Resuscitation 2017, online<\/strong><\/p><\/blockquote>\n<hr>\n<p style=\"text-align: justify;\">Benannt wurde das <strong>LAZARUS-Ph\u00e4nomen<\/strong> nach &nbsp;Lazarus von Bethanien, einem Freund von Jesus, der vier Tage nach seinem Tod durch Jesus von den Toten wiedererweckt wurde (Johannesevangelium).<\/p>\n<p style=\"text-align: justify;\">In der Medizin bezeichnet das LAZARUS-Ph\u00e4nomen, den Wiedereintritt einer spontanen Kreislauffunktion nach dem bewussten Beenden von Reanimationsmassnahmen.<\/p>\n<hr>\n<p style=\"text-align: justify;\">In einer prospektiven Kohortenuntersuchung gingen Kuisma et al. der Frage nach, wie h\u00e4ufig das LAZARUS-Ph\u00e4nomen in Finnland auftrat. Es wurden 2.102 Patienten mit Herzkreislaufstillstand eingeschlossen, hierbei wurde in 1.376 F\u00e4llen eine Reanimation begonnen. In 840 F\u00e4llen wurde pr\u00e4hospital die Reanimation eingestellt (61%, cave: kein Fall von Hypothermie!). W\u00e4hrend der sechsj\u00e4hrigen Beobachtungsphase trat das Lazarus-Ph\u00e4nomen bei insgesamt 5 Patienten auf. D.h. bei 5 Patienten (0,6%, 5\/840) trat verz\u00f6gert, aber binnen 10 min nach Beendigung von Reanimationsmassnahmen, eine Wiedereintritt einer spontanen Kreislauffunktion auf, so dass ein Puls (ROSC) zu tasten war. Der ROSC trat in 3 F\u00e4llen nach 3 min und jeweils in einem Fall nach 6 bzw. 8 min auf. Drei der Patienten mit LAZARUS-Ph\u00e4nomen verstarben an der Einsatzstelle in einem Zeitraum von 2 bis 15 min und zwei Patienten verstarben im Krankenhaus nach 1,5 bis 26 Stunden.<\/p>\n<hr>\n<p><strong>Was lernen wir aus dieser Kohortenstudie?<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify;\">Das LAZARUS-Ph\u00e4nomen tritt mit einer Inzidenz von 6\/1.000 F\u00e4llen nach Beendigung von Reanimationsma\u00dfnahmen auf.<\/li>\n<li style=\"text-align: justify;\">Kein Patient mit LAZARUS-Ph\u00e4nomen \u00fcberlebte (in dieser Studie) langfristig dieses Ereignis.<\/li>\n<li style=\"text-align: justify;\">10 Minuten nach Beendigung einer Reanimation trat kein LAZARUS-Ph\u00e4nomen mehr auf. Die Empfehlung zur weiteren Monitorkontrolle \u00fcber einen Zeitraum von 10 min nach Beendigung der Reanimation, unter Diskonnektion der Beatmung, erscheint sinnvoll.<\/li>\n<li>Die Ursache f\u00fcr das LAZARUS-Ph\u00e4nomen ist weiterhin unklar, diskutiert werden hier:\n<ul>\n<li style=\"text-align: justify;\">Auto-PEEP (erst nach Diskonnektion der Beatmung wird der Auto-PEEP reduziert und es kann wieder ein R\u00fcckfluss zum Herzen stattfinden)<\/li>\n<li style=\"text-align: justify;\">myokardiale Reperfusion<\/li>\n<li style=\"text-align: justify;\">myokardiales Stunning<\/li>\n<li style=\"text-align: justify;\">Alkalose<\/li>\n<li>verz\u00f6gerte Wirkung von (unter Reanimation gegebener) Medikamente<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr>\n<p><strong>Warum ist das LAZARUS-Ph\u00e4nomen von Interesse?<\/strong><\/p>\n<p style=\"text-align: justify;\">Wird eine kardiopulmonale Reanimation beendet, so trifft dies alle an der Notfallversorgung Beteiligten gleichermassen. Wiederbelebungsversuche wurden beendet, eine traurige Nachricht muss den Angeh\u00f6rigen \u00fcberbracht werden. Und dann passiert es: der Patient weist wieder einen Puls auf. Dies kann zu starker Verunsicherung im Team , negative Pressemitteilungen, gar juristische Auseinandersetzungen f\u00fchren, und setzt vor allem ein psychologisches Trauma bei Rettern und Angeh\u00f6rigen. Wichtig ist zu wissen, dass es das LAZARUS-Ph\u00e4nomen gibt und dann professionell darauf zu reagieren. Ob wirklich jemand das Auftreten des LAZARUS-Ph\u00e4nomen neurologisch intakt \u00fcberlebt ist unklar bzw. ist hierf\u00fcr die Datenlage unbefriedigend.
