Ann Emerg Med 2021; 77:296-304, <\/strong><a href=\"https:\/\/doi.org\/10.1016\/j.annemergmed.2020.10.016\"><strong>https:\/\/doi.org\/10.1016\/j.annemergmed.2020.10.016<\/strong><\/a><\/p>\n<\/blockquote>\n<p><strong>Methodik:<\/strong><\/p>\n<ul>\n<li>prospektive Vorher-Nachher-Beobachtungsstudie<\/li>\n<li>Paramedics des Seattle Fire Departments in den USA<\/li>\n<li>Eischlusskriterien: notfallm\u00e4\u00dfige pr\u00e4hospitale tracheale Intubationen nach Narkoseeinleitung sowie bei kardiopulmonaler Reanimation<\/li>\n<li>Kontroll-Periode:\n<ul>\n<li>Juli 2015 bis Dezember 2016<\/li>\n<li>konventionelle tracheale Intubation unter direkter Laryngoskopie<\/li>\n<li>ein Bougie wurde seit 2005 genutzt nur bei individueller Entscheidung des Paramedics (beispielsweise bei erwartet schwierigem Atemweg oder misslungenem ersten Versuch)<\/li>\n<li>823 Patienten<\/li>\n<\/ul>\n<\/li>\n<li>Trainings-Periode:\n<ul>\n<li>Januar 2017 bis M\u00e4rz 2017<\/li>\n<li>Einf\u00fchrung des verpflichtenden Einsatz eines Bougies bereits beim ersten Intubationsversuch (direkte Laryngoskopie) sowohl bei reanimationspflichtigen Patienten als auch nach Narkoseeinleitung<\/li>\n<li>135 Patienten<\/li>\n<\/ul>\n<\/li>\n<li>Bougie-Periode:\n<ul>\n<li>April 2017 bis September 2018<\/li>\n<li>standardm\u00e4\u00dfige Nutzung des Bougie bei allen Intubationen bereits im ersten Versuch<\/li>\n<li>771 Patienten<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Ergebnisse:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">der first pass success (FPS) verbesserte sich nach Einf\u00fchrung des Bougies signifikant von 70 auf 77%<\/li>\n<li style=\"text-align: justify\">diese signifikante Verbesserung des FPS konnte f\u00fcr alle Klassifikationen der Einsehbarkeit der Stimmb\u00e4nder nach Cormack und Lehane beobachtet werden<\/li>\n<li style=\"text-align: justify\">die Rate der Hypox\u00e4mien (definiert als SpO<sub>2<\/sub> &lt;90% zwischen Einleiting und 2 min nach Intubation) lag in der Kontroll-Periode bei 30%, in der Bougie-Periode bei 19% (signifikante Verbesserung)<\/li>\n<li style=\"text-align: justify\">in der H\u00e4ufigkeit der \u00f6sophagealen Fehlintubationen (definiert als unerkannte \u00f6sophageale Lage des Tubus \u00fcber &gt;5min, 3,0% vs. 2,0%) und der Herz-Kreislauf-Stillst\u00e4nde im Rahmen der Intubation (4,0% vs. 4,0%) wurde keine Ver\u00e4nderung beobachtet<\/li>\n<li style=\"text-align: justify\">Die mediane Anzahl der Intubationen pro Paramedic lag in der Kontroll-Periode bei 16 (Interquartilenabstand 7-21) und in der Bougie-Periode bei 15 (11-20)<\/li>\n<\/ul>\n<p><strong>Fazit:<\/strong><\/p>\n<p style=\"text-align: justify\">Im Unterschied zur ebenfalls k\u00fcrzlich publizierten Studie von Diver et al., die f\u00fcr innerklinische Notfall-Intubationen keinen Vorteil f\u00fcr den Einsatz eines Bougies nachwies, konnte diese Beobachtungsstudie ein deutliche Verbesserung der Qualit\u00e4tsindikatoren f\u00fcr das Airway-Management zeigen. Nat\u00fcrlich k\u00f6nnen im Lauf des Beobachtungszeitraums auch weitere Faktoren die Verbesserung im Airway-Management bewirkt haben (z.B. bessere Ausbildung, bessere Supervision). Dennoch k\u00f6nnte diese Studie darauf hinweisen, dass die standardm\u00e4\u00dfige Nutzung des Bougie und ein (vermutlich) sehr gutes Training (f\u00fcr das das Seattle Fire Department ber\u00fchmt ist) in dessen Anwendung, Vorteile bringen kann.
