Autopuls, LUCAS) werden zunehmend gerne und h\u00e4ufig im Rahmen der kardiopulmonalen Reanimation pr\u00e4hospital und innerklinisch eingesetzt. Dabei muss man die Einf\u00fchrung einer &#8222;neuen&#8220; Technik immer kritisch, aber wohlwollend begleiten. Auf <strong><a href=\"http:\/\/news-papers.eu\" target=\"_blank\" rel=\"noopener\">news-papers.eu<\/a><\/strong> haben wir daher einige Artikel der letzten Monate besprochen:<\/p>\n<ul>\n<li><strong><a href=\"http:\/\/news-papers.eu\/beatmung-unter-mechanischen-reanimationshilfen-ein-problem\" target=\"_blank\" rel=\"noopener\">Beatmung unter automatischen Reanimatuionsger\u00e4ten<\/a><\/strong><\/li>\n<li><strong><a href=\"http:\/\/news-papers.eu\/thoraxkompressionsgeraete-differenziert-betrachtet\" target=\"_blank\" rel=\"noopener\">Ergebnisse unter automatischen Reanimationsger\u00e4ten &#8211; differenziert betrachtet<\/a><\/strong><\/li>\n<li><strong><a href=\"http:\/\/news-papers.eu\/neue-daten-zur-mechanischen-reanimation\" target=\"_blank\" rel=\"noopener\">Neue Daten zu mechanischen Reanimationsger\u00e4ten<\/a><\/strong><\/li>\n<li><strong><a href=\"http:\/\/news-papers.eu\/linc-studie-weitere-ergebnisse\" target=\"_blank\" rel=\"noopener\">LINC: Neue Ergebnisse<\/a><\/strong><\/li>\n<\/ul>\n<hr \/>\n<p style=\"text-align: justify;\">Nun muss man nicht nur nach Effektivit\u00e4t und neurologischen Behandlungsergebnis der kardiopulmonalen Reanimation schauen, sondern auch nach den Problemen, die mit den Reanimationsger\u00e4ten auftreten k\u00f6nnen:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Kosten RW et al.\u00a0Safety of mechanical chest compression devices\u00a0AutoPulse and LUCAS in cardiac arrest: a\u00a0randomized clinical trial for non-inferiority.\u00a0European Heart Journal (2017) 0, 1\u20138. \u00a0doi:10.1093\/eurheartj\/ehx318<\/strong><\/p>\n<\/blockquote>\n<p style=\"text-align: justify;\">In dieser prospektiven randomisierten Studien von Koster et al. wurden 1697 Patienten eingeschlossen. 574 wurden mittels <a href=\"http:\/\/www.zoll.com\/de\/produkte\/autopulse\/\" target=\"_blank\" rel=\"noopener\"><strong>AutoPulse<\/strong><\/a> reanimiert, 559 Patienten mittels <a href=\"http:\/\/www.physio-control.com\/uploadedFiles\/countries\/Germany\/LUCAS2_BROCHURE_3303951_C.pdf\" target=\"_blank\" rel=\"noopener\"><strong>LUCAS<\/strong><\/a> und 564 mit <strong><a href=\"http:\/\/www.philips.de\/healthcare\/product\/HCM3535A\/philips-heartstart-mrx-defibrillatormonitor-monitor-und-defibrillator\" target=\"_blank\" rel=\"noopener\">Philips MRx<\/a><\/strong>. Nat\u00fcrlich k\u00f6nnen nicht all diese Patienten nachverfolgt werden.\u00a0Unter Ber\u00fccksichtigung von Ausschlusskriterien verblieben zur Analyse 115 Patienten in der AutoPulse-Gruppe, 122 Patientsen in der LUCAS-Gruppe und 137 manuell reanimierte Patienten. Letztendlich konnte ein entstandener Schaden im Rahmen der Reanimation durch eine <strong>post-mortem Computertomographie (CT)<\/strong>, eine <strong>Autopsie<\/strong> oder unter Betrachtung des <strong>weiteren klinischen Verlaufs<\/strong> bei 103 Patienten der AutoPulse-Gruppe,\u00a0108 Patienten der LUCAS-Gruppe, und 126 Patienten mit manuellen Thoraxkompressionen analysiert werden.<\/p>\n<p>Schwerwiegende <strong>Verletzungen visceraler Organe<\/strong> erlitten:<\/p>\n<ul>\n<li>12 der 103 AutoPulse-Patienten (11,6%)<\/li>\n<li>8 der 108 LUCAS-Patienten (7,4%)<\/li>\n<li>und 8 der 126 Patienten mit manuellen Thoraxkompressionen (6,4%).<\/li>\n<\/ul>\n<p><strong>Rippen- oder Sternumfrakturen<\/strong> erlitten:<\/p>\n<ul>\n<li>47 der 103 AutoPulse Patienten (45,6%), davon 3 Sternumfrakturen<\/li>\n<li>43 der 108 LUCAS Patienten (39,8%), davon 7 Sternumfrakturen<\/li>\n<li>und 52 der 126 Patienten mit manuellen Thoraxkompressionen (41,3%), davon 5 Sternumfrakturen<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Klar <strong>zuordnungsbare Todesf\u00e4lle<\/strong> aufgrund von Verletzungen, die im Rahmen der Reanimation entstanden fanden sich in drei Patienten:<\/p>\n<ul>\n<li>2 Patienten hatten beim LUCAS Leberrupturen mit Passivblutungen<\/li>\n<li>und ein Patient, der mit AutoPulse-behandelt wurde hatte einen Spannungspneumothorax mit Luftembolie<\/li>\n<\/ul>\n<hr \/>\n<p>In der <strong>Diskussion<\/strong> f\u00fchren die Autoren folgende Punkte an:<\/p>\n<ul>\n<li style=\"text-align: justify;\">Die erste Stunde zum AutoPulse 2006 (ASPIRE) wurde vorzeitig abgebrochen, da eine zu hohe Letalit\u00e4t (&#8222;trend of excess mortality&#8220;) bei mit AutoPulse-behandelte Patienten vorlag.<\/li>\n<li style=\"text-align: justify;\">Die CIRC-Studie zeigte keine \u00dcberlebensvorteil f\u00fcr die AutoPulse-Anwendung gegen\u00fcber manuellen Thoraxkompressionen.<\/li>\n<li style=\"text-align: justify;\">Die LINC- und die PARAMEDIC-Studie zeigte keinen \u00dcberlebensvorteil f\u00fcr die LUCAS-Anwendung\u00a0gegen\u00fcber manuellen Thoraxkompressionen.<\/li>\n<li style=\"text-align: justify;\">Die AHA- und ERC-Leitlinien ziehen die Anwendung von LUCAS und AutoPulse in Betracht, bei l\u00e4ngeren Transporten und bei Anwendungen zur Reanimation im Herzkatheterlabor.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Als Ursachen f\u00fcr den bisher fehlenden Nachweis einer \u00dcberlegenheit von automatischen Reanimationsdevicen f\u00fchren die Autoren an:<\/p>\n<ul>\n<li style=\"text-align: justify;\">zu lange Anbringungszeiten bis zur maschinellen Thoraxkompression<\/li>\n<li style=\"text-align: justify;\">Verr\u00fccken der vulnerablen Devices vom Thorax hin zum Abdomen bei Lagerungsman\u00f6vern<\/li>\n<li style=\"text-align: justify;\">zu kr\u00e4ftige Kompressionen<\/li>\n<\/ul>\n<p>Vorhergehende Studien zu Reanimationssch\u00e4den fanden folgende Konstellation:<\/p>\n<ul>\n<li>mehr Reanimation-assoziierte Sch\u00e4den mit LUCAS vs. manuelle Thoraxkompression (28 vs. 19%, p=n.s., Smekal et al.)<\/li>\n<li>mehr\u00a0Reanimation-assoziierte Sch\u00e4den mit manuellen Thoraxkompressionen vs. LUCAS (25 vs. 19%, p=n.s., Lardi et al.)<\/li>\n<li>f\u00fcr abdominelle Verletzungen bei AutoPulse-Anwendung (3 vs 1%, n=n.s., Pinto et al.)<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Dabei muss aber konstatiert werden, dass diese Studien alle \u00fcber relevante Limitationen verf\u00fcgen. Auch im Rahmen der vorliegenden Studien von Koster et al. gibt es Imitationen, z.B. werden in post-mortem CTs erfahrungsgem\u00e4ss h\u00e4ufiger Rippenfrakturen detektiert als bei der Autopsie. Ebenso ist zu ber\u00fccksichtigen, dass mehr Patienten nach manuellen Thoraxkompressionen \u00fcberlebten (50%) im Vergleich zu AutoPulse (37%) oder LUCAS (34%), dies ist aber keine Ausdruck der \u00dcberlegenheit da andere Gr\u00fcnde (z.B. schlechtere Ausgangsbedingungen in den mit automatischen Reanimation-Device behandelten Gruppen) f\u00fcr die Letalit\u00e4t verantwortlich sein k\u00f6nnen. Aber dieses Ergebnis kann die Aussagen der Studie von <strong>Koster et al.<\/strong> beeinflussen, da bei \u00fcberlebenden h\u00e4ufiger die klinischen Verl\u00e4ufe als Beurteilsmasstab und keine CT oder Autopisedaten genutzt wurde.<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Die Autoren schlussfolgern aus ihrer Studie unter Ber\u00fccksichtigung aller genannten Limitationen, dass LUCAS keine h\u00f6here Komplikationsrate als manuelle Thoraxkompressionen haben, aber dass es nicht ausgeschlossen werden kann, dass AutoPulse zu mehr Sch\u00e4den f\u00fchrt.
