Allerdings sind blutende Patienten im milit\u00e4rischen Einsatz in der Regel jung, gesund und nicht antikoaguliert. Das stellt sich im zivilen Rettungsdienst v\u00f6llig anders dar: viele gerade \u00e4ltere Patienten haben eine gerinnungshemmende Dauermedikation.<\/p>\n<div dir=\"auto\" style=\"text-align: justify;\">Raimund Lechner, Anna Schmid und Katharina Hanke aus der Klinik f\u00fcr An\u00e4sthesiologie, Intensivmedizin, Notfallmedizin u. Schmerztherapie des Ulmer Bundeswehrkrankenhauses haben Blutproben von antikoagulierten Patienten mit verschiedenen H\u00e4mostyptika (QuikClot Gaze, Celox Granulat, Celox-Gaze, Chito SAM 100, WoundClot-Trauma-Gaze, QuikClot-Gaze Moulage Trainer) versetzt und mit Hilfe der Rotationsthrombelastometrie gemessen. Betrachtet wurden Patienten unter Enoxaparin, Heparin + Acetylsalicyls\u00e4ure, Apixaban und Phenprocoumon (Marcumar).<\/div>\n<blockquote>\n<div dir=\"auto\" style=\"text-align: justify;\">Lechner R, Hanke K, Schmid A, et al.<\/div>\n<div dir=\"auto\" style=\"text-align: justify;\">Hemostatics in patients with inhibited coagulation \u2014 A viscoelastic in-vitro analysis.<\/div>\n<div dir=\"auto\" style=\"text-align: justify;\">Transfusion 2023. <a href=\"https:\/\/doi.org\/10.1111\/trf.17333\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/doi.org\/10.1111\/trf.17333<\/a><\/div>\n<\/blockquote>\n<div dir=\"auto\" style=\"text-align: justify;\">Ergebnis:<\/div>\n<div dir=\"auto\" style=\"text-align: justify;\">Alle getesteten Wirkstoffe beschleunigten die Gerinnung unter den gepr\u00fcften Antikoagulanzien, meist in erheblichem Ma\u00dfe. Die ausgepr\u00e4gtesten Verbesserungen wurden von Kaolin (QuikClot) und den Chitosan-Produkten (Celox Granules, Celox Gaze, Chito SAM 100) erzielt.<\/div>\n<div dir=\"auto\" style=\"text-align: justify;\">Im Hinblick auf die Antikoagulanzien wurden die deutlichsten Verbesserungen bei Enoxaparin beobachtet; gefolgt der Reihe nach von Apixaban, Heparin + Acetylsalicyls\u00e4ure und Phenprocoumon.<\/div>\n<blockquote>\n<div dir=\"auto\" style=\"text-align: justify;\">Die bisher angenommene These, dass vorallem Chitosan in antikoaguliertem Blut wirkt [1], wird nun durch diese Ergebnisse erg\u00e4nzt, die auch einen Einsatz von Kaolin im zivilen Rettungsdienst rechtfertigen.<\/div>\n<div dir=\"auto\"><\/div>\n<\/blockquote>\n<ol>\n<li style=\"text-align: justify;\">Koksal, O et al.
