August 2016 im <strong>British Medical Journal<\/strong> bringt es auf den Punkt: <strong>&#8222;Think Sepsis&#8220;<\/strong>:<\/p>\n<blockquote><p><strong>Freitag A, et al.\u00a0Suspected sepsis: summary of NICE guidance.\u00a0BMJ 2016;354:i4030 doi: 10.1136\/bmj.i4030<\/strong><\/p><\/blockquote>\n<p style=\"text-align: justify;\">Die aktuellen Ausf\u00fchrungen zur <strong><a href=\"http:\/\/www.survivingsepsis.org\/SiteCollectionDocuments\/SSC-Statements-Sepsis-Definitions- 3-2016.pdf\" target=\"_blank\">Sepsisdefinition<\/a> <\/strong>sind nicht f\u00fcr alle Anwender einfach zu verstehen und umzusetzen. Die aktuelle Zusammenfassung der <strong><a href=\"http:\/\/www.nice.org.uk\/guidance\/ng51\" target=\"_blank\">NICE Guideline zur Sepsis<\/a>\u00a0<\/strong>hat es sich zum Ziel gesetzt, dass fr\u00fche Erkennen der Sepsis zu optimieren: Daher heisst es hier:<\/p>\n<ul>\n<li><strong>\u201cThink sepsis\u201d<\/strong> bei jedem Patienten mit dem V.a. eine Infektion.<\/li>\n<li>Sepsis kann sich mit <strong>unspezifischen Symptomen und Zeichen<\/strong> und <strong>ohne Fieber<\/strong> pr\u00e4sentieren.<\/li>\n<li>Ein besonderes Ma\u00df an Aufmerksamkeit hinsichtlich Sepsis muss man bei Patienten im Alter <strong>&lt;1 Jahr<\/strong> oder <strong>&gt; 75 Jahren<\/strong>, <strong>Schwangere<\/strong>, <strong>immunsuppressive Patienten<\/strong> und solchen mit <strong>implantierten Devices<\/strong> oder aktuell <strong>erfolgten chirurgischen Massnahmen<\/strong> zu kommen lassen.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Die Autoren der <strong>NICE-Empfehlung<\/strong> f\u00fchren aus, dass sich eine Sepsis atypisch pr\u00e4sentieren kann und bei Patienten, die sich schlecht f\u00fchlen in Betracht gezogen werden muss. Insbesondere soll bei Patienten, die ein <strong>ver\u00e4ndertes Verhalten<\/strong> (&#8222;changes in behaviour&#8220; und &#8222;new altered mental state&#8220;) zeigen, gro\u00dfz\u00fcgig hinsichtlich einer Sepsis evaluiert werden. Eine <strong>Tachykardie<\/strong> wird als Hochrisikomarker einer Sepsis betrachtet. Auch muss ber\u00fccksichtigt werden, dass sich septische Patienten ohne Fieber pr\u00e4sentieren k\u00f6nnen, insbesondere \u00e4ltere Patienten, Patienten unter Krebstherapie.<\/p>\n<hr \/>\n<p style=\"text-align: justify;\">Folgende Aspekte\u00a0werden als besondere Faktoren betrachtet, die mit einer Sepsis einhergehen k\u00f6nnen. Bewusst wird hier der englische Wortlaut belassen:<\/p>\n<p>People in the following groups are at increased risk of developing sepsis:<\/p>\n<ul>\n<li><strong>People &lt;1 year old or &gt;75 years old, or very frail people<\/strong><\/li>\n<li><strong>People who have impaired immune systems because of illness or drugs:<\/strong>\n<ul>\n<li>Chemotherapy for cancer treatment<\/li>\n<li>Impaired immune function (such as those with diabetes or sickle cell disease, or people who have had a splenectomy)<\/li>\n<li>Long term treatment with corticosteroids<\/li>\n<li>Treatment with immunosuppressant drugs for non-malignant disorders, such as rheumatoid arthritis<\/li>\n<\/ul>\n<\/li>\n<li><strong> People who have had surgery, or other invasive procedures, in the past six weeks<\/strong><\/li>\n<li><strong>People with any breach of skin integrity (such as cuts, burns, blisters, or skin infections)<\/strong><\/li>\n<li><strong>Injecting drug misuse<\/strong><\/li>\n<li><strong>People with indwelling lines or catheters.<\/strong><\/li>\n<li><strong>Women who are pregnant or have given birth or had a termination of pregnancy or miscarriage in the past six weeks, in particular,\u00a0women who<\/strong>\n<ul>\n<li>Have impaired immune systems because of illness or drugs<\/li>\n<li>Have gestational diabetes or diabetes or other comorbidities<\/li>\n<li>Needed invasive procedures (such as caesarean section, forceps delivery, removal of retained products of conception)<\/li>\n<li>Had a prolonged rupture of membranes<\/li>\n<li>Either have or have been in close contact with people with group A streptococcal infection (such as scarlet fever)<\/li>\n<li>Have continued vaginal bleeding or an offensive vaginal discharge<\/li>\n<\/ul>\n<\/li>\n<li><strong>Neonates, particularly if there has been:<\/strong>\n<ul>\n<li>Invasive group B streptococcal infection in a previous baby<\/li>\n<li>Maternal group B streptococcal colonisation, bacteriuria, or infection in the current pregnancy<\/li>\n<li>Prelabour rupture of membranes<\/li>\n<li>Preterm birth after spontaneous labour (before 37 weeks\u2019 gestation)<\/li>\n<li>Suspected or confirmed rupture of membranes for &gt;18 hours in a preterm birth<\/li>\n<li>Maternal intrapartum fever &gt;38\u00b0C, or confirmed or suspected chorioamnionitis<\/li>\n<li>Parenteral antibiotic treatment given to the woman for confirmed or suspected invasive bacterial infection (such as septicaemia) at any time during labour or in the 24 hour periods before and after the birth (this does not refer to intrapartum antibiotic prophylaxis)<\/li>\n<li>Suspected or confirmed infection in another baby in the case of a multiple pregnancy<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<p>Schauen Sie sich doch einmal die NICE-Toolkits an (<strong><a href=\"http:\/\/sepsistrust.org\/clinical-toolkit\/\" target=\"_blank\">hier<\/a><\/strong>).<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Eine aktuelle Publikation vom 11.
