{"id":10509,"date":"2019-11-23T04:41:25","date_gmt":"2019-11-23T03:41:25","guid":{"rendered":"http:\/\/news-papers.eu\/?p=10509"},"modified":"2019-11-09T22:13:29","modified_gmt":"2019-11-09T21:13:29","slug":"10509","status":"publish","type":"post","link":"http:\/\/news-papers.eu\/?p=10509","title":{"rendered":"qSOFA, SOFA und Blutlaktat als Pr\u00e4diktor einer ICU-Aufnahme"},"content":{"rendered":"<div class=\"page\" title=\"Page 1\">\n<div class=\"section\">\n<div class=\"layoutArea\">\n<div class=\"column\">\n<p style=\"text-align: justify;\">Studien aus deutschen Notaufnahmen sind noch nicht sehr h\u00e4ufig in der internationalen Literatur zu lesen, jetzt liegt aber eine entsprechende aus einer universit\u00e4ren deutschen Zentralen Notaufnahme zum Thema Sepsis publiziert im <strong>Scandinavian Journal of Trauma, Resuscitation and Emergency Medicine\u00a0<\/strong>vor: Sepsis ist definiert als eine lebensbedrohliche Organdyfunktion aufgrund einer dysregulierten Inflammation infolge einer Infektion. Aber welche Vorhersagekraft haben die mittlerweile etablierten Scoresysteme qSOFA, SOFA und das Blutlaktat ?<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<blockquote>\n<p data-test=\"article-title\"><strong>Kimpel J et al.\u00a0The impact of the Sepsis-3 definition on ICU admission of patients with infection. Scandinavian Journal of Trauma, Resuscitation and Emergency Medicine (2019) 27:98 (<a href=\"https:\/\/sjtrem.biomedcentral.com\/track\/pdf\/10.1186\/s13049-019-0680-9\" target=\"_blank\" rel=\"noopener noreferrer\">PDF<\/a>)<\/strong><\/p>\n<\/blockquote>\n<p><strong>Methodik: <\/strong><\/p>\n<ul>\n<li style=\"text-align: justify;\">prospektive Observationalstudie \u00fcber 12 Monate mit konsekutiv allen \u00fcber eine Zentrale Notaufnahme (ZNA, Universit\u00e4tsklinikum Leipzig) aufgrund einer Infektion aufgenommenen Patienten<\/li>\n<li style=\"text-align: justify;\">evaluiert wurde der pr\u00e4diktive Wert von qSOFA, SOFA und des Laktatwertes f\u00fcr eine Intensivstations(ICU-)aufnahme (Interdisziplin\u00e4re Internistische Intensivstation, Universit\u00e4tsklinikum Leipzig)<\/li>\n<\/ul>\n<p><strong>Ergebnisse:<\/strong><\/p>\n<ul>\n<li>916 Patienten wurden aus der ZNA mit einer Infektion aufgenommen<\/li>\n<li>Medianes Alter: 74 Jahre (IQR 62\u201382 )<\/li>\n<li>56% m\u00e4nnlich<\/li>\n<li>219 der Patienten wurden direkt auf die ICU aufgenommen und 697 auf eine Normalstation.\n<ul>\n<li>qSOFA \u22652 bei ICU vs. Normalstation: 40% vs 13%, p&lt;0,01<\/li>\n<li>Infektionsursache\u00a0ICU vs. Normalstation: Respiratorisch 72 vs. 52%, Urogenital 15 vs. 28%, Abdominal 5 vs. 9%, Weichteilgewebe: 4 vs. 6%, sonstige 6 vs. 5%<\/li>\n<\/ul>\n<\/li>\n<li>qSOFA \u22652 wies eine Sensitivit\u00e4t von 53% und Spezifit\u00e4t von 98% f\u00fcr die Diagnose Sepsis auf<\/li>\n<li>qSOFA \u22652 wies eine Sensitivit\u00e4t von 87% aber nur eine\u00a0Spezifit\u00e4t von 40% zur Vorhersage einer ICU Aufnahme auf.<\/li>\n<li>SOFA \u22652 wies eine Sensitivit\u00e4t von 97% aber nur eine\u00a0Spezifit\u00e4t von 17% zur Vorhersage einer ICU Aufnahme auf,<\/li>\n<li>w\u00e4hrend ein SOFA \u22654 eine Vorhersage f\u00fcr eine ICU Aufnahme von 83% und Spezifit\u00e4t von 72% hatte.<\/li>\n<\/ul>\n<p><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" data-attachment-id=\"10513\" data-permalink=\"http:\/\/news-papers.eu\/?attachment_id=10513\" data-orig-file=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?fit=758%2C572\" data-orig-size=\"758,572\" data-comments-opened=\"1\" data-image-meta=\"{&quot;aperture&quot;:&quot;0&quot;,&quot;credit&quot;:&quot;&quot;,&quot;camera&quot;:&quot;&quot;,&quot;caption&quot;:&quot;&quot;,&quot;created_timestamp&quot;:&quot;0&quot;,&quot;copyright&quot;:&quot;&quot;,&quot;focal_length&quot;:&quot;0&quot;,&quot;iso&quot;:&quot;0&quot;,&quot;shutter_speed&quot;:&quot;0&quot;,&quot;title&quot;:&quot;&quot;,&quot;orientation&quot;:&quot;0&quot;}\" data-image-title=\"Bildschirmfoto 2019-11-09 um 22.06.09\" data-image-description=\"\" data-image-caption=\"\" data-medium-file=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?fit=300%2C226\" data-large-file=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?fit=758%2C572\" class=\" wp-image-10513 aligncenter\" src=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?resize=506%2C381\" alt=\"\" width=\"506\" height=\"381\" srcset=\"https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?resize=300%2C226 300w, https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?resize=750%2C566 750w, https:\/\/i0.wp.com\/news-papers.eu\/wp-content\/uploads\/2019\/11\/Bildschirmfoto-2019-11-09-um-22.06.09.png?w=758 758w\" sizes=\"auto, (max-width: 506px) 100vw, 506px\" \/><\/p>\n<ul>\n<li>SOFA wies eine receiver operating curve f\u00fcr ICU Aufnahme von 0,81 (95 CI, 0,77\u20130,86)<\/li>\n<li>Blutlakatatwert &gt;2 mmol\/l wies eine receiver operating curve f\u00fcr ICU Aufnahme von 0,55 (95% CI, 0,48\u20130,61) und<\/li>\n<li>qSOFA\u00a0wies eine receiver operating curve f\u00fcr ICU Aufnahme von\u00a00,34 (95% CI, 0,28\u20130,40) auf<\/li>\n<li>nur <strong>18 der 697<\/strong> auf eine Normalstation aufgenommenen Patienten wurden binnen 48 h auf eine Intensivstation aufgenommen (<strong>2,6%<\/strong>), von diesen hatten nur 5 Patienten einen positiven qSOFA bei Aufnahme in der Notaufnahme.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Schlussfolgerung:\u00a0<\/strong>W\u00e4hrend ein positiver qSOFA eine hohe Spezifit\u00e4t f\u00fcr eine ICU Aufnahme hatte, zeigte die niedrige Sensitivit\u00e4t Limitation f\u00fcr die routinem\u00e4ssige Nutzung bei septischen Patienten f\u00fcr die Vorhersage einer ICU Aufnahme an.
