Eigentlich hatten wir ja gelernt, dass Sauerstoff eines der ersten Medikamente bei Myokardinfarkt sind, dann kamen die Ergebnisse der <a href=\"http:\/\/circ.ahajournals.org\/content\/131\/24\/2143.long\" target=\"_blank\" rel=\"noopener\"><strong>AVOID-Studie<\/strong><\/a>\u00a0(Air\u00a0Versus Oxygen in Myocardial Infarction) und man wurde zaghafter Sauerstoff bei guter Oxygenierung (SpO2 &gt;94%) einem Patienten mit STEMI zu geben. Und nun kommen diese Ergebnisse:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Hofmann R, et al.\u00a0Oxygen Therapy in Suspected Acute\u00a0Myocardial Infarction. NEJM 2017, 28. August, \u00a0DOI: 10.1056\/NEJMoa170622<\/strong>\b (<a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa1706222\" target=\"_blank\" rel=\"noopener\">LINK<\/a>)<\/p>\n<\/blockquote>\n<hr \/>\n<p style=\"text-align: justify;\">Hofmann und Kollegen untersuchten in einem randomisierten Studiendesign (<a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa1706222\" target=\"_blank\" rel=\"noopener\"><strong>DETO2X\u2013SWEDEHEART<\/strong><\/a>), Patienten, die im pr\u00e4hospitalen oder innerklinischen Setting mit V.a. einen Myokardinfarkt behandelt wurden und nicht unter einer Hypox\u00e4mie (SpO2 &lt;90%) litten. Dabei wurden die Patienten entweder einer Sauerstofftherapie mit 6 lO2\/min \u00fcber eine offene Gesichtsmaske oder Raumluft zugef\u00fchrt.<\/p>\n<p>Beide Behandlungsarme (Sauerstoff vs Raumluft) waren hinsichtlich der Epidemiologie vergleichbar:<\/p>\n<ul>\n<li>Alter \u00a0&#8211; Median: 68 Jahre alt<\/li>\n<li>m\u00e4nnlichen Geschlechtes:\u00a068-70%<\/li>\n<li>Body-Mass-Index: 27<\/li>\n<li>Anteil an Rauchern: 21%<\/li>\n<li>Vorangegangene Myokardinfarkt: 20%, PCI 16%, Bypass-OP 6%<\/li>\n<li>Beschwerden bein Aufnahme:\n<ul>\n<li>Brustschmerz: 94%<\/li>\n<li>Luftnot: 2%<\/li>\n<li>Herzkreislaufstillstand &lt;0,1%<\/li>\n<\/ul>\n<\/li>\n<li>Zuf\u00fchrung \u00fcber Rettungsdienst: 67%<\/li>\n<li>Median der Sauerstoffs\u00e4ttigung 97%<\/li>\n<\/ul>\n<p>Interessant ist das Spektrum der Erkrankungen in dieser Untersuchung (Sauerstoff vs Raumluft):<\/p>\n<ul>\n<li>Akuter Myokardinfarkt: 75,1 vs. 76,1%<\/li>\n<li>STEMI 43,2 vs. 45,8%<\/li>\n<li>Angina pectoris 5,7 vs. 5,6%<\/li>\n<li>Andere kardiale Ursachen: je 7,7%<\/li>\n<li>Pulmonale Ursachen: je 0,5%<\/li>\n<li>Unspezifscher Brustschmerz: 7,8 vs. 7,1%<\/li>\n<li>Ander nicht-kardiale Ursachen: 3,2 vs. 3,1%<\/li>\n<\/ul>\n<p>Outcome (Sauerstoff vs. Raumluft) der DETO2X-SWEDEHEART Studie:<\/p>\n<ul>\n<li><strong>1-Jahrsmortalit\u00e4t: 5 vs. 5,1%, p=0,80<\/strong><\/li>\n<li><strong>Rehospitalisation mit akuten Myokardinfarkt: 3,8 vs. 3,3%, p=0,33<\/strong><\/li>\n<li><strong>Tod binnen 30 Tage: 2,2 vs. 2,0, p=0,67<\/strong><\/li>\n<li><strong>Median des h\u00f6hsten Troponin (hs TNT) w\u00e4hrend Krankenhausaufenthalt: 946 vs. 983, p=0,97<\/strong><\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Im Ergebnis zeigte sich in \u00dcbereinstimmung zu anderen Metaanalyse und zu der\u00a0Supplemental Oxygen in Catheterized\u00a0Coronary Emergency Reperfusion (<a href=\"https:\/\/www.karger.com\/Article\/Abstract\/398786\" target=\"_blank\" rel=\"noopener\"><strong>SOCCER<\/strong><\/a>)-Studie (Khoshnood et al., s.u.), dass keine h\u00f6here Letalit\u00e4t (all-cause mortality 1 year), keine h\u00f6here Rate an Rehospitalisierungen mit akutem Myokardinfarkt \u00a0und keine gr\u00f6\u00dfere Infarktgr\u00f6\u00dfe bei Gabe von Sauerstoff vorlag.<\/p>\n<p>Besonderheiten:<\/p>\n<ul>\n<li>DETO2X-SWEDEHEART beinhaltet 15 mal so viele Patienten wie AVOID (n= 6629 vs. n=441).<\/li>\n<li>DETO2X-SWEDEHEART rekrutierte pr\u00e4hospital und in der Notaufnahme Patienten mit V.a. Myokardinfarkt und AVOID Patienten mit einem STEMI nur in der Pr\u00e4klinik<\/li>\n<li>DETO2X-SWEDEHEART Einschluss war SpO2 &gt;90% und dann Sauerstoffapplikation mit 6LO2\/min \u00fcber eine offene Gesichtsmaske vs. Raumluft und AVOID Einschluss war SpO2 &gt;94% und Sauerstoffapplikation mit 8lO2\/min \u00fcber eine geschlossene Gesichtsmaske wenn SpO2 &lt;94% mit Ziel 94%<\/li>\n<li>in DETO2X-SWEDEHEART fand sich eine geringe Inzidenz einer Hypox\u00e4mie in der Oxygengruppe und\u00a0keine gr\u00f6\u00dfere Infarktgr\u00f6\u00dfe (gemessen an der H\u00f6he des Troponins)<\/li>\n<\/ul>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Die Ergebnisse der\u00a0DETO2X-SWEDEHEART bei einem unselektioniertem Patientenkollektiv als in der AVOID-Studie nahe, dass Sauerstoff doch nicht so sch\u00e4dlich ist, wie nach AVOID angenommen.\u00a0<\/strong><\/p>\n<\/blockquote>\n<p>Eine interessante Diskussion findet sich auf:<\/p>\n<p><a href=\"https:\/\/dasfoam.org\/2017\/09\/01\/der-sauerstoff-sorgt-weiter-fuer-wirbel\/\" target=\"_blank\" rel=\"noopener\">https:\/\/dasfoam.org\/2017\/09\/01\/der-sauerstoff-sorgt-weiter-fuer-wirbel\/<\/a><\/p>\n<hr \/>\n<p>Weitere Literatur zum Thema:<\/p>\n<blockquote>\n<ul>\n<li><strong>Khoshnood A, et al.
