DOI: 10.19224\/ai2022.307 (<a href=\"https:\/\/www.ai-online.info\/images\/ai-ausgabe\/2022\/0708-2022\/AI_07-08-2022_CME_Landsleitner.pdf\" target=\"_blank\" rel=\"noopener\">PDF<\/a>)<\/strong><\/p>\n<\/blockquote>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<p style=\"text-align: justify\">Die S2k-Leitlinie<strong> \u201ePolytraumaversorgung im Kindesalter\u201c<\/strong> fokussiert auf die spezifischen Besonderheiten und stellt eine Erg\u00e4nzung der S3-Leitlinie \u201ePolytrauma\u2009\/\u2009Schwerverletzten-Behandlung\u201c dar. Zur <strong>Beurteilung kritisch verletzter Kinder<\/strong> eignen sich v.\u202fa. solche Parameter, die weitgehend unabh\u00e4ngig von altersentsprechenden Normalwerten, Kompensationsmechanismen und Stres\u00adsoren sind: Atemarbeit, Rekapillarisierungszeit, Vigilanz, Gesamteindruck.<\/p>\n<p style=\"text-align: justify\">Bei der <strong>pr\u00e4hospitalen Versorgung polytraumatisierter Kinder<\/strong> steht die Stabilisierung der Vitalparameter im Vordergrund: Normotonie, Normoxie, Normokapnie, Normothermie und Analgesie. Da diese Ziele h\u00e4ufig auch mit einfachen Interventionen erreichbar sind, ist die Indikation zur invasiven pr\u00e4hospitalen Therapie (z.\u202fB. Notfallnarkose und in\u00advasive Beatmung) streng zu stellen. Die <strong>Schockraumversorgung<\/strong> folgt der gleichen Systematik wie bei Erwachsenen, allerdings steht bei der Diagnostik zun\u00e4chst Ultraschall in Kombination mit klinischem Bild und Laborparametern im Vordergrund. Die Indikationsstellung zur Trauma-Spirale darf kein Automatismus sein, sondern erst nach Nutzen-\u2009\/\u2009Risiko-Beurteilung erfolgen.<\/p>\n<div class=\"page\" title=\"Page 2\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p><strong>Extremit\u00e4tenverletzungen:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Die Versorgung nicht-lebensbedrohlicher Extremita\u0308tenverletzungen soll weitere Scha\u0308den vermeiden, darf die Gesamtrettungszeit beim Polytrauma aber nicht verla\u0308ngern.<\/li>\n<li style=\"text-align: justify\">Aufgrund der bei Kindern bestehenden diagnostischen Unsicherheit soll jede auch nur vermutlich verletzte Extremita\u0308t vor Umlagerung bzw. Transport immobilisiert werden.<\/li>\n<li style=\"text-align: justify\">Nur bei grober Fehlstellung mit Kompromittierung von Weichteilen und\/oder Gefa\u0308\u00dfen sind Repositionsmano\u0308ver indiziert. Sie sollen unter Analgosedierung durch Zug und Gegenzug mit dem Ziel der Weichteilentlastung und ohne das Ziel einer anatomisch korrekten Reposition durchgefu\u0308hrt werden.<\/li>\n<li style=\"text-align: justify\">Bei polytraumatisierten Patienten soll \u2013 insbesondere bei Hochrasanztrauma \u2013 bis zum Beweis des Gegenteils von einer Wirbelsa\u0308ulenverletzung ausgegangen werden.<\/li>\n<li style=\"text-align: justify\">Bei Verdacht auf ein Beckentrauma sollte eine mechanische Stabilisierung durch Beckenkompression erfolgen. Hierzu eignen sich kommerzielle erha\u0308ltliche Notfallbecken- schlingen oder bei kleineren Kindern die Umschlingung mit einem Tuch ggf. kombiniert mit Einsatz der Vakuummatratze.<\/li>\n<li style=\"text-align: justify\">Im Zweifelsfall ist deshalb zugunsten der Versorgungssicherheit eine ho\u0308here Verletzungsschwere anzunehmen (\u201eU\u0308bertriagierung\u201c) und die Entscheidung zur Schockraumversorgung zu fa\u0308llen.<\/li>\n<\/ul>\n<p><strong>Schockraumteam:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Das Basis-Schockraumteam soll im u\u0308berregionalen Traumazentrum aus mindestens 3 A\u0308rzten bestehen: Chirurg (Kinderchirurg oder Traumatologe mit kindertraumatologischer Expertise), Ana\u0308sthesist und pa\u0308diatrischer Intensivmediziner. Ein Kinderradiologe oder Radiologe mit kinder- radiologischer Expertise und ein pa\u0308diatrisch erfahrener Neurochirurg\/neurochirurgisch erfahrener Kinderchirurg sollten verfu\u0308gbar sein.<\/li>\n<li style=\"text-align: justify\">Die Leitung des Schockraumteams soll sich an den strukturellen Gegebenheiten vor Ort orientieren und kann sowohl als interdisziplina\u0308re Fu\u0308hrungsgruppe als auch in Person eines Trauma-Leaders erfolgen.<\/li>\n<li style=\"text-align: justify\">Der u\u0308berwiegende Teil des Schockraumteams soll ein spezielles Schockraumtraining (z. B. Advanced Trauma Life Support ATLS\u00ae, European Trauma Course ETC, Definite Surgical Trauma Care DSTCTM) absolviert haben.<\/li>\n<li style=\"text-align: justify\">Im Kinderschockraum sollen alle fu\u0308r die Notfallversorgung erforderlichen Materialien\/Instrumente in sa\u0308mtlichen kinderspezifischen Gro\u0308\u00dfen vorgehalten werden.<\/li>\n<li style=\"text-align: justify\">In einem u\u0308berregionalen Kindertraumazentrum sollten pa\u0308diatrische Intensivmedizin und Kinderradiologie mit in die Schockraumversorgung eingebunden sein.<\/li>\n<li style=\"text-align: justify\">Im Rahmen des Primary Survey sollte eine eFAST zur Diagnostik von freier abdominaler Flu\u0308ssigkeit, von Perikard- und Pleura-Erguss oder Pneumothorax durchgefu\u0308hrt werden. Ein prima\u0308r negativer sonographischer Befund schlie\u00dft jedoch eine abdominale oder thorakale Verletzung nicht aus.<\/li>\n<li style=\"text-align: justify\">Bei Sa\u0308uglingen &lt;1 Jahr mit Scha\u0308del-Hirn-Trauma kann eine transfontanella\u0308re und transtemporale Scha\u0308delsonographie durchgefu\u0308hrt werden, wenn dadurch der weitere diagnostische Ablauf nicht verzo\u0308gert wird.<\/li>\n<li style=\"text-align: justify\">Die Indikationstellung zur Ganzko\u0308rper-Computertomographie soll beim polytraumatisierten Kind nach Nutzen-\/Risiko-Abwa\u0308gung, aufgrund definierter Kriterien und im Konsens des Traumateams erfolgen.
