Vigilanzminderung<\/strong> (tiefe Somnolenz, Sopor oder Koma) ist eine h\u00e4ufig abzukl\u00e4rendes Leitsymptom in der Notaufnahme. Wichtig ist die Abgrenzung zu einem Trauma oder eine kardiale Ursache, trifft beides nicht zu, liegt ein \u201ecoma of unknown etiology\u201c (CUE)<\/strong> vor. Begriffsdefinition:<\/strong><\/p>\n

  • Koma ist ein unspezifisches Leitsymptom und keine Diagnose<\/li>\n
  • eine rasche und vollst\u00e4ndige diagnostische Abkl\u00e4rung ist erforderlich ist, um eine zielgerichtete und h\u00e4ufig lebensrettende Therapie initiiert zu k\u00f6nnen<\/li>\n
  • Koma ist mit einer hohen durchschnittlichen Letalit\u00e4t assoziiert, die h\u00f6her ist als f\u00fcr Polytrauma, Herzinfarkt oder Schlaganfall (25\u201350% in verschiedenen Studien)<\/li>\n<\/ul>\n

    H\u00e4ufigkeit:<\/strong><\/p>\n

      \n
    • 5-9 % aller Notaufnahmepatienten weisen eine quantitative Bewusstseinsst\u00f6rung auf<\/li>\n
    • bis zu 2% der Patienten sind bei der Ankunft in der Notaufnahme komat\u00f6s, bei erhaltener Herz-Kreislauf-Funktion<\/li>\n
    • im Krankenhaus der Maximalversorgung besteht ein Verh\u00e4ltnis von 4:1 Schlaganfall: Koma<\/li>\n<\/ul>\n

        • intrakranielle Blutungen 21%<\/li>\n
        • Isch\u00e4mie 10%<\/li>\n
        • psychiatrische Ursache 3% (psychogenes \u201ePseudokoma\u201c)<\/li>\n
        • Inflammation 2%<\/li>\n
        • Raumfordernder Tumor 2%<\/li>\n
        • neurodegenerativ 1%<\/li>\n
        • andere 1%<\/li>\n<\/ul>\n<\/li>\n
        • sonstige konservative Erkrankungen
          \n(sekund\u00e4re ZNS-Affektionen): >30 % , Mortalit\u00e4t bis 15%<\/p>\n

          • Intoxikationen 18%<\/li>\n
          • metabolische\/hom\u00f6ostatische Ursache 6%<\/li>\n
          • kardinale\/pulmonale Ursache 6%<\/li>\n

            • Auffindeort und Verlauf der Bewusstseinsst\u00f6rung<\/li>\n
            • Fremdinformationen abfragen und Rettungsdienst befragen<\/li>\n<\/ul>\n<\/li>\n
            • Notarztdiensgnosen bei Koma sind unzuverl\u00e4ssig (Diagnosekorrektheit rund 50%), daher sollte Koma unabh\u00e4ngig von der Verdachtsdiagnose in einer Klinik der Maximalversorgung versorgt werden<\/li>\n<\/ul>\n
