=4.114<\/li>\n<li>Traumapatienten n=3.492<\/li>\n<li>sonstige n=1.664<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li>Beurteilung von Narkosekomnzepten:\n<ul>\n<li>kardiale Patienten (z.B. kardiogener\u00a0Schock, Rhythmusst\u00f6rung oder\u00a0Lungen\u00f6dem) \u2013 Etomidat bzw.\u00a0Benzodiazepin plus starkes Opioid,<\/li>\n<li>Patienten mit akuter respiratorischer\u00a0Insuffizienz (septische Pneumonie\u00a0oder obstruktive Atmungsst\u00f6rungen)<br \/>\n\u2013 Propofol bzw. Etomidat und starkes Opioid oder Ketamin und Benzodiazepin,<\/li>\n<li>Patienten mit akut neurologischem Defizit (Schlaganfall, isoliertes Sch\u00e4del-Hirn-Trauma, Status epilepticus)<br \/>\n\u2013 Thiopental bzw. Propofol jeweils mit starkem Opioid oder Ketamin,<\/li>\n<li>Traumapatienten (inklusive Verbrennungen)<br \/>\n\u2013 Thiopental bzw. Benzodiazepin bzw. Propofol jeweils\u00a0mit starkem Opioid oder Ketamin,<\/li>\n<li>sonstige Patienten (z.B. akutes Abdomen,\u00a0Intoxikation, Hypothermie)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Ergebnisse:<\/strong><\/p>\n<ul>\n<li>Narkosemonitoring wird in 75% gem\u00e4ss den Empfehlungen der S1 Leitlinie Notfallnarkose durchgef\u00fchrt:\n<ul>\n<li>3-Kanal-EKG: 98%<\/li>\n<li>Blutdruckmessung: 97%<\/li>\n<li>Pulsoxymetrie: 92%<\/li>\n<li>Kapnographie: 79%<\/li>\n<\/ul>\n<\/li>\n<li>Atemwegssicherung:\n<ul>\n<li>endotracheale Intubation: 88,5%<\/li>\n<li>supraglottische Atemwege: 7,3%<\/li>\n<li>chirurgischer Atemweg: 1,9% (deutlich h\u00f6her als erwartet!)<\/li>\n<li>Videolaryngoskopie: 4,6%<\/li>\n<li>prim\u00e4rer Einsatz Videolaryngoskopie: 86%<\/li>\n<li>erschwerte Intubation: 7,6%<\/li>\n<li>Maskenbeatmung unm\u00f6glich: 1,3%<\/li>\n<li>Schwieger Atemweg: 12,9% (An\u00e4sthesisten: 11,9% vs. Nicht-An\u00e4sthesisten: 19,2%, p&lt;0,01)<\/li>\n<\/ul>\n<\/li>\n<li>Notfallnarkose:\n<ul>\n<li>Konformit\u00e4t zur S1 LL Notfallnarlose:\n<ul>\n<li>kardiale Risikopatient: 35% (h\u00e4ufige Anwendung von Propofol)<\/li>\n<li>respiratorische Insuffizienz: 51%<\/li>\n<li>neurologische St\u00f6rung: 53%<\/li>\n<li>Traumapatienten: 79%<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Was lernen wir aus der Studie:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify;\">Es liegen erstmalig tolle Ergebnisse zum IST-Zustand aus einer suffizienten Untersuchung mit rund 34-57% aller Notfallnarkosen im Untersuchungszeitraum (bei der Annahme von 3-5% Notfallnarkosen im bodengebundenen Notarztdienst) vor.<\/li>\n<li>Eine entsprechenden Aussage aus dieser Analyse abzuleiten ist nicht einfach und kann deutlichen Einfl\u00fcssen unterliegen, der Versuch ist aber schon genial.<\/li>\n<li>Auch wenn teils nur eine niedrige Konformit\u00e4t zur S1 Leitlinie Notfallnarkose vorliegt, so muss ber\u00fccksichtigt werden, das beispielsweise beim Polytrauma bei 6 ausgewiesenen Medikamentenkombinationen a priori eine deutlich h\u00f6here Wahrscheinlichkeit der Einhaltung als beim kardiale Patienten mit nur 2 empfohlenen Kombinationen vorliegt.<\/li>\n<li>M\u00f6glicherweise m\u00fcssen die Fallvignetten aus der S1 Leitlinie Notfallnarkose auch \u00fcberarbeitet werden, hier sind die Leitlinienautoren aufgerufen dies zu pr\u00fcfen.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Insgesamt sind wir hier auf einem sehr guten Weg, und im Vergleich zu Voruntersuchungen von Hinkelbein et al. hat sich im untersuchten Bereich wirklich sehr viel getan!
