Vasopressoren bei Herzstillstand:\u00a0<\/strong><\/p>\n

  • Placebo-kontrollierte Studien zeigten, dass Adrenalin beim pr\u00e4hospitalem Herzkreislaufstillstand die ROSC-Rate,\u00a0das \u00dcberleben bis zur Entlassung und das \u00dcberleben nach 3 Monaten erh\u00f6ht, es\u00a0zeigte sich aber keinen Anstieg der \u00dcberlebensrate bis zur Entlassung mit gutem neurologischen Behandlungsergebnis.<\/li>\n
  • Auch in gr\u00f6sseren und neueren Studien (8000 Patienten) wurde kein Unterschied in der \u00dcberlebenszeit bei g\u00fcnstigem oder ung\u00fcnstigem neurologischen Behandlungsergebnis nach 3 Monaten festgestellt; daher ist der Einfluss der Adrenalingabe auf das neurologische Behandlungsergebnis bei Patienten bei\u00a0pr\u00e4hospitalem Herzkreislaufstillstand unsicher.<\/li>\n
  • Die ALS Task Force<\/strong> empfiehlt bei einem Herzstillstand mit nicht-schockbarem Rhythmus so schnell wie m\u00f6glich Adrenalin zu verabreichen, es sei denn, es gibt eine eindeutig reversible Ursache, die rasch behoben werden kann. Der optimale Zeitpunkt f\u00fcr die Verabreichung von Adrenalin bei Patienten mit schockbaren Rhythmen ist unbekannt. Die ALS Task Force<\/strong> schl\u00e4gt vor, Adrenalin zu verabreichen, nachdem die ersten Defibrillationsversuche erfolglos geblieben sind. \u00a0Das optimale Timing oder die Anzahl der Schocks, nach denen Adrenalin verabreicht werden sollte, bleibt jedoch unklar.<\/li>\n
  • Die ILCOR ALS Task Force 2019<\/strong>\u00a0spricht f\u00fcr die pr\u00e4hospitale und innenklinische Situation die gleichen Empfehlungen aus.<\/li>\n
  • Die Anwendung von Vasopressin verbessert weder allein noch in Kombination mit Adrenalin das Behandlungsergebnis; daher wird zur Verringerung der Komplexit\u00e4t Adrenalin allein vorgeschlagen.<\/li>\n<\/ul>\n

    • HYPERION<\/strong> zeigte, dass bei pr\u00e4hospitalem und innerklinischem Herkreislaufstillstand bei komat\u00f6sen Patienten mit initialem nicht-schockbaren Rhythmus an Tag 90 ein neurologisch gutes \u00dcberleben mit 10,2% in der 33\u00b0C-Gruppe vs. 5,7% in der Normothermiegruppe (Risk Difference, 4,5%; 95% CI, 0,1-\u00018,9; p=0,04) vorlag.<\/li>\n
    • Die ALS Task Force<\/strong> entschied sich jedoch daf\u00fcr, die Aktualisierung der Empfehlung zu verschieben bis Ergebnisse zum TTM-2 (Targeted Hypothermia Versus Targeted Normothermia After Out-of-Hospital Cardiac Arrest) RCT vorliegen.<\/li>\n<\/ul>\n
