Die \u00fcberarbeitete S3-Leitlinie \u201ePolytrauma \/ Schwerverletzten-Behandlung\u201c 2016 begrenzt die Indikation eindeutig auf \u201emassiv-blutende Patienten\u201c; die 4. Auflage der European Trauma Guideline\u201c 2016 spricht von \u201esignificant haemorrhage\u201c. TXA ist ein Medikament mit entsprechenden Risiken und Nebenwirkungen. Zudem ist TXA nur ein Antifibrinolytikum, kein H\u00e4mostatikum. Die Hyperfibrinolyse beim Trauma-Patienten findet sich aber nur bei 2-18% der Patienten und selbst beim massivtransfundierten Traumapatienten nur in einem Drittel der F\u00e4lle. Eine gest\u00f6rte (\u201esubphysiologische\u201c) Fibrinolyse (sog. \u201eShutdown\u201c) ist nach Trauma sogar h\u00e4ufiger als die Hyperfibrinolyse.<\/p>\n<ul>\n<li style=\"text-align: justify;\">Cotton BA et al. Hyperfibrinolysis at admission is an uncommon but highly lethal event associated with shock and prehospital fluid administration. J Trauma Acute Care Surg 2012; 73:365-70<br \/>\n\u201eThe trauma registry was queried for patients who were our highest-level trauma activations and admitted directly from the scene (October 2009-October 2010). \u2026 Admissionr-TEG values were captured on 1996 consecutive admissions. Only 41 patients (2%) had hyperfibrinolysis at\u00a0admission\u00a0r-TEG\u201c<\/li>\n<li style=\"text-align: justify;\">Moore HB et al. Hyperfibrinolysis, physiologic fibrinolysis, and fibrinolysis shutdown: the spectrum of postinjury fibrinolysis and relevance to antifibrinolytic therapy. J Trauma Acute Care Surg 2014; 77:811-7<br \/>\n\u201eThrombelastography and clinical data were prospectively collected on trauma patients admitted to our Level I trauma center from 2010 to 2013. Patients with an Injury Severity Score (ISS) of 15 or greater were evaluated. \u2026 One hundred eighty patients were included in the study. The median age was 42 years (interquartile range [IQR], 28-55 years), 70% were male, and 21% had penetrating injuries. The median ISS was 29 (IQR, 22-36), and the median base deficit was 9 mEq\/L (IQR, 6-13 mEq\/L). Distribution of\u00a0fibrinolysiswas as follows: shutdown, 64% (115 of 180);\u00a0physiologic, 18% (32 of 180); and\u00a0hyperfibrinolysis, 18% (33 of 180). Mortality rates were lower for the\u00a0physiologic\u00a0group (3%) compared with the\u00a0hyperfibrinolysis\u00a0(44%) and\u00a0shutdown\u00a0(17%) groups (p = 0.001)\u201c<\/li>\n<li style=\"text-align: justify;\">Chapman MP et al. The \u201eDeath Diamond\u201c: Rapid thrombelastography identifies lethal hyperfibrinolysis. J Trauma Acute Care Surg 2015; 79:925-9<br \/>\n\u201eWe queried our trauma registry for patients who either died or spent at least 1 day in the intensive care unit, received at least 1 U of packed red blood cells, and had an admission rTEG. Within this group of 572 patients. we identified 42 pairs of nonsurvivors and survivors who matched on age, sex, injury mechanism, and New Injury Severity Score (NISS).\u00a0\u2026 Within the matched group, 17% of the patients developed hyperfibrinolysis.\u201c<\/li>\n<li style=\"text-align: justify;\">Kashuk JL et al. Primary fibrinolysis is integral in the pathogenesis of the acute coagulopathy of trauma. Ann Surg 2010; 252:434-42<br \/>\n\u201eConsecutive patients over 14 months at risk for postinjury\u00a0coagulopathywere stratified by transfusion requirements into massive (MT) &gt;10 units\/6 hours (n = 32), moderate (Mod) 5 to 9 units\/6 hours (n = 15), and minimal (Min) &lt;5 units\/6 hours (n = 14).\u00a0\u2026 A total of 34% of injured patients requiring MT had primary fibrinolysis\u2026\u201c<\/li>\n<li style=\"text-align: justify;\">siehe hierzu auch die hervorragende \u00dcbersicht von Prof. Marc Maegele: Unkritischer Gebrauch von Tranexams\u00e4ure beim Traumapatienten.
