(<a href=\"https:\/\/link.springer.com\/content\/pdf\/10.1007\/s00134-021-06506-y.pdf\" target=\"_blank\" rel=\"noopener\">PDF<\/a>)<\/strong><\/p>\n<\/blockquote>\n<p>Hier ganz frisch <strong>einige Empfehlungen<\/strong> (Cave: Freie \u00dcbersetzung: Korrekturen vorbehalten):<\/p>\n<\/div>\n<p><strong>Kernaussage 41:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit septischem Schock und kardialer Dysfunktion mit persistierender Hypoperfusion trotz ad\u00e4quatem Volumenstatus und arteriellem Blutdruck wird empfohlen, entweder Dobutamin zu Norepinephrin hinzuzuf\u00fcgen oder Epinephrin allein zu verwenden.<\/li>\n<\/ul>\n<p><strong>Kernaussage 42:<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit septischem Schock und kardialer Dysfunktion mit anhaltender Hypoperfusion trotz ad\u00e4quatem Volumenstatus und arteriellem Blutdruck raten wir von der Anwendung von Levosimendan ab.<\/li>\n<\/ul>\n<p><strong>Kernaussage 43: <\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit septischem Schock wird empfohlen, die invasive \u00dcberwachung des arteriellen Blutdrucks der nicht-invasiven \u00dcberwachung vorzuziehen, sobald dies praktikabel ist und die Ressourcen zur Verf\u00fcgung stehen.<\/li>\n<\/ul>\n<p><strong>Kernaussage 44: <\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit septischem Schock empfehlen wir die periphere Gabe von Vasopressoren, um den MAP wiederherzustellen, anstatt die Einleitung zu verz\u00f6gern, bis ein zentraler ven\u00f6ser Zugang gelegt wurde.\u00a0Wenn Vasopressoren peripher verabreicht werden, sollten sie nur f\u00fcr kurze Zeit und in eine Vene in oder proximal der Fossa antecubitalis verabreicht werden.<\/li>\n<\/ul>\n<p><strong>Kernaussagen 45: <\/strong><\/p>\n<ul>\n<li>Es liegen keine ausreichenden Erkenntnisse vor, um eine Empfehlung zur Anwendung restriktiver bzw. liberaler Fl\u00fcssigkeitsstrategien in den ersten 24 Stunden nach der Initialtherapie bei Patienten mit Sepsis und septischem Schock abzugeben, die nach der anf\u00e4nglichen Initialtherapie immer noch Anzeichen von Hypoperfusion und Volumendepletion aufweisen.\u00a0Eine Fl\u00fcssigkeitsreanimation sollte nur erfolgen, wenn der Patient Anzeichen einer Hypoperfusion aufweist.<\/li>\n<\/ul>\n<p><strong>Kernaussage 46:<\/strong><\/p>\n<ul>\n<li>Es liegen keine ausreichenden Erkenntnisse vor, um eine Empfehlung f\u00fcr die Verwendung konservativer Sauerstoffziele bei Erwachsenen mit sepsisbedingter hypox\u00e4mischer respiratorischer Insuffizienz abzugeben<\/li>\n<\/ul>\n<p><strong>Kernaussage 47: <\/strong><\/p>\n<ul>\n<li>Bei Erwachsenen mit Sepsis-induzierter hypox\u00e4mischer Ateminsuffizienz empfehlen wir die Verwendung von nasalem Sauerstoff mit hohem Durchfluss (High-Flow Nasal Calnula, HFNC) gegen\u00fcber nicht-invasiver Beatmung.<\/li>\n<\/ul>\n<p><strong>Kernaussage 48: <\/strong><\/p>\n<ul>\n<li>Es liegen keine ausreichenden Erkenntnisse vor, um eine Empfehlung f\u00fcr den Einsatz der nicht-invasiven Beatmung im Vergleich zur invasiven Beatmung bei Erwachsenen mit sepsisbedingter hypox\u00e4mischer respiratorischer Insuffizienz auszusprechen.<\/li>\n<\/ul>\n<p><strong>Kernaussage 49: <\/strong><\/p>\n<ul>\n<li>Bei Erwachsenen mit Sepsis-induziertem ARDS empfehlen wir eine Beatmungsstrategie mit niedrigem Tidalvolumen (6 mL\/kg) gegen\u00fcber einer Strategie mit hohem Tidalvolumen (&gt; 10 mL\/kg).<\/li>\n<\/ul>\n<p><strong>Kernaussage 50: <\/strong><\/p>\n<ul>\n<li>F\u00fcr Erwachsene mit Sepsis-induziertem schwerem ARDS wird ein oberer Grenzwert f\u00fcr den Plateaudruck von 30 cm H2O empfohlen, statt h\u00f6herer Plateaudr\u00fccke.<\/li>\n<\/ul>\n<p><strong>Kernaussage 51: <\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit mittelschwerem bis schwerem Sepsis-induziertem ARDS empfehlen wir die Verwendung eines h\u00f6heren PEEP gegen\u00fcber einem niedrigeren PEEP.<\/li>\n<\/ul>\n<p><strong>Kernaussage 52: <\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit sepsisbedingtem Atemversagen (ohne ARDS) empfehlen wir die Beatmung mit niedrigem Tidalvolumen im Vergleich zur Beatmung mit hohem Tidalvolumen.<\/li>\n<\/ul>\n<p><strong>Kernaussage 53:\u00a0<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">F\u00fcr Erwachsene mit Sepsis-induziertem mittelschwerem ARDS empfehlen wir die Anwendung traditioneller Rekrutierungsman\u00f6ver.<\/li>\n<\/ul>\n<p><strong>Kernaussage 54:\u00a0<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei der Anwendung von Rekrutierungsman\u00f6vern empfehlen wir, keine inkrementelle PEEP-Titration\/-strategie anzuwenden.<\/li>\n<\/ul>\n<p><strong>Kernaussage 55:\u00a0<\/strong><\/p>\n<ul>\n<li style=\"text-align: justify\">Bei Erwachsenen mit durch Sepsis verursachtem mittelschwerem ARDS empfehlen wir die Beatmung in Bauchlage f\u00fcr mehr als 12 Stunden t\u00e4glich.<\/li>\n<\/ul>\n<p><strong>Kernaussage 56:<\/strong><\/p>\n<ul>\n<li>Bei Erwachsenen mit durch Sepsis verursachtem mittelschwerem ARDS empfehlen wir die Verwendung von intermittierenden NMBA-Bolusinfusionen anstelle einer NMBA-Dauerinfusion.<\/li>\n<\/ul>\n<p><strong>Kernaussage 57:\u00a0<\/strong><\/p>\n<ul>\n<li>F\u00fcr Erwachsene mit Sepsis-induziertem schwerem ARDS empfehlen wir die ven\u00f6s-ven\u00f6se (VV) ECMO, wenn die konventionelle mechanische Beatmung in erfahrenen Zentren versagt, die \u00fcber die n\u00f6tige Infrastruktur f\u00fcr den Einsatz verf\u00fcgen.<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>Nun zu Teil 6 der\u00a0Surviving Sepsis Campaign Guideline 2021 ist am 02.10.2021 publiziert worden: Evans L, et al.
