DOI: 10.19224\/ai2022.307 (<a href=\"https:\/\/www.ai-online.info\/images\/ai-ausgabe\/2022\/0708-2022\/AI_07-08-2022_CME_Landsleitner.pdf\" target=\"_blank\" rel=\"noopener\">PDF<\/a>)<\/strong><\/p><\/blockquote>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<p style=\"text-align: justify\">Die S2k-Leitlinie<strong> \u201ePolytraumaversorgung im Kindesalter\u201c<\/strong> fokussiert auf die spezifischen Besonderheiten und stellt eine Erg\u00e4nzung der S3-Leitlinie \u201ePolytrauma\u2009\/\u2009Schwerverletzten-Behandlung\u201c dar. Zur <strong>Beurteilung kritisch verletzter Kinder<\/strong> eignen sich v.\u202fa. solche Parameter, die weitgehend unabh\u00e4ngig von altersentsprechenden Normalwerten, Kompensationsmechanismen und Stres\u00adsoren sind: Atemarbeit, Rekapillarisierungszeit, Vigilanz, Gesamteindruck.<\/p>\n<p style=\"text-align: justify\">Bei der <strong>pr\u00e4hospitalen Versorgung polytraumatisierter Kinder<\/strong> steht die Stabilisierung der Vitalparameter im Vordergrund: Normotonie, Normoxie, Normokapnie, Normothermie und Analgesie. Da diese Ziele h\u00e4ufig auch mit einfachen Interventionen erreichbar sind, ist die Indikation zur invasiven pr\u00e4hospitalen Therapie (z.\u202fB. Notfallnarkose und in\u00advasive Beatmung) streng zu stellen. Die <strong>Schockraumversorgung<\/strong> folgt der gleichen Systematik wie bei Erwachsenen, allerdings steht bei der Diagnostik zun\u00e4chst Ultraschall in Kombination mit klinischem Bild und Laborparametern im Vordergrund. Die Indikationsstellung zur Trauma-Spirale darf kein Automatismus sein, sondern erst nach Nutzen-\u2009\/\u2009Risiko-Beurteilung erfolgen.<\/p>\n<p><strong>Kurze Zusammenfassung wichtiger Punkte:<\/strong><\/p>\n<div class=\"page\" title=\"Page 2\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<ul>\n<li style=\"text-align: justify\">Das kindliche Polytrauma ist sowohl pra\u0308- als auch intrahospital ein seltener Notfall.<\/li>\n<li style=\"text-align: justify\">Ein polytraumatisiertes Kind sollte innerhalb einer Transportzeit von 30 Minuten in ein geeignetes Traumazentrum transportiert werden.<\/li>\n<li style=\"text-align: justify\">Die Behandlung kindlicher Polytraumata sollte in einem Traumzentrum erfolgen, das u\u0308ber spezielle Expertise fu\u0308r Kinder und eine Kinderintensivstation verfu\u0308gt.<\/li>\n<li style=\"text-align: justify\">Obgleich es bisher keinen Nachweis eines Vorteils f\u00fcr das Behandlungsergebnis gibt, empfiehlt sich ein handlungsorientiertes und algorithmenbasiertes Teamtraining in zertifizierten Traumakursen (z. B. Pre-Hospital Trauma Life Support PHTLS\u00ae, International Trauma Life Support ITLS).<\/li>\n<\/ul>\n<p><strong>Atemwegsmanagement, Beatmung und Notfallnarkose:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei polytraumatisierten Kindern mit Apnoe oder Schnappatmung sollen pra\u0308hospital Notfallnarkose, Atemwegssicherung (mittels Tubus oder Larynxmaske) und Beatmung durchgefu\u0308hrt werden.<\/li>\n<li style=\"text-align: justify\">Bei anderen Notfallsituationen wie z. B. Hypoxie und\/oder Hypoventilation trotz ausgescho\u0308pfter nicht-invasiver Therapie, schwerem Scha\u0308del-Hirn-Trauma in Kombination mit weiteren Verletzungen sowie trauma-assoziierter ha\u0308modynamischer Instabilita\u0308t sollte die Indikationsstellung zu Narkose und invasiver Beatmung nur nach sorgfa\u0308ltiger Nutzen-Risiko-Analyse erfolgen.<\/li>\n<li style=\"text-align: justify\">Die klinische Untersuchung des Thorax soll mindestens Inspektion und Palpation, die Bestimmung der Atemfrequenz und die Auskultation der Lunge umfassen.<\/li>\n<li style=\"text-align: justify\">Im Falle einer raschen Verschlechterung der Kreislaufsituation, einseitig abgeschwa\u0308chtem \/ fehlendem Atemgera\u0308usch unter kontrollierter Beatmung und Ausschluss anderer Ursachen (z. B. einseitige Tubuslage) soll ein Spannungspneumothorax angenommen und sofort behandelt werden.<\/li>\n<li style=\"text-align: justify\">Bis zum Kleinkindalter generell sowie fu\u0308r kritisch kranke oder verletzte, hypoxiegefa\u0308hrdete Kinder aller Altersklassen wird die pa\u0308diatrisch modifizierte kontrollierte Rapid Sequence Induction (RSI) empfohlen.<\/li>\n<li style=\"text-align: justify\">Als einfaches und sicheres \u201eKochrezept\u201c zur Durchfu\u0308hrung einer Notfallnarkose bei polytraumatisierten Kindern kann daher die Kombination von 2 mg\/kg Esketamin und 1 mg\/kg Rocuronium i.v. \/ i.o. gelten.<\/li>\n<li style=\"text-align: justify\">Wa\u0308hrend der Notfallnarkose und der anschlie\u00dfenden Weiterversorgung sollen EKG, Blutdruck, Sauerstoffsa\u0308ttigung und endtidales CO2(Kapnographie) u\u0308berwacht sowie die beatmungssynchrone Thoraxexkursion engmaschig klinisch gepru\u0308ft werden.
