Hier noch einmal eine kurze Zusammenfassung der EpiSEP Studie<\/strong>:<\/p>\n

Infektion, Sepsis und septischer Schock geh\u00f6ren zu den h\u00e4ufigsten Ursachen f\u00fcr die Behandlung in der Notaufnahme (ED). Die meisten verf\u00fcgbaren Daten stammen von Intensivstationen und schlie\u00dfen nosokomiale Infektionen ein, die w\u00e4hrend des Krankenhausaufenthalts erworben wurden. Epidemiologische Daten aus deutschen Notaufnahmen sind bisher nicht verf\u00fcgbar, obwohl die Notaufnahme eine der ersten Anlaufstellen f\u00fcr Patienten ist. Ziel dieser Studie war es, die Epidemiologie, Ursachen, Diagnose, Mortalit\u00e4t und Behandlung von Patienten mit Infektionen in der Notaufnahme zu untersuchen.<\/p>\n

Material und Methoden:<\/strong> In dieser retrospektiven Beobachtungsstudie an einem einzigen Zentrum wurden routinem\u00e4\u00dfig erfasste Daten aus dem Patientendatenverwaltungssystem und dem Krankenhausinformationssystem ausgewertet. Alle erwachsenen Patienten, die im Studienzeitraum vom 01.01. bis 28.02.2019 wegen einer Infektion in der Notaufnahme vorstellig wurden, wurden eingeschlossen. Ausschlusskriterien waren Alter \u2264 17 Jahre und unvollst\u00e4ndige Aufzeichnungen. Drei Gruppen (I. Infektion, II. Sepsis und III. septischer Schock) wurden gem\u00e4\u00df den SEPSIS-3-Definitionen definiert.<\/p>\n

Ergebnisse:<\/strong> W\u00e4hrend des Studienzeitraums wurden insgesamt 6.607 Patienten in der Notaufnahme behandelt. Von diesen Patienten hatten 19,3 % (n = 1.278) eine Infektion (Durchschnittsalter 56 \u00b1 23 Jahre, 50 % weiblich). Die Infektionsherde verteilten sich wie folgt: Atemwege 35 %, Urogenitaltrakt 18 %, Kiefer-Gesichtsbereich\/Ohren\/Nase\/Hals 14 %, intraabdominal 13 %, Weichteile 10 %, zentrales Nervensystem 1 %, andere Ursache 3 % oder unbekannte Ursache 6 %. Eine reine Infektion, eine Sepsis und ein septischer Schock lagen bei 86, 10 bzw. 3 % vor. Es gab signifikante Unterschiede bei den Vitalparametern sowie bei den verschiedenen Notfallsepsis-Scores zwischen den vordefinierten Gruppen [I vs. II vs. III: SOFA (pts.): 1 \u00b1 1 vs. 4 \u00b1 2 vs. 7 \u00b1 3 (p < 0,0001), systolischer Blutdruck (mmHg): 137 \u00b1 25 vs. 128 \u00b1 32 vs. 107 \u00b1 34 (p < 0,05), Herzfrequenz (bpm): 92 \u00b1 18 vs. 99 \u00b1 23 vs. 113 \u00b1 30 (p < 0,05), Atemfrequenz (min-1): 18 \u00b1 4 vs. 20 \u00b1 7 vs. 24 \u00b1 10 (p < 0,05)]. In den drei Gruppen wurden in 34, 81 bzw. 86 % der F\u00e4lle Blutkulturen angelegt und in 50, 89 bzw. 86 % der F\u00e4lle wurden in der Notaufnahme Antibiotika verabreicht. Die 30-Tage-Sterblichkeitsrate betrug in den drei Gruppen 1,6, 12,0 bzw. 38,1 %.<\/p>\n

Schlussfolgerung:<\/strong> Diese Studie ist die erste, die die H\u00e4ufigkeit, das Management und die Ergebnisse von Patienten, die als Infektion, Sepsis und septischer Schock klassifiziert werden, in einer deutschen Notaufnahme zeigt. Die Ergebnisse unserer Praxisdaten sind wichtig f\u00fcr das Qualit\u00e4tsmanagement und erm\u00f6glichen die Optimierung der Behandlungspfade f\u00fcr Patienten mit Infektionskrankheiten.<\/p>\n<\/div>\n","protected":false},"excerpt":{"rendered":"
