Anaesthesist 2017;\u00a0DOI 10.1007\/s00101-017-0299-z<\/strong><\/p><\/blockquote>\n<hr \/>\n<p>Hier einige f\u00fcr die Notfallmedizin relevante Aspekte und Empfehlungen aus der <strong>Surviving Sepsis Campaign<\/strong> (SSC 2016), die seit 2004\u00a0alle 4 Jahre \u00fcberarbeitet werden:<\/p>\n<blockquote><p><span style=\"color: #993300;\"><strong>Definition:\u00a0Sepsis ist nun definiert als lebensgef\u00e4hrdende\u00a0Organdysfunktion verursacht\u00a0durch eine fehlregulierte Wirtsantwort\u00a0auf eine Infektion.<\/strong><\/span><\/p><\/blockquote>\n<hr \/>\n<p><span style=\"color: #800000;\"><strong>INITIALE VERSORGUNG<\/strong><\/span><\/p>\n<ul>\n<li><strong>Therapiebeginn ohne Verz\u00f6gerung <\/strong>(Sepsis und septischer Schock sind medizinische Notf\u00e4lle und es wird der sofortige Beginn der Behandlung empfohlen)<\/li>\n<li><strong>Volumentherapie mindestens\u00a030 ml\/kgKG kristalline L\u00f6sungen innerhalb der ersten 3 h <\/strong>im Rahmen der Sepsis-induzierten Hypotension\n<ul>\n<li>Volumenbelastungsversuche (\u201efluid challenge\u201c)<\/li>\n<li>bei hohem Volumenbedarf zus\u00e4tzlich Humanalbumin<\/li>\n<li>keine Verwendung von Hydroxy\u00e4thylst\u00e4rke<\/li>\n<li>Kristalloide \u00fcberlegen gegen\u00fcber Gelatinepr\u00e4paraten<\/li>\n<\/ul>\n<\/li>\n<li>nach der initialen Volumengabe sollte eine weiter Volumengabe durch die wiederholte Evaluation des h\u00e4modynamischen Status geleitet werden.<\/li>\n<li>erweitertes h\u00e4modynamisches Monitoring bei unklarer klinischer Diagnose<\/li>\n<li>Bevorzugte Anwendung dynamischer Parameter des h\u00e4modynamischen Monitorings<\/li>\n<li><strong>Ziel:mittlerer arterieller Druck &gt;65mmHg<\/strong>\n<ul>\n<li>Noradrenalin als 1. Vasopressor<\/li>\n<li>Vasopressin (bis 0,03 U\/min) als Erg\u00e4nzung zu Noradrenalin<\/li>\n<li>Adrenalin als Erg\u00e4nzung zu Noradrenalin<\/li>\n<li>Dopamin nur in Ausnahmef\u00e4llen<\/li>\n<li>keine Anwendung von Dopamin in \u201eNierendosis\u201c<\/li>\n<li>Dobutamin bei Hypoperfusion trotz ausreichender Volumenbelastung und Vasopressorentherapie<\/li>\n<\/ul>\n<\/li>\n<li><strong>Ziel: Normalisierung der Laktatkonzentration<\/strong><\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Eine Reevaluation soll durch klinische Untersuchung und ein Monitoring erfolgen, dass die Herzfrequenz, den Blutdruck, die arterielle Oxygenierung, die Atemfrequenz, die Temperatur, die Urinausscheidung und andere beinhaltet. Invasives und nicht-invasives Monitoring soll genutzt werden.<\/p>\n<hr \/>\n<p style=\"text-align: justify;\"><span style=\"color: #800000;\"><strong>SCREENING<\/strong><\/span><\/p>\n<ul>\n<li style=\"text-align: justify;\">Screening von station\u00e4ren Hochrisikopatienten als Routine<\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #800000;\"><strong>DIAGNOSE<\/strong><\/span><\/p>\n<ul>\n<li>mikrobiologische Diagnostik, einschlie\u00dflich mind. zwei Paar Blutkulturen (aerob und anaerob) vor Antibiotikagabe, falls ohne relevante Verz\u00f6gerung vor antimikrobieller Therapie m\u00f6glich<\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #800000;\"><strong>ANTIMIKROBIELLE THERAPIE<\/strong><\/span><\/p>\n<ul>\n<li>antimikrobielle Therapie <strong>schnellstm\u00f6glich, innerhalb 1 h<\/strong> (\u201eso schnell\u00a0wie m\u00f6glich nach Erkennen und innerhalb\u00a0der ersten Stunde von Sepsis und\u00a0septischen Schock\u201c)<\/li>\n<li><strong>initial kalkulierte antimikrobielle Breitspektrumtherapie<\/strong> gegen Bakterien und ggf.
