Hierbei gibt es verschiedene Akronyme und Schemata, die eine strukturierte \u00dcbergabe erm\u00f6glichen, Informationsverlust vermeiden sollen und ein einheitliches Vorgehen bei der Patienten\u00fcbergabe an einer Schnittstelle (Rettungsdienst \u2013 Klinik oder innerklinische, z.B. ZNA \u2013 ICU oder OP \u2013 ICU) erleichtern.<\/p>\n<p>In einer aktuellen \u00dcbersicht sind verschiedene Konzepte zusammengefasst:<\/p>\n<blockquote>\n<p style=\"text-align: justify;\"><strong>Rossi R. Konzepte f\u00fcr eine strukturierte Patienten\u00fcbergabe. <\/strong><strong>Notfall Rettungsmed 2020;\u00a0<\/strong><strong>23:93\u201398. https:\/\/doi.org\/10.1007\/s10049-019-0599-8<\/strong><\/p>\n<\/blockquote>\n<p style=\"text-align: justify;\">Erkenntnisse \u00fcber den Nutzen strukturierter \u00dcbergaben in der Patientenversorgung werden vorgestellt, insbesondere um eine optimale Patientensicherheit gew\u00e4hrleisten zu k\u00f6nnen. Zahlreiche, behandlungsrelevante Informationen k\u00f6nnen insbesondere in Notfallsituationen, bei h\u00e4ufigen Personalwechseln und bei unstrukturierten \u00dcbergaben verloren gehen und damit die weitere Versorgung des Patienten gef\u00e4hrden. Nicht zuletzt hat die WHO die Standardisierung des \u00dcbergabeprozesses als wichtigen Beitrag zur Erh\u00f6hung der Patientensicherheit empfohlen.<\/p>\n<p>Der Autor stellt im Folgenden verschiedene, international etablierte \u00dcbergabekonzepte vor:<\/p>\n<ul>\n<li style=\"text-align: justify;\"><strong>ASHICE<\/strong> (<strong>A<\/strong>ge, <strong>S<\/strong>ex, <strong>H<\/strong>istory, <strong>I<\/strong>njuries\/<strong>I<\/strong>llness, <strong>C<\/strong>onditions, <strong>E<\/strong>vents):<br \/>\nzur telefonischen Voranmeldung genutztes Akronym aus UK\/Irland<\/li>\n<li style=\"text-align: justify;\"><strong>SOAP<\/strong> (<strong>S<\/strong>ubjective [Anamnese], <strong>O<\/strong>bjective [Befunde], <strong>A<\/strong>ssessment [Analyse und Zusammenfassung], <strong>P<\/strong>lan [empfohlene Ma\u00dfnahmen]:<br \/>\neinfaches \u00dcbergabeschema f\u00fcr alle pr\u00e4klinschen\/klinischen Bereiche<\/li>\n<li style=\"text-align: justify;\"><strong>I-PASS<\/strong> (<strong>I<\/strong>ntroduction [Vorstellung Funktion\/Person], <strong>P<\/strong>atient [relevante Daten], <strong>A<\/strong>ssessment [Befunde, Anamnese], <strong>S<\/strong>ituation [Situation vor Ort, Versorgung], <strong>S<\/strong>afety [sicherheitsrelevante Zusatzinformationen]:\u00a0Universelles \u00dcbergabetool, erweiterte Version \u201eI-PASS the Baton\u201c<\/li>\n<li style=\"text-align: justify;\"><strong>IMIST <\/strong>(<strong>I<\/strong>dentifikation [Patient], <strong>M<\/strong>echanismus [Verletzung bzw. Standardisierte \u00dcbergaben k\u00f6nnen die Patientensicherheit nachweislich erh\u00f6hen und sind f\u00fcr den Informationsfluss essentiell.<\/p>\n<p style=\"text-align: justify;\">Welches Tool angewandt wird, h\u00e4ngt von lokalen Gegebenheiten ab und sollte jeder Rettungsdienstbereich\/jede Klinik ein passendes Schema \u00fcbernehmen.
