Post <a href=\"http:\/\/news-papers.eu\/atemwegsregister-in-der-notfallmedizin-2\" target=\"_blank\" rel=\"noopener\"><strong>Atemwegsregister in der Notfallmedizin<\/strong><\/a>). W\u00e4hrend in viele anderen Teilen der Welt systematische Atemwegsregister bestehen, fehlen diese im deutschsprachigen Raum bisher vollst\u00e4ndig.<\/p>\n<p style=\"text-align: justify;\">Vor diesem Hintergrund wurden am Universit\u00e4tsklinikum Leipzig \u00fcber ein Jahr das pr\u00e4hospital bereits durchgef\u00fchrte und das innerklinische erfolgende Atemwegsmanagement bei kritisch kranken nicht-traumatologischen Schockraumpatienten in einer <strong>prospektiven monozentrischen Studie<\/strong> evaluiert.<\/p>\n<p style=\"text-align: justify;\">W\u00e4hrend der <strong>einj\u00e4hrigen Studienphase<\/strong> wurden 34.303 Patienten in der Zentralen Notaufnahme behandelt, von denen 21.074 wegen eines nicht-traumatologischen Notfalls versorgt wurden. 532 dieser Patienten wiesen eine lebensbedrohliche Situation mit einer Schockraumindikation auf.<\/p>\n<ul>\n<li style=\"text-align: justify;\"><strong>150<\/strong> dieser 532 Patienten (28,2%) hatten ein <strong>pr\u00e4hospitales Atemwegsmanagement<\/strong> erhalten.\n<ul>\n<li style=\"text-align: justify;\">In 16 F\u00e4llen (11%) musste der inserierte Atemweg innerklinische auf einen Endotrachealtubus gewechselt werden (in 2 F\u00e4llen bei einer unbemerkten \u00f6sphagealen Intubation und in 15 F\u00e4llen aufgrund eines einliegenden Larnyxtubus).<\/li>\n<li>9,3% der im Schockraum aufgenommenen Patienten hatten pr\u00e4hospital einen Larynxtubus inseriert bekommen.<\/li>\n<li>57% der Beatmungen mittels Larynxtubus wurden als insuffizient bewertet.<\/li>\n<li>nur 83% der pr\u00e4hospital mit einem Atemweg versorgten Patienten hatten eine Kapnographie erhalten<\/li>\n<\/ul>\n<\/li>\n<li><strong>136<\/strong> (26%) der kritisch kranken Patienten (die kein pr\u00e4hospitales Atemwegsmanagemet erhalten hatten) erhielten eine Atemwegssicherung direkt nach Aufnahme w\u00e4hrend der i<strong>nnenklinischen Schockraumversorgung<\/strong> (davon 2 mittels Einlage eines Endotrachealtubus in ein vorbestellendes chirurgisches Tracheostoma)\n<ul>\n<li><strong>First-Pass Intubation Success<\/strong> (FPS) = 71%<\/li>\n<li><strong>Overall-Pass Intubation Success<\/strong> (Gesamterfolgsrate der Atemwegssicherung) = 100%.<\/li>\n<li><strong>multiple Intubationsversuche<\/strong> = 29%<\/li>\n<li>Ein niedriger Cormack\/Lehane (C\/L) Grade war mit weniger Intubationsversuchen (C\/L1\/2 vs. 3\/4: 1,2\u00b10,5 vs. 1,8\u00b11,2, p=0,0002) assoziiert.<\/li>\n<li><strong>FPS pro C\/L Grad<\/strong>: C\/L 1: 80%, C\/L2: 78%, C\/L3: 57%, C\/L: 25%<\/li>\n<li>Die <strong>Komplikationsrate<\/strong> betrug 43%.\n<ul>\n<li>Hypotension: 20%<\/li>\n<li>Desaturierung: 9%<\/li>\n<li>Aspiration: 3%<\/li>\n<li>endobronchiale Intubation: 1,6%<\/li>\n<li>Herzkreislaufstillstand: 3,1%<\/li>\n<\/ul>\n<\/li>\n<li><strong>Vergleich<\/strong> direkt Laryngoskopie (n=94) vs. Videolaryngoskopie (CMAC, Storz, Macintosh-like blade, n=40):\n<ul>\n<li>ben\u00f6tigte Intubationsversuche = 1,2\u00b10,5 vs. 1,2\u00b10,4, p=0,887<\/li>\n<li>C\/L 1\/2 = 81 vs. 74%, p=0,334<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" data-attachment-id=\"8980\" data-permalink=\"http:\/\/news-papers.eu\/?attachment_id=8980\" data-orig-file=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/03\/Intubationsbedigungen_OcEAN-1-e1550257757470.jpg?fit=300%2C113\" data-orig-size=\"300,113\" data-comments-opened=\"1\" data-image-meta=\"{&quot;aperture&quot;:&quot;0&quot;,&quot;credit&quot;:&quot;&quot;,&quot;camera&quot;:&quot;&quot;,&quot;caption&quot;:&quot;&quot;,&quot;created_timestamp&quot;:&quot;0&quot;,&quot;copyright&quot;:&quot;&quot;,&quot;focal_length&quot;:&quot;0&quot;,&quot;iso&quot;:&quot;0&quot;,&quot;shutter_speed&quot;:&quot;0&quot;,&quot;title&quot;:&quot;&quot;,&quot;orientation&quot;:&quot;0&quot;}\" data-image-title=\"Intubationsbedigungen_OcEAN\" data-image-description=\"\" data-image-caption=\"\" data-large-file=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/03\/Intubationsbedigungen_OcEAN-1-e1550257757470.jpg?fit=1024%2C386\" class=\"alignnone wp-image-8980\" src=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/03\/Intubationsbedigungen_OcEAN-1.jpg?resize=661%2C249\" alt=\"\" width=\"661\" height=\"249\" \/><\/p>\n<h6><strong>Abb.1: Intubationsbedingungen und Cormack\/Lehane Grad und die assoziierte Anzahl an ben\u00f6tigten Intubationsversuche bei 136 kritisch kranken nicht-traumatologischen Schockraumpatiennten aus der OcEAN-Studie.<\/strong><\/h6>\n<ul>\n<li style=\"text-align: justify;\">Pr\u00e4hospital und innenklinisch sollte <strong>immer eine Kapnnographie<\/strong> Anwendung finden.<\/li>\n<li style=\"text-align: justify;\">Weitere Anstrengungen sind zu unternehmen um einen noch h\u00f6heren <strong>First-Pass Intubation Success<\/strong> zu erreichen (Zielwert mindestens 84% (s. Emerg Med Austral. 2017;29:40\u201348)<\/li>\n<li style=\"text-align: justify;\">Obwohl die Videolaryngoskopie mit keiner besseren Einstellbarkeit der Stimmbandebene und keiner niedrigeren Anzahl an Intubationsversuche assoziiert war, war die <strong>Videolaryngoskopie ein wichtiges Hilfsmittel<\/strong>. M\u00f6glicherweise hat die Lernkurve von in der Videolaryngoskopie nicht so erfahrenen Kolleginnen und Kollegen mit diesem Ergebnis zu tun.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Die <strong>OcEAN-Studie<\/strong> zeigt erstmalig entsprechende Atemwegsmanagementergebnisse aus einer deutschen Zentralen Notaufnahme.
