Nun legten <strong>Ellie M. Crompton<\/strong> und Kollegen eine neue Metaanalyse zu diesem Thema auf:<\/p>\n<blockquote><p><strong>Crompton EM, et al. Meta-Analysis of Therapeutic Hypothermia for\u00a0Traumatic Brain Injury in Adult and Pediatric Patients.\u00a0Crit Care Med 2017; 45:575\u2013583<\/strong><\/p><\/blockquote>\n<hr \/>\n<p>Kurz zusammengefasst:<\/p>\n<ul>\n<li>in ihrer Analyse fanden die Autoren 41 Studien mit erwachsenen Patienten (n=3,109; Alter: 18\u201381 Jahre) and 8 Studien mit p\u00e4diatrischen Patienten (n=454; Alter: 3 Monate-18 Jahre)<\/li>\n<li><strong>Bei Erwachsenen<\/strong> fand sich im Vergleich mit normothermen Patienten\n<ul>\n<li>eine <strong>18% Reduktion der Mortalit\u00e4t<\/strong> bei Anwendung einer Hypothermie<br \/>\n(Risk Ratio: 0,82; 95% CI, 0,70\u20130,96; p=0,01)<\/li>\n<li>eine <strong>35% Verbesserung im neurologischen Behandlungsergebnis<\/strong><br \/>\n(Risk Ratio: 1,35; 95% CI, 1,18\u20131,54; p&lt;0,00001).<\/li>\n<li>die optimale Behandlungsstrategie bei erwachsenen Patienten ist eine minimale Temperatur von <strong>33\u00b0C f\u00fcr 72 Stunden<\/strong> mit nachfolgender spontaner (nat\u00fcrlicher) Wiedererw\u00e4rmung<\/li>\n<\/ul>\n<\/li>\n<li><strong>Bei p\u00e4diatrischen Patienten<\/strong> zeigte sich ein gegenteiliges Ergebnis mit\n<ul>\n<li>einem <strong>66% Anstieg in der Mortalit\u00e4t<\/strong><br \/>\n(Risk ratio, 1,66; 95% CI, 1,06\u20132,59; p=0,03)<\/li>\n<li>einer <strong>nur marginalen Verschlechterung des neurologischen Behandlungsergebnises<\/strong><br \/>\n(Risk Ratio, 0,90; 95% CI, 0,80\u20131,01; p=0,06).<\/li>\n<\/ul>\n<\/li>\n<li><strong>Schlussfolgerung der Autoren:<\/strong> Hypothermia ist bei erwachsenen Patienten mit Sch\u00e4del-Hirntrauma als benefiziell anzunehmen, kann aber bei Kindern nicht empfohlen werden.<\/li>\n<\/ul>\n<hr \/>\n<p><strong><a href=\"https:\/\/twitter.com\/news_papers_eu?refsrc=email&amp;s=11\" target=\"_blank\" rel=\"noopener noreferrer\">News-Papers.eu auf TWITTER<\/a>\u00a0 \u00a0 \u00a0 \u00a0\u00a0<a href=\"http:\/\/news-papers.eu\/follow\" target=\"_blank\" rel=\"noopener noreferrer\">Follow news-papers.eu<\/a><\/strong><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Hypothermie wird schon seit l\u00e4ngeren im Rahmen der Intensivtherapie beim isolierten Sch\u00e4del-Hirntrauma (SHT) diskutiert.
